Yup. They can't pin it on poultry so they're looking for some other bird....

No mention of the possibility that he could've gotten H5N1 from cats or some other mammal (including humans), of course.

Any follow up studies on patients who "survived" or tested negative after Tamiflu treatment would be beneficial to help us understand if there are milder forms of H5N1.

Bird flu claims 91st victim in Indonesia

Jakarta (dpa) - A 31-year-old Indonesian man has died of bird flu, raising the country's death toll from the H5N1 avian influenza virus to 91 - the world's highest, a Health Ministry official said Saturday.

The man, identified by his initials MN, originally from Bengkalis district of Riau province in Sumatra island, died Tuesday just as he was admitted to Pekanbaru's Arifin Achmad Hospital, said Dasir Nurdin, an official at the Health Ministry's bird-flu information centre.

Nurdin said that the man fell sick on October 31 and sought medical treatment at a health clinic nearby on November 3 before being admitted three days later to the provincial designated hospital for bird flu.

"The man died on his arrival in the hospital," Dasir Nurdin said.

The Health Ministry's provincial team is still investigating the history of the man's illness.

His death was Indonesia's 91st out of 113 diagnosed cases of bird flu in humans. Both figures are the highest in the world. Not counting Indonesia's latest fatality, at least 204 people have died in 12 countries in Asia and Africa of the disease, according to World Health Organization statistics.
The most common way to contract the H5N1 avian influenza virus is through contact with sick fowl. Although bird flu remains mainly an animal disease, experts fear the virus could mutate and spread from human to human, turning it into a pandemic that could kill millions of people worldwide.

A quick note about this couple -- keep in mind that just because they are married, that doesn't mean they aren't blood relatives. In fact, there's good chances that, being from Sumatra, they are related.

Cousin-marriage is quite common in Indonesia -- and very common in Sumatra, I think -- so spouses are very often blood relatives. Back during the Karo cluster I found info that the Karo peoples (the Gintings, for instance) regularly marry their 1st or 2nd cousins. Don't know what we're dealing with in this case, but it's quite likely that this couple are (were) related, too:

http://www.google.com/search?hl=en&q=riau+cousin+marriage&btnG=Search

However, none of this necessarily negates the possibility that this could be an h2h bf cluster -- not in my book, anyway!

The Ministry of Health of Indonesia has announced a new case of human
infection of H5N1 avian influenza (AI). A 31-year-old male from the
Bengkalis District, Riau Province developed symptoms on 31 Oct
[2007], was hospitalized on 3 Nov [2007], and died in an AI referral
hospital on 6 Nov [2007].

Identification of the source of his infection is ongoing and includes
investigation into a large swallow farm in close proximity to the
case's house.

Of the 113 cases confirmed to date in Indonesia, 91 have been fatal.

Communicated by:
ProMED-mail Rapporteur Marianne Hopp

[This appears to be the same fatal case of avian influenza virus
infection reported in ProMED-mail archive 20071111.3656. However, the
current report (above) is the 1st that explicitly mentions close
proximity to a swallow farm. The birds are undoubtedly bird nest soup
swiftlets (_Collocalia fuciphaga_, although its specific and
subspecific taxonomic status is debated), raised for their very
expensive nests. Indonesia exports 27 tonnes of nests annually, which
indicates the magnitude of this industry. The status of the swiftlets
as H5N1 virus hosts is unknown and the results of the investigation
will be of interest. Images of these birds and their nests can be
seen at
