GENEVA, May 24 ? A 32-year-old Indonesian man who died Tuesday of avian influenza may have contracted the disease while caring for his 10-year-old son, who died earlier, according to World Health Organization experts.

"The father was closely involved in caring for his son, and this contact is considered a possible source of infection," the agency said in a statement.

The case may be the first in which the virus made a double jump, involving three people. In all earlier cases of suspected human-to-human transmission, the virus passed from an infected person to another after close and prolonged contact.

Most cases of human infection arise from close contact with infected birds, but experts fear small mutations in the virus could allow it to jump easily from human to human, which could spark a world-wide pandemic. The Indonesian cases are being watched closely because of fears that the virus there may have developed such an ability.

WHO scientists said it is clear the virus jumped from person to person in the Indonesian cluster, but only after prolonged exposure to an infected person who was coughing up infected particles.

The cases are part of a family cluster in the village of Kubu Sembelang in the north of the island of Sumatra.

Seven members of the family became infected after a family gathering on April 29 and six have died. The hostess of the gathering, who died May 4 of respiratory illness, was probably also a victim of the H5N1 strain of avian flu, but she was buried without specimens being taken so officials aren't confirming the flu possibility.

However, because the clinical course of her disease is "compatible with H5N1 infection," the agency said, officials are considering the 37-year-old woman to be the index case.

WHO officials noted that three of the infected family members slept in the same room as the index case, who was coughing heavily at the time. Those cases include the woman's two sons and a brother, who is the only surviving victim of the outbreak.

The WHO working hypothesis, in the case of the most recent death, is that the virus jumped twice between people?once from the index case to the boy, and then from the boy to his father.

But according to published reports, that's based on the assumption that the incubation time for the H5N1 strain is longer than the two to four days seen in regular human influenza.

If the assumption is wrong, that would imply the boy?who died May 13, two weeks after the family gathering?was infected by someone other than the index case. That would mean the virus made three human-to-human jumps, according to Henry Niman, Ph.D., of recombinomics.com, a company that studies the genetics of viral evolution. Dr. Niman was quoted by the New York Times.

So far, the WHO said, there's no evidence of "efficient human-to-human transmission" and no sign that the outbreak is spreading into the local community.

The H5N1 strain of avian flu is highly dangerous when it infects humans. As of Wednesday, WHO said, 218 people have been infected and 124 have died.

The H5N1 virus is known to prefer to attack receptors deep in human lungs, rather than those in the nose and throat, like the standard seasonal flu. That biological difference is regarded as the reason the virus is only transmitted through close contact with infected birds or other infected humans.

GENEVA - A family of eight infected with bird flu in Indonesia likely passed the disease among themselves, but world health officials said Wednesday there is no reason to raise its pandemic alert level.

It is the fourth ? and largest ? family cluster of bird flu cases likely transmitted from person to person since the start of the outbreak in Hong Kong in 2003, World Health Organization spokesman Gregory Hartl said.
But this case may mark the first time bird flu has passed from person to person to person, a top U.S. health official said.
The previous clusters all involved someone who was infected by a sick bird and then spread the virus to others. This new cluster appears to involve a cascade of transmission, said Dr. Julie Gerberding, director of the U.S. Centers for Disease Control and Prevention, in a telephone briefing from Geneva.
The family members' close physical proximity is probably responsible for the spread of the disease, Hartl said.
"It fits the kind of pattern perfectly which we've seen so far," Hartl told The Associated Press. Global and U.S. health officials say tests on virus samples taken from the family do not indicate any significant changes.
Investigators say the family, living in the remote northern Sumatra village of Kubu Sembelang, were infected with a strain of H5N1 bird flu that was genetically the same as the virus found circulating in the area earlier. Tests are still being carried out on poultry there.
Infections have not been detected in health-care workers or others in the village, but those who may have come in contact with the family are being given Tamiflu as a precaution, Gerberding said.
WHO has suspected that in rare cases bird flu may have passed from one person to another, although people usually catch it from chickens and other poultry.
Experts have long believed the virus is spread when people breathe it in ? possibly in dust from bird droppings or in droplets sneezed or coughed by humans into the air.
But it remains unclear exactly how the virus spreads in family groups ? whether through respiratory systems, food, infected surfaces or a combination of these, Hartl said.
"Which transmission mode is most important, we really don't know yet," he said. "When you get all of these things together, it becomes perhaps more likely."
Other experts have suggested family members have a genetic weakness to the disease. In all four family clusters recorded so far, only direct blood relatives ? not spouses ? have caught bird flu.
WHO will leave its pandemic alert level unchanged at 3, where it has been for months, meaning there is "no or very limited human-to-human transmission."
Six of the seven family members who caught bird flu have died, the most recent on Monday. An eighth family member who died was buried before tests could be done, but she was considered to be among those infected with bird flu.
"... It apparently can only spread between human to human when there is extensive and close contact between someone who is already showing clinical signs of the disease and the uninfected person," Hartl said.
In the three previous family clusters ? in Indonesia, Thailand and Vietnam ? the number of infected relatives was much smaller, Hartl said. CDC officials noted that the evidence for human transmission was stronger in the Indonesia and Thailand clusters, but human transmission could not be ruled out in the Vietnam cluster.
Health workers have found no sign the latest Indonesian case has moved outside the family, and there is also "no evidence that efficient human-to-human transmission has occurred," WHO said in a statement.
Still the size of the cluster and the failure to determine the source of the infections was worrying, Peter Cordingley, spokesman for WHO's Western Pacific region. Bird flu has killed 124 people worldwide since the virus started ravaging poultry stocks across Asia in 2003.

What reasoning exists for WHO to conclude as they do? How can they ignore these facts? How can the world's people and countries tolerate inaccurate conclusions from the WHO? Is there no punishment for their incapacity to state facts accurately? Do they not know the incubation time for Indonesian flu, after nearly a hundred people there have sickened and died?

WHO assumes that the incubation time for bird flu in humans is 7 to 10 days, longer than that of regular flu, she said.

Henry Niman, who runs recombinomics.com, a Web site tracking the genetics of flu cases, argues that the incubation period is closer to the two to four days of regular flu, so the boy may have been infected by another family member, meaning that the virus may have made three consecutive human-to- human jumps.

But Cheng said the health agency's "working hypothesis" was still that it had jumped only twice.

The WHO working hypothesis on a limited transmission chain is similar to its investigations of earlier clusters, which focused on common sources other than humans, leading to a gross underestimate of the number of cases of human-to-human transmission (H2H) and length of transmission chains. The clusters in Turkey and Azerbaijan also involved long transmission chains, but the WHO used tortured arguments to eliminate H2H, which created faulty data, which then created additional faulty data such as the 7 to 10 day incubation time.

The two clusters that WHO linked to duck blood pudding are good examples. None of the H5N1 positive patients had the meal within the common 2-4 day incubation period. In the cluster in Hanoi in 2004/2005, the time between the meal and symptoms was 1 and 17 days. A WHO committee review of H5N1 in the New England Journal of Medicine stated "The case-to-case intervals in household clusters have generally been 2 to 5 days, but the upper limit has been 8 to 17 days, possibly owing to unrecognized exposure to infected animals or environmental sources". However, this upper limit of 17 days is due to WHO's efforts to link the brother to the meal, instead of the index case. The brother developed symptoms when the index case died, which is a common feature of the familial clusters that involve H2H. Moreover, the use of 7-10 days as indicated above, reduces the number of links in the transmission chain.

The shorter incubation time of 2-5 days also explains the 5-10 day gap between the onset date for the index case and other family members. H5N1 is still not efficiently transmitted H2H, so transmission peaks several days after disease onset, which when coupled to the incubation time, creates the 5-10 day gap between the index case and other family members, which was common in the first 15 clusters which were documented in an H5N1 familial cluster paper by WHO and the CDC.

These shorter incubation periods not only support H2H in the smaller cluster of 2 or 3, but also indicate that the clusters that have long intervals between the first and last member involve a number of transmission. These long intervals were clearly present in the larger clusters in Turkey and Azerbaijan.

However, the current cluster in north Sumatra is the deadliest. Only one of the eight members has survived, raising concerns in the changes in the eight H5N1 gene segments. Those sequences have been sequestered at a private WHO database. Two sequences from the first confirmed case in Indonesia were released, but the sequences of the other six gene segments, as well as full sequences from the other H5N1 cases in Indonesia should be release immediately.

At least two distinct H5N1 sequences are co-circulating in Indonesia, creating conditions fro dual infections and recombination. Therefore the sequences should be available to the scientific community for detailed analysis.

Thank you. The WHO just makes it up as it goes. The latest is a long incubation time coupled with genetic predisposition, which is also nonsense because the cluster includes a husband and wife.

Why would the press remain so insistent in the face of these facts to continue to espouse the WHO line?

WHO has a political agenda, it appears, rather than an agenda which demands revelation of the scientific facts when known, especially when their political agenda is at risk.
