In three successive days, from Friday (9/6) up to Sunday (11/6), three children from one family died. Expected, because in the surrounding environment the death of the poultry especially the chicken, had happened they died because of AI.

"But we were still continuing to prove the cause of the death of three children. Because of this death his characteristics cluster or mengelompok with the story of the death of the poultry around it," said Ayla Kayrus, the Section Head the Prevention and Observation of the Lampung Illness of the Health Service that was contacted per the telephone, on Tuesday (13/6).

"Because the twin died in the house, the team survailance from the Community Health Centre and the Health Service of Tanggamus could not take the example of blood both of them. Now, the example of Masyani blood could be taken," said Ayla.

The head of the Medical RSAM Service Field, Arief Effendi, that was contacted was separated said according to parents's story of the three casualties, in the environment around the house the death of the poultry could happen twice, to be precise around two months and half of the previous month. However the hospital side still could not confirm the three of them died because of bird flu. Results of the sample of Masyani blood in fact showed the existence of the respiratory tract infection, not bird flu.

With the occurrence of this case, the Lampung Health Service continued to carry out the monitoring and to remind the community around to guard the cleanliness of the environment. "Today the service of province livestock breeding sprays the location of the case and carries out investigation," he said.

The third sibling was hospitalized in Bandar Lampung which is the capital of Lampung province in southern Sumatra. Presumably the village that the family is from is somewhere in the area. The Karo cluster was in northern Sumatra -- the Ginting family were hospitalized in Medan (which you can see on the map).

Here is an earlier cluster in Lampung

The third family member who tested negative and was said to have typhus in the cluster from last year, was recently confirmed to have H5N1 antibodies and therefore all three family members are WHO confirmed.

The confirmation of the third case was reported Feb 6 (4 months after initial reports)

The above translations describes the death of three suspected H5N1 bird flu cases. Two of the siblings died at home, so no samples were collected, The third sibling died shortly after admittance and a serum sample is being tested.

Birds are reported to have died in the area, and last year there was a three member familial cluster that was H5N1 confirmed (although confirmation of the third case was via sero-conversion and was not reported until 4 months after initial reports). The cluster had a characteristic time gap between disease onset dates, indicating human to human transmission. The three family members survived.

The latest cluster is more typical of clusters in Indonesia, which have a very high case fatality rate. Surveillance in Indonesia is poor and failure to collect samples in index cases is common.

JAKARTA, July 3 (Reuters) - A World Health Organisation laboratory test has confirmed a 5-year-old Indonesian boy who died last month was infected with bird flu, a health ministry official said on Monday.
His death takes the total number of confirmed bird flu fatalities in the country to 40.
The victim died on June 16 in Tulungagung in East Java province after being admitted to hospital on June 8, I Nyoman Kandun, director general for communicable disease control at the health ministry, told Reuters.
The infection was confirmed to be from the H5N1 avian virus by a WHO laboratory in Hong Kong, he said.
An official at the health ministry's bird flu centre who declined to be identified said: "There was a dead chicken near his house."

The chicken cage was 15 metres (49 ft) from the boy's home, the official added.
Indonesia has seen a steady rise in human bird flu infections and deaths since its first known outbreak of H5N1 in poultry in late 2003, and has registered more deaths this year than any other country.
Indonesia has 220 million people and an estimated 1.2 billion chickens, some 30 percent of them in the yards of homes in both rural and urban areas.

The bird flu virus is endemic in poultry in nearly all of the 33 provinces in Indonesia, a country of 17,000 islands sprawling across some 5,000 km (3,100 miles).

Despite the climbing death toll, the government has resisted mass culling of birds, saying it is too costly and impractical.

Vaccination has been preferred to culling, which has been done only sporadically at selective farms and their immediate surroundings.

Bird flu remains essentially an animal disease but many countries around the world are on alert over fears it may mutate into a disease that could pass easily among people and trigger a pandemic, killing millions.
Indonesia drew international attention in May when the virus killed members of a single family in North Sumatra. Experts said there could have been limited human-to-human transmission in this cluster case.
But they stressed genetic analyses of the virus have not shown all of the traits that are known so far to allow it to spread easily among people.

Re: Indonesia - Three siblings dead / bf suspected

There was an isolated report of two brothers with bird flu in Tulungagung at the same time, but their ages were 7 and 11 months, so it's not clear if this 5 year-old was even ever reported. The younger one, Aditya, died, though we don't know if he was ever tested, and the older one, Rizal, apparently recovered and tested negative. Anyway, more importantly, is this 5 year old related to these two boys?
