Suspect H5N1 Bekasi Fatality In Indonesia
Recombinomics Commentary 18:36
March 9, 2009

An eight-year-old kid who was suspected to be positive disease have a positive bird flu, finally exhale the last breath.

Andriansyah (8), fourth child of the couple and Mimin Ilyas, the village of Kandang, Sukakarya Village, Kecamatan Karang Bahagia, Kabupaten Bekasi died in this Friendship Hospital, East Jakarta, Sabtu (28/2/2009).

"My child has been sick since a week ago.

The above comments describe the recent fatal suspect H5N1 case (8M) in Bekasi (see updated map). One confirmed case was described in the latest WHO update on Indonesia. That case developed symptoms last year and died Januray 2. A second confirmed case was acknowledged by the MOH announcement last week and was subsequently acknowledged in local media.

However, most of the cases have died recently, and although at least one was said to have been lab confirmed, none have been officially classified as confirmed cases. WHO updates have lagged and have not confirmed the cases recently announced by the MOH. WHO updates on Indonesia and have only described two cases which developed symptoms last year.

Only one of the suspect cases has survived, but remains hospitalized. This clustering of cases in Bekasi this year has raised concerns that transmission to humans has become more efficient and exposure to the sub-strain commonly isolates from human cases has increased. Initially most poultry isolates did not mach human H5N1, which had a novel cleavage site. However, more closely related poultry isolates began to appear in 2007, which may be higher today. Phylogenetic analysis of the H5N1 isolated from patients in 2008 indicates those recent human cases have retained the novel cleavage site.

The continuing increase in fatal Bekasi cases, coupled with the lack of WHO updates on these cases, continue to increase concerns.

Depok Residents Are Tested for Bird Flu

March 10, 2009

http://news.okezone.com/index....

Depok: The H5N1 virus has increasingly spread in Depok where all the subdistricts are stated as endemic for the spread of the bird flu virus.

That was marked by the number of chickens that suddenly died in six Depok subdistricts. These chickens died suddenly resulting from being positively attacked by the avian influenza virus. This was in accordance with results of the rapid tests that were carried out against these chickens. The Section Head the Health of the Depok City Agriculture, Animal and Veterinary Agency, Arifin, said that at this time the deaths of poultry resulting from the bird flu virus has reached 300 tails. "Finally there were 23 chickens that died suddenly in the Sukamaju District, Sukmajaya, Depok," he said, on Tuesday (10/3/2009).

Arifin added that the residents in and around the environment where there were cases of chicken dying suddenly have had their blood samples take to be tested in the Laboratory in IPB Bogor.

"We took blood samples of the residents and the chickens, moreover we continued spraying disinfectant in the chicken coops. Basically for us the officials are always on the alert," he said.

In the meantime the Depok City Veterinary, Agriculture and Animal Service surgeon, Alfian said that many communities did not yet understand how to handle poultry that died suddenly as a result of bird flu.

"Moreover there were residents who threw these chicken carcasses into the river, in a way that precisely makes the virus H5N1 increasingly spread," he explained.

Furthermore, said Alfian, the characteristics of the chickens that died suddenly as a result of the bird flu virus was blue and the saliva had phlegm.

"The main thing that is necessary is that residents reported with a maximum of 1x24 the hour if they find cases of chickens suddenly dying to the Agriculture Service," he appealed.

Previously it was suspected that a bird flu case claimed one fatality in the Pancoranmas Subdistrict, Depok. The Depok City Agriculture officials also continued to carry out the socialization in cooperation with students from the Indonesian University to give counselling to the community in order to interrupt the spread of the bird flu virus.

Re: Indonesia: The Latest Human Suspected & Confirmed Cases January 29 +

There seems to be a lot of confusion surrounding the names, ages, and dates of onset and death for recent cases in Indonesia. Indonesian officials have failed to clarify the confusion surrounding information on these suspected and "locally confirmed" cases. Reviewing the translations in this thread suggest the possible presence of a family cluster in South Jakarta.

From post #259 on March 4th, we learn that 3 year old Isnaelia Rahmadiyanti from RT11/02 Petukangan South, Pesanggarahan, Southern Jakarta, died from a suspect bird flu infection on February 2.

04/03/2009
Pesanggrahan most Serious Bird Flu

Jakarta - the Pesanggrahan Subdistrict, Southern Jakarta, was sentenced as the territory that was most serious towards bird flu. From 11 cases of bird flu since 2005 to 2009, 5 among them happened in Pesanggarahan.

"From 10 subdistricts in South Jakarta, Pesanggrahan most serious bird flu because the area" of "outskirts and most communities maintained the poultry," said Kasudin Peternakan Municipal Government Southern Jakarta Chaidir Taufik, to the reporter in his office, Street Prapanca Raya, South Jakarta, on Wednesday (4/3/2009).

According to the Municipal Government note, the case of last bird flu struck Isnaelia Rahmadiyanti, the child was 3 years old the resident of the Jalan DamaiComplex PDK RT11/02 Petukangan South, Pesanggarahan, Southern Jakarta, last February 2.

"Positively his bird flu was just known on February 13 after being treated in RS Persahabatan," obviously him. It was further that he explained, after the Isnaelia death, totalling 170 poultries in the subdistrict already in-swepping. Today swepping the poultry was done in the New Hut Ii RT02/06, Pesanggrahan, Southern Jakarta. According to the resident around, Rasmatin (50), 3 were from his 100 chickens taken by the official. In the place, said he, at least had 200 chickens that were maintained by the resident.

"That was taken by three chickens, but that was cut off only one," he said. In the meantime, parents Isnaelia Rahmadiyanti apparently still the trauma on the death of his only child. He permitted his child to die on February 2.

The "death one hour after being treated in RS Persahabatan." Beforehand my child could be treated in RS Aminah, said casualties's father Tri Santono (27) when being met in his house.

http://www.detiknews.com/read/2009/...rev=_t&usg=ALkJrhg4KA5pI36LDKOuQOGmWPHfqdi4ZA

We also learn from post #317 on March 7 that Simadiah Rahmayanti (3), also a resident RT 011/02 the Pesanggrahan District, the Pesanggrahan Subdistrict, Southern Jakarta, died on Monday March 2 and that the government took fast measures to immediately destroy poultry in the area.

The poultry in Pesanggrahan was destroyed

04-03-2009

The death of the patient suspect bird flu from Pesanggrahan, Southern Jakarta, made the Government of the City of Administrasi Southern Jakarta (the South Jakarta Municipal Government) immediately took fast measures. So that the similar case is not repeated, the South Jakarta Municipal Government direct men-sweeping the poultry that still was maintained by the resident around. Results, eight tails of the poultry succeeded in being destroyed. The South Jakarta Municipal Government gave time till two weeks in the future so that the resident destroys his kept poultry. The patient suspect bird flu that died in the maintenance in the hospital (RS) the Friendship was Simadiah Rahmayanti (3). The resident RT 011/02 the Pesanggrahan District, the Pesanggrahan Subdistrict, Southern Jakarta, this blew out the last breath last Monday (2/3).

Uptil now, to prevent spread him bird flu, the South Jakarta Municipal Government continued to intensify the certification activity and sweeping. ?Kita not robbed, sweeping and certification of the poultry continue to still was carried out by us through the official kelurahan,? Mangara Pardede words, the Secretary of the City (Sekko) South Jakarta to the reporter when leading the activity sweeping the poultry in Street Merpati Raya, RT 01/06, Pesanggrahan, South Jakarta, on Wednesday (4/3).
-snip-
Therefore, he chose Pesanggrahan as the main priority to prevent the spreading of bird flu in Southern Jakarta. In the meantime, since 2009, in the Pesanggrahan Subdistrict has been destroyed totalling 178 tails of the food poultry that consisted of the chicken, the duck, the duck, et cetera. Whereas 144 other tails have gotten the maintenance certificate of the pet poultry. ?Sesuai with No Regional Regulations 4 in 2007 about the Control of the Maintenance and the Circulation of the Poultry, actually the community already was not permitted still maintained the poultry, obviously him. Except, if being maintained to the best of 25 metre from the radius of the settlement of the resident and got permission as well as was supplemented with the certificate. In sweeping this poultry, the official only succeeded in pacifying eight tails of the poultry belonging to the resident who was still wandering in the settlement region. ?Karena heavy rain, we just could eight ekor,? firm Chaidir. These poultries were at once destroyed by the official by means of being cut off. Chaidir also added, gave tolerance for two weeks so that the community could destroy his property poultries. ?Jika was not destroyed also, be forced we will return to the place we will return to this place and would memusnahkannya,? he accused.

Because the two individuals have similar surnames, ages, and are from the same geographic location but died exactly one month apart, they may be a pair of siblings or neighborhood playmates that died from bird flu. Alternatively, there could be some serious news reporting discrepancies and only one young child from Petukangan South, Pesanggarahan, Southern Jakarta, actually died.

It is unfortunate that the Indonesian Government is not willing to clarify information on suspected and confirmed human bird flu infections in Indonesia. In contrast, the Egyptian Government quickly and decisively repudiated an internet rumor regarding the "confirmation" of bird flu in two individuals in Cairo, Egypt. see: http://www.flutrackers.com/forum/showpost.php?p=212686&postcount=62

All of this leads one to ask: Is the failure of the Indonesian Government to respond to and clarify news reports about human bird flu infections a function of deliberate obfuscation or is it a simple case of bureaucratic incompetence?
