Care Room Full of Bird Flu
Yuke and EFFENDI

PADANG - Singgalang suspect bird flu patients in hospital. M. Djamil continues to grow. As a result, a special room at the hospital was full. Yesterday opened a new isolation room. If both full back room, the patient will be sent to hospitals Achmad Muchtar Bukittinggi.
Thursday (10 / 3) into the patient from Baso, Agam. The entry of patients that are elderly that require the hospitals to add capacity room, which previously only for 10 people.
"At first only capacity for 10 people, but by giving us more room service and bed. Lokasinyanya still a block to suspect the patient's room. This is in anticipation, for patient referrals from the region, "said Chief of Medical Services, Hospital. M. Djamil Padang, Yusirwan Joseph, accompanied by PR Gustafianof, to Singgalang.
Patients who entered yesterday rushed to the M. Djamil since suffering a high fever. Previously he had received treatment from medical personnel in the area. Officers recommend that the patient's family refers to the hospital. M. Djamil.
"After considering and weighing it is impossible to deny the patient, disekapati add a new room. But if the room was too full, the patient will be referred to the hospital. Ahmad Muchtar, "said Yusirwan.
Since last week, hospital. M. Djamil have treated nine patients with suspected, consisting of infants, adolescents and adults. The average patient from the city of Padang. One patient from the Calendar( Dharmasraya). Their current condition, much better than at any entrance to the hospital.
Previously, the hospital. M. Djamil Padang ask clinic or general practitioner in their respective areas of origin do not refer patients a high fever. Because the capacity of suspect bird flu patient's room, at government-owned hospitals are not sufficient.
"The capacity of the bed only for patients suspected of eight people, now added two more beds. While that has been filled nine, now left only one bed, "said Chief of Medical Services Hospital. M. Djamil Padang, Yusirwan Joseph.
It is said, there are important things that should be considered health care workers in the area, both in the clinic or general practitioner, ie a person suffering symptoms of bird flu. Such as high fever around 39 degrees celsius, direct contact with poultry, cough, fatigue, sore throat, headache, no appetite, vomiting, abdominal pain, joint pain, diarrhea. Other symptoms of infection of the lining of the eye (conjunctivitis). In a deteriorating situation, there was severe respiratory distress, namely severe shortness of breath, low oxygen levels while carbon dioxide levels rise. This occurs because the flu infection spreads to the lungs and cause inflammation of the lungs (pneumonia).
According to him, if just a high fever with no contact with poultry or their homes are not near the cage birds, do not need to be referred. This is to keep a place for patients who are actually infected with bird flu. At the moment the place no longer exists, the RS. M. Djamil Padang had no other alternatives in doing intensive care for victims of bird flu.
In Sumatra, there are two referral hospitals of patients suspect bird flu, namely RS. M. Djamil and RS Ahmad Muchtar Bukittinggi. In the RS. M. Djamil current average Viru patients suspected of contracting the H5N1 that come from the city of Padang. While the capacity of RS. M. Padang Djamil limited. During the 2011 RS. M. Djamil Padang already treating 14 patients, 10 of whom are still hospitalized.

Broken VCR
RS. M. Djamil as government-owned hospitals in 2009 and then receive help Visual Conversion Reaction (VCR), to examine blood samples of patients suspected of contracting bird flu. But the equipment was damaged by an earthquake 30 September 2009. Until now these tools can not be used.
"We do have a tool to examine blood samples of patients suspected of contracting bird flu. But the device is damaged and can not be used, "explained Yusirwan.

He mentioned, the existence of a tool that can find the results of blood samples from patients early. However, although blood samples have been checked samples remains sent to the Health Research and Development Agency (Research and Development) in Jakarta. The aim is to compare blood samples examined, positive or negative.

Padang must outbreaks
Yesterday in Padang, West Sumatra Deputy Governor Muslim eunuch asked the mayor of Padang to set out common occurrence (KLB) bird flu to Padang. Deadly bird flu that had 2,000 chickens in a number of districts that must be addressed proactively.
"You can not just simply appealed to the appealed again. Must outbreaks. Act of extermination must be made at points indicated bird flu outbreak. Looking at recent developments, outbreaks should be established, "said Deputy Governor told reporters on Thursday (10 / 3) in his room.
Previously Vice collect relevant range, including inviting the Mayor of Padang. Present on the occasion, Sekdako Padang Emzalmi together Pernakhutbun Syahirman Kadis, head of West Sumatra Livestock Edwardi, the ranks of West Sumatra Health Office and the party Dr M Djamil.
According to Deputy Governor, domesticated chicken culling bird flu-affected communities will be recognized difficulties because not all people want. Therefore, Padang replacement policy must take on their pet chickens who destroyed it.
"This replacement can be done, if bird flu breaks out and the area declared outbreaks of bird flu. An area can be defined outbreaks if there are positive cases of bird flu. Padang already have a positive, "said Deputy Governor.
He also emphasized that the determination of this bird flu outbreak, only in birds not in humans. Moreover, given Kadis Pernakhutbun Padang, from 11 districts, 9 districts found cases of bird flu on 21 points.
Syahirman admitted, for the suggested course, people do not respond correctly. Evidence, some time ago, in the Olo Nanggalo, already agreed upon starting RT, RW, village heads, community leaders will destroy the chicken around it. There are at least 100 individuals. But finally given up only 10 fish. Yesterday the pattern of destruction has been proposed, provided 190 million fund. Big cock and replaced Rp15 thousand chicks Rp5.000. For the first disbursement of Padang had outbreaks.
Until Thursday, the number of chickens died 900 tails, while based on information collected 2,000. Could be, because when the data collection had buried his chicken. (*)

Ok - so none of the suspected cases in Indonesia, who have the symptoms of "bird flu", have tested positive for H5N1.

I don't believe it. I think the titer in the test kit does not match the current clade.

How can, literally, dozens of people in Indonesia, who have been in the vicinity of dead H5N1 positive poultry, and who have the symptoms of H5N1 illness, all test negative? 100%? All?

It does not make mathematical sense.

H5N1 kills. A lot. None of the suspected cases have died., Perhaps the authorities are being overly cautious and treating any ILI in the vicinity of H5N1 infected poultry as suspected H5N1. H5N1 is pretty widespread in poultry there, judging by the articles, so many cases of ILI would thus be suspect H5N1.

We have to be very carefully about considering CFR rates for H5N1. Egypt with 127 cases and only 41 deaths has a CFR of .32. Indonesia, with a 142 deaths among 172 cases reported to WHO, has a CFR of .82. Although there may be different strains of H5N1 circulating in various countries, I imagine the difference in the CFR between these two countries is statistically significant. Given that H5N1 is endemic in both of these countries and poultry flocks are a family affair in both of these countries there should not be such a difference. The higher death rate in Indonesia can not be attributed to late treatment because Indonesia routinely uses a "Tamiflu blanket" both as treatment and as prophylaxis. Symptomatic individuals in Indonesia almost always seem to receive antivirals with the 24 hour window. Depending on the pattern of testing, individuals under antiviral treatment almost always test negative. I don't think follow up antibody studies are ever completed. If there are such studies the results are never reported.

I am left with the conclusion that Indonesia is underreporting the number of human survivors of H5N1 infections.

Re: Indonesia - Padang, West Sumatra - 16 currently suspected cases of bird flu in Padang (second isolation room opened), 9 with positive rapid H5N1 test go home - March 2 +

"While the other nine have been allowed to go home due his condition had improved, although positive results of their test rapid bird flu," he said."

Very interesting. Either the officials have no faith in the rapid tests and think these people are not H5N1 positive, or, it is acknowledged that there is a low cfr version of H5N1 circulating and these people can be treated with standard anti-virals and flu home treatments.
