The family developed high fever and then were brought to Datoe Binangkang hospital and later transferred to Kandou hospital.

The patients are now recovering, but so far there is no examination result of the cause of the disease.

In the meantime, about 25 chickens in Gogagoman died and tested positive H5N1 infection. The chickens died-off without showing common bird flu signs such as bluish comb/wattle and subcutaneous bleeding of fetlock, which is raising a suspicion of possible virus mutations, told the authority of Livestock Service in Kotamobagu.

The reports (here and elsewhere) seem to indicate that all individuals were from the same family (apart from one HCW? contact), and that all were symptomatic initially at the time of being placed in isolation.

What is not indicated is how many family members did not develop symptoms from this initial group; if every member of the family became ill at the same time, it is concerning as this could indicate a very high transmission rate - regardless of the type of flu involved. One would presume that rapid tests were positive to have prompted this action by the authorities?

Any further information on this family group, and especially the numbers of those who did not become symptomatic following exposure would be welcomed.

This now seems inaccurate from the later reports and it would seem that instead chickens died and were tested, and family members became ill with high fevers and sought medical assistance independently - it also sounds as if necessary isolation procedures could not be put into place in the early stages. Although the media reports do imply that since that time proactive monitoring of locals has been put into place in the area where the chickens died; however, it isn't clear from these reports when the chickens died and how many days passed before these individuals became ill.


The reports about the suspect cases are also a little confusing; Up to 10 patients were reported as being from Gogagoman, but later reports imply at least two locations and several family groups. Additionally, one contact of one of the patients is reported as being in isolation and it is not clear who this might be, so I thought I would try to break this down so we can add to lists later if necessary.

I am not sure I get that sense of things from the patients point of view from reading the interviews and reports; it is also odd that they all came down with illness within 2-3 days of eating H5N1 infected chickens (this is confirmed) without any report of any one individual being ill at the time of the holiday meal who could have passed another 'seasonal' flu to them at the same time. Another issue is that even if it were a 'seasonal' flu that infected them all, there was the potential for a co-infection with H5N1 as they all ate the infected chicken and were exposed to other H5N1 sick chickens at the same time, so either way cause for a lot of caution.

If it is H5N1, the chances are we will never know for sure one way or the other given the protocol for H5N1 infection in Indonesia (i.e 2- 3 positive tests where tamiflu is administered between tests 1 and 2/3; only if all are tests are positive is a patient declared as infected; this is very likely why the official mortality rate in Indonesia is so high, as only individuals who fail to respond to treatment are recorded as infected - in my opinion).

What is important here is that the infection (if H5N1) has responded well to treatment, which was sought early - and this is a good thing. The fact that all of them were suffering from high fever and breathlessness at the time of hospitalisation but rapidly recovered following treatment would tend to indicate that even if it was an H5N1 infection, viral load would be very low at this stage i.e post tamiflu treatment and it is unlikely that anyone would be infectious.

What is particularly interesting / concerning here from my perspective is the reports that H5N1 in chickens was showing a different infection to usual i.e no blue wattle in the chickens that died, and that if these individuals had been positive for H5N1 at least on the first test, it could indicate an increased propensity for human infection along with a reduction in virulence.

The chances are we will never know one way or the other for certain.

From my perspective, so long as this incident passes without large additional numbers of hospitalisations of 'suspects' arising from infected chickens or contacts with other suspects, then the chances are we don't have a more infective virus circulating in chickens, which is what I would hope will be the case.
