Twelve Residents of Dusun IV Air Batu Asahan are AI Negative

Result of Real Time Polymerase Chain Reaction (RT-PCR) test by National Institute of Health Research and Development (Balitbangkes Depkes) is negative of avian influenza (AI) virus to 12 residents of Afdeling 5 Damuli Dusun IV Air Batu, Asahan District, North Sumatera .

Now they are hospitalized in Kisaran District Hospital and Adam Malik Hospital, Medan . Those who are in Adam Malik Hospital, Medan (AI referral hospital) are FH (f, 7 year) and MSM (m, 8 year), getting better, while the remains are in Kisaran District Hospital.

This latest information is received by Center for Public Communication from MoH AI Post on August 9, 2008.

The cases were found during active surveillance on August 5�7, 2008 by North Sumatera Provincial Health Office and Asahan District Health Office.

Therefore, some efforts are attempted, such as free hospital treatment, oseltamivir provision, taking and delivering specimen to MoH Balitbangkes, suspect observation, contact surveillance and health information.

By now, MoH team is still implementing active surveillance to detect new cases as well as Communication, Information and Education on Avian Influenza.

Rapid Test by Asahan District Farming Office and Veterinar laboratory research of North Sumatera Provincial health Office to 15 birds (chicken and duck) suddenly dead on July 28, 2008 in Afdeling 5 Damuli Dusun IV Air Batu, Asahan District, figured out AI positive infection.

Mostly people in Afdeling 5 Damuli Dusun IV Air Batu are bird (chicken, duck and wild duck) farmers, usually for consumed and sold out. The coops are usually right next to the house, and sometimes the birds are not cooped.

Children usually wear no sandal when playing around while birds are freely on the surrounding. The sanitation in that village has not met the health requirements. It could be the risk factor, especially in AI infected areas.

The Control of Bird Flu During the First Semester in 2008

Until this, the spread of Bird Flu (FB) still came from the poultry to humankind. AI Control during Semester I in 2008

By today, transmission of avian influenza (AI) is still from birds to human. Laboratory research of National Institute of Health Research & Development (Litbangkes) and Eijkman Institute to AI case contact in 5 infected provinces found that there is no human-to-human transmission. This is the progress of AI control during Semester I 2008.

MOH AI Posts in 12 provinces of infected human case reports that in 7 provinces there is no new case found in the last 10 months to 2 years. Those 7 provinces are Lampung, infection in 2005 (no new case for 2 years 9 months); South Sulawesi, infection in 2006 (no new case for 2 years); South Sumatera, infection in March 2007 (no new case for 15 months); East Java, infection in March 2007 (no new case for 15 months); Riau, infection in April 2007 (no new case for 14 months); North Sumatera, infection in May 2007 (no new case for 13 months); Bali, infection in August 2007 (no new case for 10 months).

On the other hand, another 5 provinces with new case found are: DKI Jakarta (last case 13 May 08), West Java (last case 14 May 08), Banten (last case 11 Nov 08), Central Java (last case 23 April 08) and West Sumatera (last case 29 March 08).

In Banten, the last case is AS (m, 38 year), Tangerang City resident, who works as cargo driver. His onset was on 4 July 2008. On 8 July, he got symptoms of fever, cough, difficulty in breath and progressive Pneumonia on left thorax. The risk factor was birds (chicken and duck) in neighboring area. Bird specimen has been taken and sent to animal laboratory in Wates (Ministry of Agriculture).the research is in progress.

He was hospitalized in Usada Insani Hospital , Tangerang. On 9 July, he was referred to Tangerang District Hospital and died on the next day, 10 July 2008. Research in laboratories of Litbangkes and Eijkman Institute, Jakarta , on 11 July 2008, found that AS was positive of AI infection.

As consolidation of influenza pandemic preparedness, government launched Simulation Exercise on Containment of an Epicenter of Pandemic Influenza in Jembrana District, Bali on 25 � 27 April 2008. This is the first comprehensive simulation in the world. It took place at health center, Control village area, Jembrana District Hospital , Tabanan referral hospital and Sanglah Hospital in Denpasar. The simulation also covered screening in Ngurah Rai International Airport , Despasar.

It involved and was also covered by national and international mass media and observers. In general, many observers appreciated the drill of simulation. Today, the protocol of containment of an epicenter of pandemic influenza is in progress of finishing, relating to the findings during simulation, while film of simulation is being edited.

Number of AI case during July � December 2005, 2006 and 2007 tended to decrease. Cases in semester II 2007 decreased 6.2% comparing to in semester II 2006. Cases in 2006 decreased 20% from 2005, and number of AI suspects in semester II 2007 decreased 30.6% from 2006.

The efforts also cover laboratory examination, AI positive case detection, observation to contacts of positive case and health information. AI patients in hospitals have been given special treatment since they are suspected of AI infection. They are taken in isolation room, given oseltamivir and managed by a team lead by as well as all cost covered by Ministry of Health (MOH). For the dead cases, cost of burying is covered by MOH.

In addition, MOH also provides trainings. MOH cooperated with University of Indonesia and University of Gajah Mada , Yogyakarta , has included curriculum of Field Epidemilogy Training Program/FETP Indonesia and is finishing BSL-3 laboratory by Litbangkes MOH.

Indonesia Bird Flu Handling Effort During June 2005 � August 2008

, September 18, 2008 - Based on Ministry of Health Bird flu Information Center record, up to August 2008, from twelve provinces that have recorded bird flu on human, seven provinces haven't reported any bird flu case in at least 10 months.

A bird flu epidemiological surveilance found a bird flu positive case in Tangerang, Banten Province in July 2008. JK, a 20-year old man from Tengerang, Banten Province died from bird flu on July 31 2008.

During August 2008, there are no new cases. Up to now, exposure to sick birds remains the primary source of human infection with Bird Flu. Based on the test of the speciment and contacts of the cases during January – 31 August 2008 there are no indications of human to human transmission

Data during January – August 2008 shows that number of case are decreasing. During Janury – 31 August in 2006, 2007, 2008, the number of bird flu cases respectively are 45 cases, 31 cases, and 20 cases. This data shows that the number of cases dercreses 35.4% compare to the same period last year, and 55.5% decrease compare to 2006.

As for the number of suspect cases, data shows a 45% decrease compare to the same period last year, and 25% decrease compare to 2006.

The Government of Indonesia had conducted several efforts in controllong bird flu. Those efforts involve trainings, free treatment for bird flu suspect and confirmed cases –which includes free corpse handling for positive cases.

Providing oseltamivir at the local clinics, referral hospitals and also at the district and provincial level health agencies.

A training for bird flu spokesman in 12 regions. Communicating and giving out information on bird flu handling thru mass media. Table top simulation in handling influenza pandemic epicenter in Palu, Central Sulawesi Province in August 2008.

The government also has formulate the modules of a contigency plan in handling an influenza pandemic epicenter in district and provincial level.

At the international level, Indonesia government together with other developing countries and the Non-Alignment Movement has come up with the World Health Assembly Resolution (WHA 60.28) in May 2007 about actions needed to improve transparency, fairness and equality in using information, diagnosis, drugs, vaccine, and other technologies.

137 BIRD FLU CASES IN RI UP TO AUG 2008

Jakarta 23/9/2008 (Kominfo-Newsroom) – Total of avian flu cases in Indonesia up to Aug. 2008 comes to 137, with 112 fatalities.

Avian Influenza (AI) cases are found in 12 of 33 provinces. And sufferers are 51% male and 49% female, according to Erna Tresnaningsih, Director of Control of Diseases caused by Animals, Health Ministry, here Tuesday (23/8).

AI medicines are only available in state hospitals or Puskesmas (public health centers).

Tamiflu is not available at private hospitals and dispensaries. The government is afraid they might be exploited by irresponsible business circles.
