Having done some digging and looking at the various contexts the terminolgy has been used in, I think that flat and flattest relates to a patient being in a critical or stable condition, along the lines of our terminology where we can have a patient who is on a relative scale of health ranging from a 'serious but stable condition', a 'critical but stable condition', or just a plain 'critical condition. ie flat, most flat or flattest??

I didn't see the discussion, but kamus-online translates "terpapar" as "spread out" FWIW. Prostrated/critical, maybe? According to my "Indonesian: A Comprehensive Grammar" (which unfortunately I haven't read as such!) "ter" + adjective makes the adjective superlative, and indeed "papar" is translated as "smooth", "level", "flat" so that makes sense. "ter" can do a number of other things, but the other things seem to produce nouns or verbs, whereas in this context we really do want an adjective.
