Diagnostic yields are higher with throat specimens than with nasal swabs because of higher viral loads of influenza A (H5N1) in the throat.1,3 However, nasal swabs are useful for detecting human influenza viruses, so collection of both specimens is recommended. If they are available, tracheal aspirates have higher viral titers and yields than specimens from the upper respiratory tract.3 Negative results in single respiratory specimens do not rule out influenza A (H5N1) virus infection,21 and repeated collection of multiple specimen types is recommended.52 Previous antiviral treatment may reduce the diagnostic yield. Detection of influenza A (H5N1) viral RNA in feces or blood may provide prognostic information,3 but it has lower diagnostic sensitivity than influenza A (H5N1) viral RNA in respiratory specimens.

Commercially available rapid assays for influenza-antigen detection have poor clinical sensitivity for the detection of influenza A (H5N1) virus (Table 2 of the Supplementary Appendix),1,20,21 and they do not differentiate between human and avian subtypes of influenza A viruses. Although rapid antigen tests have similar analytic sensitivity for detecting human and avian influenza A (H5N1) viruses, they require 1000 times higher levels of virus than viral cultures to be positive.53

The detection of anti-H5 antibodies is essential for epidemiologic investigations and may provide retrospective diagnostic confirmation in patients. Seroconversion generally occurs 2 to 3 weeks after infection. Microneutralization assays are the most reliable methods for detecting antibodies to avian viruses, but they are labor-intensive and require biosafety level 3 facilities and appropriate strains of influenza A (H5N1) viruses. As compared with initial samples, elevations of four times or more or single titers of 1:80 or more in convalescent-phase samples are considered to be diagnostic.52 Modified nonpathogenic influenza A (H5N1) virus generated by reverse genetics or lentivirus pseudotyped with H5 hemagglutinin54 may provide alternatives for performing neutralization tests in biosafety level 2 facilities. Hemagglutination-inhibition assays with the use of horse erythrocytes show promising results but require further validation.

This would appear to be an extremely important cluster of cases, certainly worthy of international study (MUCH more so than the widely publicized Jiangxi China Father-Son case that was sadly lacking in vital details, but apparently worth writing up in Lancet).

As pointed out above, rigorous testing is required to determine with a degree of confidence, whether or not H5N1 infection is present. Antiviral drug treatment complicates detection, obviously.

The nurse's case suggests that family genetics may not be a dominant feature of susceptibility. Locality, however, is a dominant feature.

Indonesian public health officials should retain the exceptionally important patient (all, not just viral) samples obtained from this cluster of patients, storing them carefully for re-analysis.

Now would be a VERY good time to capture index and associated case exposure details.

We will be revisiting this cluster for in-depth analysis, later on.

PENGASINAN, MONDE: Kondisi korban suspect flu burung, Myi (16), yang kini dirawat secara intensif diruang isolasi RSPI Sulianti Saroso mulai membaik, tapi kakaknya justru mulai batuk dan sesak nafas?

Kerabat keluarga korban, Ugi yang turut mengikuti perkembangan kondisi kesehatan korban di rumah sakit,mengatakan hal itu. ?Kondisinya makin membaik, tetapi masih dikarantina di ruang isolasi dan belum diperbolehkan pulang,? tuturUgi yang merupakan teman kerja ayah korban.

Sebelumnya, Rabu (2/4) korban dinyatakan suspect flu burung oleh RS Bakti Yudha dan langsung dirujuk keRS Sulianti Saroso karena keterbatasan alat untuk penanganan flu burung di rumah sakit tersebut.

Sementara itu, berdasarkan keterangan Ugi, kakak korban yang berinisial Ai kini kondisinya mulai mengalamipenurunan disertai batuk dan sesak napas, sama persis dengan keluhan awal yang diderita adiknya.

?Kakaknya kondisinya kini juga menurun, gejala yang dialaminya juga sama seperti batuk-batuk dan sesaknapas, saya dan ayahnya sudah menyuruhnya ke rumah sakit tetpai hingga kini sepertinya belum dilakukan,?terangnya.

Saat Monde mencoba melihat kondisi Ai di kediamannya, rumahnya tampak sepi dan tidak diketemukankeberadaan Ai saat itu. Para tetangga yang ditemui Monde juga tidak mengetahui keberadaannya.

?Dari tadi sih saya belum lihat adaorang, mungkin pergi dari tadi pagi kali,? ujar salah seorang tetangganya.Menurut keterangan Ugi, kakaknya telah melakukan pengobatan di salah satu balai pengobatan danmengatakan kondisinya tidak apa-apa walaupun sering batuk-batuk.

?Saya dan ayahnya ingin dia cek ke rumah sakit intinya supaya tahu sakit apa, biar jelas, tetapi orangnyatetap nggak mau, padahal gejalanya hampir sama dengan adiknya,? tegasnya.Sementara itu, Camat Sawangan Usman Haliyana beserta Lurah Pengasinan Muksin Mawardi melakukanpengecekan terhadap situasi wilayah RT 01/RW 03 yang didapati unggas positif flu burung, kemarin.

?Memang unggas yang positif flu burung tersebut merupakan peliharaan warga yang proses pemeliharaanyamemang kurang baik dan sering dibiarkan lepas di lingkungan,? tuturnya.Untuk itu ia mengimbau kepada masyarakat Pengasinan di sekitar wilayah tersebut untuk memelihara danmengandangkan unggas peliharaanya secara baik, dan diberikan perawatan secara intensif.

Dari setidaknya 16 pintu masuk unggas yang akan ke Kota Depok hanya satu pintu saja yang saat inimendapatkan penjagaan ketat pengawasan dari Dinas Pertanian (Distan).Hal itu dituturkan oleh Kepala Seksi Kesehatan Hewan dan Kesehatan Masyarakat veteriner (Kesmavet)Deddy Sujadim kepada Monde di ruang kerjanya, kemarin.

Di antaranya, katanya, dengan melakukan biosecurity, yakni melakukan penyemprotan ayam dengandesinfektan di lokasi pemotongan. Kepada sejumlah pengusaha potong unggas pun dianjurkan untuk takmemotong ayam yang telah mati atau pun yang sedang sakit .
