At least 23 people from 4 villages in Pegunungan Bintang regency, Papua have died from acute respiratory infection since January [2008], a local health official says.

"This is not an extraordinary situation, and health workers have been providing medical care in the 4 villages," Pegunungan Bintang Health Office head Darius Salamuk told The Jakarta Post by cell phone Monday [24 Mar 2008].

The affected villages are Okteneng, Kaeb, Kokiabakon and Bakonaib, all in Kiwirok district.

Salamuk said respiratory diseases were a major problem in Papua's central mountain areas. One reason for this, he said, is that people sleep near the fire in their "honai" homes due to the cold weather, exposing them to smoke [A honai is a hut made of sticks and thatch.].

"This is a common health condition suffered by most of the people in
remote villages. Sufferers could be cured if they sought medical treatment immediately, but because they're beyond the reach of medical services, some of them cannot be saved," said Salamuk.

Medical workers must walk for days or use small planes to reach the
remote villages.

Salamuk said the team dispatched by the regency administration to provide medical attention to villagers had found a number of rudimentary problems. "The villagers lack clean water and good sanitation. Their housing conditions are poor, and they sleep by the fire in their honai to warm themselves at night. They also lack food that meets nutritional standards," said Salamuk.

These problems are compounded by the lack of medical care. "The villages are only served by an auxiliary community health clinic in Mipol village, which takes at least 6 hours to reach on land from the 4 villages," he said.

JAKARTA - A 15-year-old Indonesian boy has died of suspected bird flu, a doctor said Thursday.
The boy, identified only as AY, was being transferred to a hospital in West Java when he died late Wednesday, said Hadi Yusuf, head of the facility’s bird flu team.<o:p></o:p>
‘He has a history of contact with poultry on the family’s own backyard farm,’ Yusuf said.<o:p></o:p>

The thatched huts are very primitive. The dirt floor is covered with straw and the low ceiling has a trap door leading up to the 3 ft. high straw covered sleeping loft. Sounds cozy but the smoldering fire down below, without a chimney, virtually turns everyone into smoked jerky. This may ward off mosquitoes and keep them warm on cold nights but eye problems are common.

Hadi Jusuf, head of the hospital's bird flu mitigation team, said the condition of the patient was very poor upon arrival at the hospital on Wednesday evening.
"He had breathing difficulties and was unconscious. Moreover he had acute lung wounds," Hadi was quoted by national newspaper The Jakarta Post as saying.
The boy from the town of Subang, identified with initials AY, had been treated at a smaller hospital four days earlier with doctors diagnosing him of dengue fever.
A week earlier, his younger brother died because, according to the same hospital doctors, of dengue fever.
AY's blood samples had been sent to a laboratory of the Ministry of Health in Jakarta to ascertain whether AY was infected with the H5N1 virus or not, Hadi said.
Bird flu has infected 129 Indonesians so far, killing 105 of them, which is the highest death toll in the world.

More effective medication needed in fighting bird flu

More effective medication is needed to fight the H5N1 virus that causes Avian Influenza, as symptoms of resistance toward Osetamivir, the current medicine, had appeared in bird flu patients in some regions, an official at an Avian Influenza Summit said.

"We have learnt some bird flu patients in Hong Kong and Vietnam have shown resistance to old medicines," head of the National Working Group on Avian Influenza Eradication Amin Subandrio told reporters at the summit held at Nusa Dua resort Thursday. [Old medicines? Adamantanes? Or seasonal human influenza A/H1N1 resistant to oseltamivir? I.O.H.]

"That's why we gather here, to find out whether or not we are ready to tackle the possibility that people are becoming immune to bird flu medication," Amin said.

Representatives from 25 countries attended the two-day summit, including from the U.S., Japan, South Korea, Indonesia and Kenya, who discussed the need for new medication to fight the AI virus

An exhibition demonstrating medical breakthroughs in treating bird flu by pharmaceutical companies from participating countries was also held.

Many countries are currently using Osetamivir, locally known as Tamiflu, to treat patients diagnosed with bird flu.

Tamiflu must be given to bird flu patients early as it reacts with the virus while it is still in the patient's blood. Once the virus enters the patient's lungs, the tablet is not much use, Amin said.

"We must then mix it with other medication to avoid further damage," he said.

The disease has killed 105 of the 129 people who have contracted the disease in Indonesia.

Amin said Tamiflu was still needed to treat patients suffering from bird flu.

"But we must be very careful not to give this medication to people who haven't been officially diagnosed with bird flu. If we do, they could become resistant to the medication in future," he said.

With new bird flu medication expected to be available in five to ten years time, campaigns on the necessary precautions to take were essential to avoid the disease, Amin said.

Head of the Surveillance and Epidemiology division of the Agriculture Ministry, Muster Any Ragman Nor, said the campaign has generated promising results in some regions but had failed in some others.

"The campaign failed in slum areas where poultry breeders put their poultry cages near their houses. It is not that they are unaware of the new rule that prohibits this, but they lack the funds to build new cages outside the housing area," he said.

Separately, a group of activists from the Asia-Pacific's People for the Ethical Treatment of Animals (PETA) held a rally in front of the summit venue. Two of them wore giant chicken costumes and held a banner calling for people to stop eating meat and become vegetarian.

Ashley Fruno of PETA said people should really stop eating meat as it had long been known to be a source of human illness.

"Besides killing an animal just to get its meat is just another way of justifying slaughter. We should be more caring towards other creatures," Fruno, who has been a vegetarian for 12 years, said Wednesday.

She said the vegetarian lifestyle had been proven to prevent degenerative diseases such as heart attacks and high blood pressure.

Suspected bird flu patient dies

BANDUNG: A 15-year-old boy from Subang, West Java, suspected of suffering from bird flu died Wednesday evening just one hour after being admitted to Hasan Sadikin Hospital in Bandung to receive intensive medical treatment.

Hadi Jusuf, head of the hospital's bird flu mitigation team, said the condition of the patient, identified only as AY, was very poor upon arrival at the hospital at 4:30 p.m.

"He had breathing difficulties and was unconscious. Moreover he had acute lung wounds," Hadi explained Thursday.

AY had been treated at Cieereng Hospital four days earlier, his high body temperature leading doctors to believe he suffered from dengue fever.

A week earlier AY's younger brother died at the same hospital due, according to the hospital doctors, to dengue fever, Hadi said.

AY's blood samples had been sent to a laboratory at the Ministry of Health in Jakarta to ascertain whether AY was infected by the H5N1 virus or not, Hadi said. -- JP

JATI, METRO-- Rumah Sakit Dr M Jamil Padang merawat pasien suspect flu burung. Seorang balita bernama Alifah Qasya berusia 1 tahun 11 bulan, warga Ngarai Sianok Kota Bukiktinggi, dirawat intensif di ruang isolasi IRNA Penyakit Dalam RS Dr M DJamil Padang. Alifah tiba di rumah sakit Senin (24/3) sore.
Pasien yang mengalami demam tinggi disertai sesak nafas berat langsung dirawat di ruang isolasi.

Menurut Kepala Bagian Medik RSUP M Djamil Kamis (27/3), kondisi pasien memperlihatkan gejala seperti terserang flu burung - meskipun pasien tidak ada riwayat kontak dengan unggas. Kondisi tubuh dengan demam tinggi, batuk dan flu. ?Berdasarkan surat rujukan dari puskesmas dan keterangan keluarga, pasien langsung dirawat di ruang isolasi. Melalui prosedur tetap (protab) flu burung, dilakukan rapid tes terhadap daerah pasien,? ujar Irayanti.

Menurutnya, dari hasil rapid tes yang dilakukan tim medis penanganan flu burung, sampel darah pasien menunjukkan hasil negatif. Tapi karena protab yang ada, sampel darah pasien tetap dikirim ke Laboratorium Pengembangan Kesehatan (Litbangkes) Departeman Kesehatan RI di Jakarta - untuk memperoleh hasil yang lebih akurat. Hasil dari Litbangkes akan keluar lima hari. Namun, Irayanti mengungkapkan, biasanya kalau hasil rapid tes menunjukkan negatif, kemungkinan hasil laboratorium juga negatif.

Apalagi, pasien tidak ada riwayat kontak dengan unggas.Tapi, karena prosedur untuk penanganan pasien suspect flu burung, maka untuk kepastian rumah sakit tetap menunggu hasil sampel darah Litbangkes. Dari informasi yang didapat oleh keluarga pasien, Alifah mengalami demam tinggi sekitar 1 minggu. Sebelumnya, kakak Alifah meninggal dunia seminggu sebelumnya dengan gejala yang sama dengan adiknya yaitu, panas tinggi, batuk dan sesak nafas berat.

?Informasi dari ibu pasien, Alifah panas tinggi setelah kakaknya meninggal seminggu sebelumnya. Karena kuatir pihak keluarga dan dokter yang merawatnya d Bukiktinggi merujuk ke M Jamil,? kata Irayanti. Hingga hari ketiga di rawat intensif di ruang isolasi Irna Penyakit Dalam, Alifah kondisinya belum stabil. Panasnya masih tinggi, sesak nafas dan batuk. Pasien tidak bisa dikunjungi oleh orang lain kecuali dokter yang merwat dan ibu pasien yang mendampinginya. (ren)
