H5N1 Clusters and Denials Raise Indonesian Pandemic Concerns

Kandun dismissed the possibility of more bird flu cases in the same family after the boy's brother died recently. Confirmed cluster cases raise concerns over human-to-human transmission. "It is not correct that there is a cluster in Subang," Kandun said, adding that the brother has died of dengue fever.

The above denial of an H5N1 cluster in Subang is cause for concern. Both brothers were initially diagnosed as dengue fever. However, since dengue fever is transmitted by insects, they likelihood of two fatal cases in the same family with distinct disease onset dates is remote. Consequently, the second case was tested for H5N1 and was positive, strongly suggesting that the index case was also H5N1 positive and infected his brother. The denial of the cluster by the director general of communicable disease control in Indonesia raises serious transparency problems, and is similar to denials of H5N1 in the first confirmed cluster in Indonesia, almost three years ago. Those cases were initially said to have died from bacterial pneumonia, although the father was subsequently H5N1 confirmed, and the H5N1 from that patient is a target for vaccines directed to clade 2.1, the clade of H5N1 in Indonesia.

In addition to the death of a family member of an H5N1 confirmed case in West Java, a similar cluster has been reported in the local media in Sumatra. H5N1 has been confirmed in a young child, but the recent death of a family member has not been reported in the English language press. Similarly, the nurse who cared for one or both family member is in isolation with bird flu symptoms, raising concerns of a expanding cluster.

The two clusters above, as well as a third confirmed case, located in Bekasi, which is near the two brothers in West Java, raises pandemic concerns. Although clusters in Indonesia are not new, the government denial of the cluster in West Java and the failure of the English press to recognize the cluster in Sumatra, raises concerns that the number of cases and clusters are significantly higher than the reported confirmed or suspect cases.

More information of testing in these two clusters would be useful. Dengue fever claims raise concerns that additional H5N1 cases are being mis-reported.

Two Indonesian youths have died from bird flu, a health ministry
official said on Monday [31 Mar 2008], taking the confirmed death
toll in the country worst affected by the virus to 107.

A 15-year-old boy from Subang, in West Java, died on Wednesday [26
Mar 2008] in an area where chickens had died, said Nyoman Kandun,
director general of communicable disease control at the ministry.

An 11-year-old girl from Bekasi, east of Jakarta, who died on Friday
[28 Mar 2008] also tested positive for the virus, the official said.

"There were dead chickens in the boy's neighbourhood, but in the
girl's case it is still unclear," Kandun said via a mobile phone
text message. Kandun dismissed the possibility of more bird flu
cases in the same family after the boy's brother died recently.
Confirmed cluster cases would raise concerns over human-to-human
transmission. "It is not correct that there is a cluster in Subang,"
Kandun said, adding that the brother has died of dengue fever.

Earlier on Monday, a 22-month-old girl from Sumatra's Bukit Tinggi
tested positive for bird flu and the health ministry was checking her
neighbourhood for possible backyard farming. "Her condition is
improving, and she is being treated at a Padang hospital," Lily
Sulistyowati, a health ministry spokeswoman, said by telephone.

Including the latest deaths, Indonesia has had 132 confirmed cases
of the virus. Contact with sick fowl is the most common way
of contracting the H5N1 virus, which is endemic in bird populations
in most of Indonesia. According to United Nations' Food and
Agriculture Organisation (FAO) data on March 19, bird flu has
infected 31 out of 33 provinces in Indonesia.

total to 132, and the number of fatalities to 107. The global total
of cases becomes 376 and the global death toll 238.

None of these 3 new Indonesia cases are related. They occurred in

AGY (M, 15) citizen of Cimerta village, Subang district, West Java had symptom of fever and hard breath since March 19, 2008, before he was brought to Subang Local Public hospital, March 22, 2008. There were 3 dead chickens in his environment. AGY was then referred to Hasan Sadikin Hospital on March 26, 2007 in unconsciousness condition and died on the same day. From examination result of Health Development and Research Board, AGY was stated positive infected by H5N1 virus. This result was strengthening by laboratory examination of Eijkman Institution, Jakarta.

While in other part of West Java, ZAH (F, 12) Harapan Baru residence, West Bekasi also had some symptom of fever and cough since March 19, 2008. ZAH has just brought to Infectious Hospital, Suliati Saroso Jakarta on March 27, 2008 and died on March 28, 2008. By the same laboratory, ZAH also stated positively infected by H5N1, avian influenza virus.

In Bukit Tinggi, ALQ (1 year old) avian influenza positive is residence of Lembah Ngarai Sianok. Besides fever, cough, running nose and hard breathe, ALQ also had diarrhea. Early symptom showed on March 17, 2008 and ALQ was brought to Stroke Government Public Hospital, Bukit Tinggi (22/03/08) before got treatment in M. Djamil Hospital, Padang on March 24, 2008. He still in patient in that hospital, AQL condition now stabil without ventilator.

Ministry of Health team which is an integrated team with Ministry of Agriculture has come to do some extent investigation towards contact history and that avian influenza cases. Health center and hospital have to be aware to patient with symptom same with influenza. Community also asked to concern to bring their family who indicate influenza symptom to the nearest health center and hospital if there is bird sudden dead in their houses.

Today, avian influenza is still a disease which is transmitted from bird to human. Cutting contact with bird and the importance of keeping self- and environmental cleanliness are some effort that has to be increased in community to prevent us from infected by H5N1 virus, Try to always wash hands and foot with water and soap every has outdoor activity. This message needs to be informed by parents to children which usually interact to outside environment, telling children not to play with bird or even hold dead bird.

People should also keep the cleanliness of cooking and eating tools by washing it with water and soap. Birds are safe to be consumed if it is clean and well cooked before. Besides that, for public community, separate bird from human. Separate the old and the new bird, at least for two weeks. For those who take care of bird, keep the bird cage cleanliness by washing it with disinfectant and clothes bleaching.

Another infant death case of malnutrition reported in Indonesia

JAKARTA, April 1, (Xinhua) --

A nine-month-old baby, identified as Rasa, died of malnutrition on March 31 after undergoing medical treatment at Dr Soewandi hospital in Surabaya, national Antara News Agency reported on Tuesday.

"Rasa was admitted to the hospital on Sunday, March 30, but the baby of a construction worker died a day later," hospital spokesperson Indriati was quoted by Antara as saying.

According to her, Rasa's death brought the number of malnutrition fatalities in Surabaya, provincial capital of East Java, to three in 2008.

She added that the three fatalities were among 47 children under five who had been admitted to Dr Soewandi Hospital since January this year.

Communication and Information Minister Mohammad Nuh asked for enhancement of social solidarity, but not medical treatment only, to tackle the increasing incidence of malnutrition.

According to data collected by the Surabaya municipal health service, there were 9,557 undernourished under-five infants in Surabaya in 2007.

It is estimated that there are over 30 million of people in Indonesia live in beggarliness without sufficient supply of foods.

A mother with her infant died of malnutrition at home in Makassar, Sulawesi last month became the focus of media in Indonesia.

Last reports from Indonesia during February were stating that the officials were having a hard time containing the spread. They were close to saying it was out of hand. Since the flu virus spreads quicker in the colder temperatures. I would have to say this fall may be a hazardous one..........

I am confused. Afifah (Alifah) Qansa (1 year 11 months) is admitted April 1, 2008 to RS M. Djamil Kota Padang (post #173) as a suspected BF patient. Post #175 indicates that Afifah is now positive. This can't be ALQ because she was admitted to RS M. Djamil Padang on March 24 2008 and reported positive by DEPKES on March 31.

In Bukit Tinggi ALQ (F, 1 year) positive bird flu was the resident of Lembah Ngarai Sianok. Apart from the fever, the cough, pilek and breathless, ALQ also experienced diarrhoea. The sign of early began apparently on March 17 2008 and ALQ was carried to RSUP Stroke Bukittinggi (22/03/08 ) before being treated in RS M. Djamil Padang on March 24 2008. Still was treated in this hospital, the AQL condition currently stable without the ventilator.

Meanwhile post #173 notes that being treated in the same hospital in the isolation unit are "Two other suspected patients are Etriani (29) and Annisa." (But no mention of ALQ) Post#176 states that Annisa may now be positive for bird flu, but they are waiting on a telephone call from Jakarta. But if they are waiting for results on Annisa, then she can not be ALQ either since ALQ is already confirmed.

So who is ALQ?

Or are there some simple misreporting errors of names, ages, dates of onset, hospital admission, timing, etc. that I can not see?
