GENEVA, Dec 9 (Reuters) - Indonesia has confirmed two new cases of human bird flu, the first officially reported since September in the country which remains the hardest-hit by the deadly virus, the World Health Organisation said on Tuesday.

A nine-year-old girl in Riau province developed symptoms on Nov. 7 after poultry apparently died at her home, the WHO said in a statement. She was hospitalised five days later and discharged on Nov. 22 after recovering.

A two-year-old girl from East Jakarta died on Nov. 29 after developing symptoms on Nov. 18, it said. "Investigations into the source of her infection suggest exposure at a live bird market."

The two latest cases took Indonesia's known number of bird flu infections to 139, including 113 deaths, since 2003, according to the United Nations health agency.

The primary focus of this week's WHO gathering is to discuss, and hopefully resolve, issues pertaining to the sharing of virus samples and vaccines.

Indonesia is claiming IP (Intellectual Property) rights on viruses that emerge from within their borders, and believe that they should control their distribution and use.

Indonesia has refused to `share' virus samples until guarantees are made that they will be provided with vaccines, made from these samples, at an affordable price.

Additionally, Indonesia has been slow to report human cases of H5N1 infection - preferring to provide aggregate totals every few months instead of real-time reporting.

This dispute has been ongoing for two years, and during that time the world has had very little access to virus samples from Indonesia, creating a dangerous blind spot for scientists hoping to detect changes in the virus.

While the official death toll stands at 113 in Indonesia, due to inconsistent and oft times inaccurate testing, few observers believe that number fully reflects the situation.

The Ministry of Health of Indonesia has announced two new confirmed cases of human infection with the H5N1 avian influenza virus. A 9-year-old female from Riau Province developed symptoms on 7 November and was hospitalized on 12 November. She recovered and was discharged from hospital on 27 November. Laboratory tests confirmed the presence of the H5N1 avian influenza virus. Investigations into the source of her infection indicate poultry deaths at her home on 2 November.

It was not immediately clear why the WHO toll does not include a 15-year-old Indonesian girl in central Java whose doctor said she had died in early November. Presence of the virus had been confirmed in a health ministry lab, according to Agus Suryanto, head of the medical team treating the girl.

The above comments confirm an H5N1 case in Riau Province and question why a fatal infection of a 15F in central Java was not reported. The WHO situation update also described confirmation of a fatally infected 2F from East Jakarta.

The 9F had been described by the local media, but the 2F was not. However, there have been a large number of recent suspect cases and clusters in the local media, raising concerns over undercounts, as indicated in Reuter?s comment on the lab confirmed 15F.

The reporting of the two recent cases is a marked improvement over reports of earlier cases this year, which were delayed by weeks or months. However, the steady stream of suspect cases or clusters in Indonesia and reports that 50% of poultry sold in Java was H5N1 positive create causes for concern.

The above comment from the most recent WHO situation update describes a second recently H5N1 confirmed case in Indonesia. The above case was in East Jakarta, where there have been many previously confirmed cases. This is also true of the case in Riau. However, recent reports have indicated 50% of the poultry sold in markets in Jakarta, Banten, and West Java is H5N1 positive, raising concerns of widespread human cases in the area. The two most recent cases were reported relatively soon after confirmation. However, earlier cases were reported weeks or months after confirmation, raising serious concerns.

In addition to the news blackout on confirmed cases, other lab confirmed cases and clusters have been denied and in other cases, index cases of H5N1 confirmed clusters have been said to be due to lung inflammation, dengue fever, and typhus.

The concerns surrounding these cases has been compounding by a withholding of samples from third party analysis, and a lack of sequences data. The last human H5N1 sequences are from patients who died almost two years ago (January 2007).

Recently a large PCR confirmed cluster was reported in Makassar, and was reported to be lab confirmed was denied by the Ministry of Health. The denial was followed by the placement of a WHO team into the region to monitor H5N1 in poultry. The H5N1 situation in poultry has also been unclear since Indonesia has not filed an OIE report since 2006 and has halted shipment of poultry samples to the WHO regional lab in Australia.

These denials of lab confirmed cases and the withholding of samples and sequences have raised serious pandemic concerns.

Timely reports of confirmed cases and more interactions with third parties, including release of samples and sequences, would be useful.

The experts fear that the virus may develop into form that is more dangerous and spread through people killing millions of them around the world.
A WHO bird-flu handbook says, ?It is impossible to predict how lethal the pandemic strain might be. 'Modeling research using today's global population has projected that at minimum between 2 [million] and 7.4 million people might die?.
Since 2003, at least 71 people died from virus H5N1 in Asia. Other countries that reported deaths are Vietnam, Thailand, Indonesia, Cambodia and China.
Indonesia is now taking strong steps to prevent the spread of bird flu viruses.

Suspected birdflu patient dies


Tangerang (ANTARA News) - A suspected birdflu patient of Bintaro, Tangerang, Banten, known by her initials as Su (24) died on Tuesday.

Su's aunt, Sumiati, said before she died her niece was intensively treated at Sari Asih hospital in Ciledug, Tangerang.

She said Su initially had breathing problems, cough and a high fever and as her condition worsened her family then took her to the nearest hospital.

"However she finally died after being treated in the hospital for three days," she said.

Su's body was taken to Punggelan in Banjarnegara, Central Java, for burial.

The head of the Tangerang health service, Hari Heryanto, said his office could not as yet confirm if the cause of Su's death was birdflu virus.

"Her blood sample is still being examined and the result of laboratory tests is needed to confirm if the cause is really the virus," he said.

The district of Tangerang which is located west of capital city Jakarta has been known as an endemic area and recorded the highest number of birdflu cases so far totalling 17 that led to 15 deaths. (*)
