Reporters Get the Bird Flu Jitters

MEDAN, North Sumatra, Jun 23 (IPS) - ''I really feel strongly that the issue of health and safety of reporters covering avian flu must be addressed by the management of news organizations,'' said Daenk Haryono of the North Sumatra-based ?Harian Global' daily.

''Many times I've seen my colleagues go out to the field, not aware that they should be at least wearing face masks and gloves,'' he added. ''It seems like the editors just want our stories and couldn't give a **** about our safety,''


Haryono has every reason to worry. Two reporters have so far been admitted to hospital, suspected to be suffering from avian influenza or bird flu. The latest victim from the popular 'Tempo' daily was rushed to hospital last week when he developed high fever after covering the mass culling of chickens and the funeral of a bird flu victim in West Java.

Indonesia, the world's fourth most populous country has reported the world's highest number of human deaths from bird flu this year, with 51 cases to date and 39 deaths. The world total is 228 cases with 130 deaths. The lethal H5N1 virus is already endemic in birds in Indonesia.

In North Sumatra, the spectre of a deadly global flu pandemic was raised early this month when the World Health Organisation (WHO) admitted it was having difficulty identifying the source of bird flu infection in a case where seven people in the same family died.

It is Indonesia's largest cluster to date and has raised fears of human-to-human transmission of the H5N1 virus.

?'Good reporting is essential to educate the public about the risk of outbreaks in their poultry and how to protect themselves in the eventuality of a human pandemic,'' said David Swayana of the ?Harian Waspada' daily.

?'But news organizations should make preparations to protect their reporters in the field before they are sent out to report stories on the illness,'' added the journalist at a training seminar for news reporters organized by the U.S.-based media development agency Internews and the United Nations Children's Fund (UNICEF).

The Brussels-based International News Safety Institute has the following guidelines for news media staff covering stories on bird flu:

- Avoid direct contact with poultry that has no apparent symptoms, as well as with sick or dead poultry and any surfaces that may have been contaminated by poultry or their faeces or secretions;

- Wash your hands thoroughly and as frequently as possible and definitely after any potential contact. You should use soap and water where possible, or waterless alcohol based hand rubs when soap is not available and hands are not visibly soiled;

- You may want to use personal protective equipment like gloves, masks, overshoes;

- You must monitor your health for 10 days after your last exposure. If you become unwell with a fever, develop a cough or have difficulty breathing you must seek medical help immediately.

But to be fair, a few large media organizations in Indonesia do have guidelines for their reporters.

?'It is compulsory for all members of the news crew when they're out reporting in the field to wear masks and gloves,'' said Fitrianti Megantara of the Jakarta-based Trans TV. ?'It's written down in black and white for us to follow,'' she said.

Trans TV cameramen, too, according to Megantara, have been instructed by their management to keep their distance from victims and are excused from filming in houses where there have been deaths.


She said her TV station's journalists are also subject to a compulsory blood test for the H5N1 virus if they had been reporting in the field for a week or more.

While face masks are deemed as standard protection gear, many journalists complained that villagers often didn't want to talk to them if they wore one.


?'It's really difficult getting interviews if I were to wear a face mask,'' said Iir Sairoh of the West Java-based ?Radio Cindelaras'. ?'I seem to scare off people with it,'' she added.

But the radio journalist revealed that not wearing a face mask, each time she conducted an interview has a traumatizing effect on her.

?'It's okay when I'm doing the interview, with the adrenalin flowing to beat the deadline. But after I've filed my piece that's when I become scared,'' she told the seminar.

?'That's when I realise that I haven't had any protection and it just plays on and on in my head that I'm going to get sick and die,'' said Sairoh, while sharing her experiences with her colleagues.

Sairoh revealed that once she had a fever after returning from a village where there was a bird flu outbreak.

?'That was very scary and I told myself that if the fever persisted for another day I would go to the nearest hospital. Fortunately it did not,'' she said.
?'Maybe I'm just a hypochondriac,'' added Sairoh, laughing it off.

Prof. Luhur Suroso, the avian influenza team head in Medan's Adam Malik hospital, had this advice for journalists at the seminar. ?'Though human-to-human transmission of avian flu still has not been confirmed scientifically, you need to take precautions while covering the issue in the field,'' he reminded them.

?'There are basic measures you can take to reduce your chances of catching and spreading the disease,'' said the chest specialist. ?'Wearing face masks and gloves should be compulsory for reporters.''

Indonesia's response to a fatal bird flu cluster was littered with failings, according to international experts, and would have speeded rather than slowed the global spread of the virus had it involved a pandemic strain of H5N1.

In interviews with the Financial Times, international experts and local health officials detailed a catalogue of problems in the health system's response to the seven deaths in one family over the past six weeks.

The difficulties of Indonesia, which has suffered more human deaths from the virus than any other country in the past 12 months, mean it is often cited as a possible launching point for a global influenza pandemic should the H5N1 virus mutate into a form more easily transmissible among humans. Jakarta has asked for $900m (?494m) in grants over three years to finance a plan it submitted to an international donors conference in Beijing. But international experts argue $200m a year is far more realistic. More-over, many of the things Jakarta is asking for are things it should be prepared to pay for itself, they add.

"It's a little bit absurd. I don't know why people don't call them on this," said one expert. "What is this money going to be used for? It's going to be used to build government institutions that should be there in the first place. It's not [bird flu] specific. It's things . . . any civilised country should have."

With no laws to stop him, the father who died in the cluster checked himself out of the hospital and went into hiding even as epidemiologists were examining samples from him.

JAKARTA (Reuters) - The poultry death rate from the H5N1 avian flu virus in Indonesia, the country with the most human bird flu deaths this year, is worsening, possibly due to poor vaccination coverage, a senior government official said on Thursday.

Mathur Riady, director-general of livestock production at the agriculture ministry, said one million fowl, half of them quail, died of bird flu in the first three months of 2006.

In 2005, deaths for the year as a whole were 1.2 million.

"Based on the data, there is a tendency of a rise in bird flu-related deaths in poultry. It may be caused by low vaccination coverage, particularly in small farms and backyard chickens," Riady told reporters.

"But it may also be because the reporting system on poultry deaths is getting better."

The rise in poultry deaths caused by bird flu could increase international concern over Indonesia's effort to combat the virus, which according to the World Health Organisation (WHO) has killed 131 people across the globe since 2003.

In Indonesia, 40 people have died from the virus, endemic in poultry in nearly all the country's 33 provinces.

The government has so far shied away from mass culling, citing lack of funds and impracticality, with vaccination a preferred method to prevent the spread of bird flu among poultry.

Animal health experts say the large number of backyard farms and lack of resources are major impediments for effective vaccination in Indonesia, a sprawling country of 17,000 islands stretching across 5,000 kilometres (3,100 miles).

It is common for households in the country to keep a handful of chickens in their yards for personal consumption or extra revenue. The National Commission of Avian Flu Control and Preparedness estimates there are around 300 million such backyard chickens.

Riady said the agriculture ministry is holding a tender to buy 50 million vials of bird flu vaccine.

"The government has around 100 million vials but they are running out, so we are looking for addition of 50 million vials," he said.

The government will also expand door-to-door checks on poultry and birds -- which currently focus on the main island of Java -- to Sumatra and Bali, Riady said without elaborating.

Saying chickens are the only known source of bird flu infection in Indonesia so far, animal health experts Thursday urged the government to focus more on combating H5N1 among poultry.

Longtime veterinarian and physician Mangku Sitepoe said although veterinarians had found H5N1 in wild fowls, pigs, flies, birds and water, the source of all human infections in the country thus far had been chickens. [Dunno how he knows this??]

"After compiling and studying research done by veterinarians throughout the country, it is only sick chickens that have been able to transmit the virus to humans so far," he said.

Charles Rangga Tabbu of Yogyakarta-based Gadjah Mada University (UGM) supported Mangku's analysis, saying H5N1 in other animals had not yet mutated into a form that infects humans.


He argued the fight against bird flu should focus on reducing the virus' circulation among chickens. He urged the government to invest more human and financial resources in the animal health sector to stop the disease before it "spills over" to other species.

"The infection of other animal species should be halted as best we can," he said.

Experts believe some animals, such as pigs, could function as a so-called "mixing vessel", allowing H5N1 to mutate into a form that more readily infects humans.

Mangku also believes that the highly pathogenic virus might have already mutated into forms that are more easily transmissible within certain groups of humans.

"The virus has been circulating for some period of time here, it must have been changing into other forms," he said at a national veterinary congress here Thursday.

"I wonder why the Health Ministry is still denying it, while the World Health Organization has admitted it is possible," said the expert on animal-to-human diseases, who is also a member of the National Commission on Bird Flu.


Late last month the WHO stated that its investigation into a cluster of bird flu cases in North Sumatra, where the virus killed seven members of a single family in May, showed that H5N1 had probably passed between family members there but had not evolved into a more widely transmissible form.

The Health Ministry has consistently rejected the statement, saying that the virus transmission is still in the animal-to-human phase.

However, Mangku could not explain why, if the virus had mutated, it could only pass between blood relatives.

"I guess there is something related with genetics, but we ought to do some research on that," he said.

Charles, however, insisted that the virus had not mutated into a form that could more easily pass from human to human.

"What I do know is that the disease has become endemic among chickens raised on small farms and in backyards," said the dean of the UGM Veterinary School.

The Ministry of Health in Indonesia has confirmed the country's 54th
case of human infection with the H5N1 avian influenza virus.

The case, which was fatal, occurred in a 44-year-old man from East
Jakarta, Jakarta Province. He developed symptoms on 24 June, was
hospitalized on 10 July, and died on 12 July.

The case may have acquired the infection from poultry around his home
and animal health authorities have taken samples for testing. Another
potential source of infection was a local wet market, which he regularly
visited during the course of his work as the owner of a food stall.

Of the 54 cases confirmed to date in Indonesia, 42 have been fatal.

The bird flu virus has now reached easternmost Papua after attacking poultry populations in Java and Sumatra in the western section of the country.

About 174 chickens are believed to have died of the H5N1 virus in Mimika regency as of July 11, while 414 others were culled due to suspected infection.


Based on a lab analysis from the Bogor Veterinary Research Center conducted on July 19, three of 37 birds tested were positive for H5N1, an official said Monday.

"Test results showed that three of the chickens contain the H5N1 virus; one of them was from SP3 Timika Jaya and two were from the traditional market," head of the Papua Husbandry Office AR Pintadewa told The Jakarta Post in Jayapura.

According to Pintadewa, officers from the Mimika Husbandry Office received a report from a farmer in Sempan village, Timika, on July 11 that his chickens were sick.

They found 40 birds with H5N1 symptoms at the farm.

Inspections of the traditional market also found many chickens with symptoms of the disease.

"The place was immediately treated with disinfectant and chickens showing clinical symptoms of infection with the virus were culled to prevent an outbreak ..."

He said most vulnerable to the virus were the Indonesian Thai game variety, known locally as ayam Bangkok, that came from outside Timika. "We are still examining where they came from. The virus can be spread by air."

To prevent the spread of the virus to other areas, Pintadewa said the Mimika regent issued a circular banning transportation of chickens out of the regency.

In Medan, North Sumatra, three children from Karo regency admitted to Adam Malik Hospital in Medan after showing bird flu symptoms are believed to have been taken home by their parents.

Hospital director Nur Rasyid said the children were not in their beds when nurses made their afternoon rounds Sunday to administer medicine.

"We don't know where the patients and their families are now. They left without notifying hospital authorities."

Rasyid said the patients' families had requested their release since last Friday, after reports they tested negative for H5N1, but doctors refused because they had yet to receive the official results from the Health Ministry.

He said the medical team wanted to continue to monitor the children's progress and prevent a relapse in their condition.

"Medically, the patients' conditions have improved considerably, but one of them still needs serious attention due to a problem in his lungs."

The two female siblings and an 18-month-old male neighbor come from the same village of Sumbul, Kabanjahe district. They were admitted to the hospital on Aug. 1, after being referred from Kabanjahe Hospital.
