Denpasar (Bali Post) - RS Sanglah on last Friday (1/9) accepted the consignment patient from RS Tabanan that it was suspected was infected by the bird flu virus. The patient, mentioned GAA that still was eight months, according to his father, old has been feverish since six days (on Sunday (27/8)). When the first day the fever, he was at once brought to the community health centre took medicine. Until his medicine was finished, his fever did not descend also. Because of that on last Friday (1/9) GAA was at once brought to RS Tabanan. Berhubung the GAA environment close to the poultry, the side of RS Tabanan at once reconciled him to RS to get further help.

"Indeed his grandfather could buy the chicken from the Market Bringkit last Wednesday (23/8), for the Tilem need. Combine all other ingredients, except turmeric water, and mix thoroughly. Fill the inside of the suckling pig with this mixture, close the belly with string or thin satay skewers. Rub the outside of the pig with turmeric water until the skin is shiny yellow. Place the suckling pig on a roasting rack and roast in hot oven (220°C/425°F) for approximately one hour. Rest for 10 minutes in warm place before serving.
When serving, first remove the crisp skin with a strong carving knife, then loosen meat from the bones and cut into even dice or slices. Place a heaped tablespoon of stuffing on each serving plate, then top with meat and skin. Traditionally this dish is eaten with Jukut Nangka Mekuah and steamed rice.
Helpful hint:
If you have a large barbecue with a rotisserie or constantly turning spit, you can cook the pig over charcoal for an authentic Balinese flavour.

Among the delay of the co-ordination centre-area, the regency/the city currently becomes the prop to control bird flu. If not preceptive, this Garuda birds country could become the source of the world misfortune. All of us was convinced that will not happen at this time if and only if we wanted to work hard to know this virus.

Are they saying their swine AI is idenitical to the Sumatra (?Karo) cluster?
And that it has a2,3 receptor preference? Or are they saying it doesn't yet have full RBS capability, i.e., only 227?

BTW - what swine flus currently reside in Balinese swine? Any 228 or 226 mutations?

Indonesian H5N1 Swine and Bird Sequences Are Similar

Recombinomics Commentary

The recent meeting on human H5N1 bird flu in Indonesia included phylogenetic trees of HA and M genes. The HA tree identified two separate groupings of the human isolated, color coded in green. The Karo cluster was related to avian isolates. This group is shaded in pink. Another human isolate, IDN/06/05 is the second confirmed Indonesian human case and it is located on a nearby branch. However, the remaining human cases formed a separate branch, located at the bottom of the tree. The only non-human isolate on this branch is an H5N1 isolate from a cat, feline/IDN.CDC1/06. Like the other isolates on the branch, it had a novel cleavage site, RESRRKKR.

The presence of the novel cleavage site in human and cat isolates raised the possibility that the source of the human infections was mammalian rater than human. This first human isolate in Indonesia was in Banten in July 2005. At the time H5N1 was found in swine in Banten and a recent presentation included two H5N1 swine isolates from Banten, as well as 19 chicken isolates from locations throughout Indonesia. These are listed in a phologenetic tree and show that the swine sequences are similar to bird sequences in Indonesia. Moreover, the swine sequences have the common HA cleavage site RERRRKKR, which is in all but two 2003 isolates on the phylogenetic tree. Similarly, most of the Indonesian bird isolates in Indonesia are RERRKKR.

These data indicate the H5N1 in the swine in Banten are not the source of the H5N1 in humans in and around the Jakarta area. In the past year, the human isolates continue to point away from an avian source. There are now over 50 avian isolates collected between 2003 and 2005 in Indonesia. There are a few isolates with novel cleavage sites of RERRRIKR, RERRRIKK, RERRRK_R, and GERRRKKR. However, the vast majority of the bird sequences are RERRRKKR. In contrast, other than the Karo cluster and the second human isolate in Indonesia, all other human isolates form a separate branch on the tree and virtually all have an RESRRKKR cleavage site.

The failure to find a matching source for these sequences, other than one cat isolate, is cause for concern. This failure raises serious surveillance issues in Indonesia and raises doubts concerning transparency in WHO updates, which continue to point toward bird interactions, but fail to identify H5N1 in contact poultry and fail to find matching sequences in a growing number of bird isolates.

The human sequences continue to evolve, yet the sequences of the isolates remain sequestered in a WHO private database. The only HA sequence made public is the sequence from the first human case confirmed in Indonesia. That sequence was placed in the WHO database on August 1, 2005. The phylogenetic tree lists human sequences from over 25 individuals.

The time for release of these data has passed.

AI finding on pigs was due to some college students? research which diagnosed several ill pigs from May to June, 2006. Out of 20 pigs, two were positively infected by H5N1 virus. ?The pigs are in Gianyar and Tabanan,? said I Gusti Ngurah Mahardika, a professor at the faculty, Friday (6/10).

The virus grows in ill pigs and the pigs cannot be cured by a series of medical treatment. After surgery, there are red blotches on the spleen. By imono histokimia technology, the red blotches are viruses on the pig?s tissue.

However, H5N1 virus was not found in the animal?s tissue. ?Probably the virus only passed by, or is called an ?opportunistic? virus,? said Mahardika.

The finding has not yet been publicized as scientific study, but it has been conveyed to the Bali Breeding Service as a warning.

Virus contagion, according to Mahardika, is likely because with the pattern of chicken and duck breeding, the animals are free to enter pig stalls. In Bali, 900,000 pigs live side by side with other cattle.

Brief explanation: They apparently did autopsies of these sick pigs and found "red blotches" on the spleen. This is one of the most common findings of autopsies of all mammals (including humans) with late-stage bird flu (or Spanish Flu for that matter). The "blotches" are actually petechiae caused by internal hemorrhaging due to very low platelet counts. They performed immunohistochemistry on the blotches and isolated the virus, which just simply means that the virus is present in the spleen and thus the bloodstream. The "blotches" are NOT "viruses", as they say--in fact that doesn't even make any sense. Anyway, I hope that helps.
