There is no need to panic, indeed it will do no good whatsoever, but gird yourselves for a stressful set of reports coming out of Indonesia. Everything seems to be going wrong. In summary, the focus appears to be in three general areas right now.

The first is in the Medan-Karo-Karangrejo area--this is the area with the widely reported Karo cluster, followed in August by a large Karangrejo cluster of about 9--that was subsequently discounted, and now more cases being taken to Medan every other day.

The second is the southern part of the island of Sulawesi, in which there have been several groups of people, some confirmed, others suspected.

The third and most concerning however is the Garut-Tasikmalaya-Bandung area. In May and June there were two clusters, one involving a reporter you will recall, and the other with 4 dead siblings, suspected nurse, etc. This area blossomed into attention when the Garut cluster in Cikelet began in early August, in which several people died, and 20 or so were sick. They subsequently blanketed Cikelet with Tamiflu. Then, a family visited Garut and returned to Bandung, the two boys have now died, their sister is sick with what we're told is H1N1, their cousin San is now hospitalized, and their friend and neighbor Gugun, who used to stay with them, is now sick. Additionally, two other neighboring youth are very sick. Finally, in the district next door to Garut of Tasikmalaya, we have Atg and his/her son Mus hospitalized now. It remains to be seen if they have any relationship to Garut.

Indonesian health authorities ruled out human-to-human transmission of bird flu among three siblings, as the second of two brothers in the trio died, bringing the nation's toll to 52.

Taufik Zakaria, who was diagnosed as being infected with the H5N1 virus this week, died early yesterday, a nurse on the isolation ward of the Hasan Sadikin hospital in West Java's Bandung said.

Mr Zakaria's 25-year-old brother died last Sunday displaying symptoms of the virus, while the pair's sister had been hospitalised also showing H5N1-like symptoms, raising fears that transmission between people had occurred.

But authorities have said that both men appeared to have caught the virus from the same source, while a health ministry official said that the sister was ill with ordinary influenza and would not need further testing.

The 25-year-old was buried before samples could be taken for testing for the virus, while the 15-year-old sister "is doing better, her condition is improving'', said the nurse, who asked to be identified as Adam.

Runizar Rusin, head of the national bird flu centre, confirmed that the teenage girl, Intan Mustika, did not have the H5N1 virus.

"It's just an ordinary human influenza. From all medical and clinical aspects shown in her tests, it's clear that she has not contracted bird flu,'' he said.

"There are no concerns that would require additional testing be performed for her or that she could have contracted bird flu from her siblings.''

The World Health Organisation said in a statement posted late Wednesday on its website that she had initially tested negative for the H5 virus subtype and positive for the H1 subtype, indicating infection with normal influenza.

Indonesian health officials had said on Wednesday that further tests were necessary before she could be confirmed as being free of bird flu.

The case comes shortly after another bird-flu death in Thailand in which nine tests had be carried out before a 59-year-old man was officially confirmed as having died of the disease.

The man, from Non Bua Lam Phy Province in north-eastern Thailand, died August 10, Thai officials said, a full month after contracting H5N1.

The WHO and local officials have also said that the Indonesian brothers had come into contact with dead chickens while feeding their dogs, and household birds had been infected with the virus.

The fact they fell ill at almost the same time showed they had caught the virus from the same source rather than from each other, the bird flu information centre said on Wednesday.

So-called cluster cases, where the H5N1 virus is spread from human to human rather than from poultry increase the chance of the virus mutating to become easily transmissible between people.

Such a development, scientists fear, could lead to a global pandemic with a potential death toll of millions.

Indonesia is grappling with ongoing outbreaks of the H5N1 virus in its poultry and has reported 68 confirmed human cases, 52 of which have now been fatal the highest number anywhere in the world.

The virus has been detected in poultry across 30 of the archipelago nation's 33 provinces.

The national bird flu centre's Dr Rusin said that infections from backyard chickens was still the usual method of infection.

"It is difficult to change decades of habits among people in this country. Now we are trying to convey the message that you can contract bird flu and be killed by it,'' he said.

"We are optimistic that we can slowly change attitudes by widely distributing this message,'' Dr Rusin said.
