One Suspected Case of Polio Reported in Bihar Under Investigation

One suspected case of poliovirus has been reported in Darbhanga district of Bihar. The case is under detailed investigation. As a measure of abundant precaution, an immunization drive is being carried out in 28 districts of Bihar early next week.

The case is suspected to be of wild poliovirus type 3 (WPV3). Bihar has not had any case of WPV3 since January 2010. India has been polio free since 13 January 2011. The WPV3 strain is prevalent in Nigeria and Pakistan.

Further information will be shared when the results of the full investigations are available.

What seems to be unusual about this case in Darbhanga is that for some reason, the article suspects WPV3 specifically. Has some test come back positive? It would be a major setback if the wild virus were indeed found in Bihar after almost 3 years.

Could this actually be a re-imported case from elsewhere in the world? Or might it be a vaccine-derived strain (VDPV) which might initially have tested positive as a wild virus?

One suspected case of polio reported in Bihar under investigation

One suspected case of poliovirus has been reported in Darbhanga district of Bihar. The case is under detailed investigation. As a measure of abundant precaution, an immunization drive is being carried out in 28 districts of Bihar early next week [week of 15 Oct 2012].

The case is suspected to be of wild poliovirus type 3 (WPV3). Bihar has not had any case of WPV3 since January 2010. India has been polio free since [13 Jan 2011]. The WPV3 strain is prevalent in Nigeria and Pakistan.

Further information will be shared when the results of the full investigations are available.

[Given the official source of the information above -- the Government of India Press Bureau -- and the additional information that a wild poliovirus type 3 (WPV3) is suspected, it is highly likely that this case will be confirmed as a WPV3-associated case of poliomyelitis. As the press release notes, India has not had a confirmed case of WPV associated polio since 13 Jan 2011, 21 months ago. India had been removed from the list of polio-endemic countries earlier this year (2012), and was headed toward a polio-free status if there were no reports of WPV associated disease for 3 consecutive years.

Of note is that WPV3 has not been reported in Bihar for almost 3 years (last report January 2010). The mention that the WPV3 strain is prevalent in Nigeria and Pakistan, both still classified as polio-endemic countries, is most likely a prelude to the genetic studies on this WPV3 isolate to determine the most likely origin of this virus. If found to be a recently imported WPV3 from either one of these countries, this case may not be counted as an indigenous case, but rather a recent importation from a known polio-endemic area. Of note, during the period 1 Jan 2012 to 3 Oct 2012 there have been 17 cases of WPV3 identified in Nigeria and 3 cases of WPV3 identified in Pakistan (one of which was a mixed infection with both WPV1 and WPV3 identified)(see http://www.polioeradication.org/Dataandmonitoring/Poliothisweek.aspx). The date of most recent onset of WPV3 associated polio was 12 Aug 2012 in Nigeria and 18 Apr 2012 in Pakistan (see http://www.polioeradication.org/Por...ring/Wild_poliovirus_list_2007_2012_02Oct.pdf).

Polio Scare in Bihar, WHO Says Test Negative
New Delhi | Oct 11, 2012

It turned out to be a false alarm when a case of suspected wild poliovirus was reported from Darbhanga district of Bihar today, the country's first in 21 months, only to be found negative by evening.

Much to the relief of India's anti-polio drive which has helped the country remain polio-free for the last 21 months, the WHO after detailed examinations confirmed that it was not a case of polio virus.

"The suspected case of polio reported from Darbangha in Bihar has been found negative for wild poliovirus," a release from UNICEF said, adding that "The ERC Mumbai laboratory, where the samples were being investigated, confirmed the case as negative this evening."

The Government, which announced rolling out an immunisation drive in 28 districts of Bihar as a measure of abundant precaution after the suspected case was reported, called it off by the evening.

"Since the case has come out to be negative, there is no need for an emergency vaccination drive," said a top Health Ministry official...

Like alert, I'm curious as to why this one resulted in a national press release from the MOHFW.

Wow. Can we send this to ProMED?

There's something really odd about the way this was reported now. I still think there is a risk that a VDPV could initially test positive for WPV if it contained enough of the wild virus. I believe VDPV is often the result of reassortment between the vaccine virus and other enteroviruses, inlcluding WPV.

The suspected case of polio reported from Darbhanga in Bihar has been found negative for wild poliovirus.

Earlier in the day on being informed of the suspected case of polio, the Government had initiated rolling out an immunization drive in 28 districts of Bihar as a measure of abundant precaution, while carrying out detailed investigation of the case.

The ERC Mumbai laboratory, where the samples were being investigated, confirmed the case as negative this evening.

[The 1st report from the Ministry of Health and Family Welfare stating there was a suspected case of WPV3 associated polio in Bihar was released on 11 Oct 2012 at 17:29 IST; the above report stating that wild poliovirus infection had been ruled out was released on 11 Oct 2012 at 19:53 IST - approximately 2.5 hours later. In the interval between the 2 official press releases, ProMED-mail issued a report on the suspected case based on the information that WPV3 was the suspected etiology. It is not often that "false alarms" involve a mention of a specific WPV type. More information on the epidemiologic and laboratory investigations leading to the initial suspicion that this was due to a WPV3 infection would be greatly appreciated.

One senses a large sigh of relief with the final laboratory results, as India has not confirmed acute flaccid paralysis (AFP) cases due to WPV since January 2011 and is currently 15 months away from being certified as polio-free.
