The subsiding river will leave behind a host of challenges, both before the common man and district authorities. "The biggest threat is of infectious diseases," said additional district magistrate (finance) RK Pandey.

His concern gains weight from the fact that over 40,000 persons in the state capital have been affected by the river. Pumps will not be able to rid the affected areas of water completely, which will stay collected in potholes and pits, giving room to microbes to breed. Although , chief medical officer Dr AK Shukla said that elaborate plans were designed to tackle the situation, but people could save themselves from falling ill by taking simple precautionary measures.

Malaria and dengue top the list of diseases giving sleepless nights to health department. "Both the diseases can be life threatening because of their nature," warned Dr DP Mishra, senior physician at Balrampur Hospital.
He explained, people could save themselves from those diseases by reacting immediately after the water receded. "All they need to do is to fill potholes with mud or stones after pumping out water from them. He also suggested that people should use nets and wear full-sleeve clothes to cover their bodies during day-time .

With special mention to dengue, he said, "dengue mosquito breeds in fresh or rain water that accumulates in air coolers, old tyres, pots and so on." He suggested such conditions should be avoided. "The least a person can do is to pour oil or burn diesel on water. This will not let the mosquito breed," he said.

Gastro-intestinal diseases and infections like typhoid, virals and bacterial hepatitis are other dangers. "People should assume that water in flood-affected areas cannot be completely safe for consumption," said another doctor.
"To combat this group of illnesses, one must check the quality of drinking water. The best way out is to use boiled water even if it comes through the filter . But, if this sounds cumbersome, use of chlorine tablets can be an option," he said.
Doctors said although there was no sure shot way to escape simple gastro diseases , they could escape their seriousness. For example , one could avoid a serious GI infection, treating which might take months, by ensuring that the quality of water was safe. In the flood-affected areas, people could ensure cleaning of water filling system.
For instance, if they had been using a rubber tube for filling water, they could change the tube. If that was not possible, they could at least wash it thoroughly before using.

People in the affected areas also need to take care of their skin. The reason being insects would increase after the water was pumped out. Besides, the damp weather conditions also prove favourable for simple skin rashes and allergies. Government dermatologists suggested that people needed to take care of their feet especially , if they were living in the flood-affected areas.

"Walking in water makes the feet slightly numb. As a result, the sensation of insect bite is also not felt. This uncalled for ignorance can lead to late realisation of the disease. Nonetheless, damp feet invite infections caused by fungi and worms like ringworm," said Dr Talwar, a skin specialist and president of Indian Medical Association , Lucknow chapter.

:tiphat:Okieman..Comment: Please see map. Kailali is just across the border from Uttar Pradesh. Also please note this article is addressing disease in adults versus children. Many articles concerning this "mystery fever" make it appear that it is only effecting children, but adults are catching it also. Since it appears many different diseases are circulating in the region it may be certain ones are infecting children more (encephalitis) while others are infecting all ages.

Hundreds of People's Liberation Army personnel living in cantonments under the Kailali-based Seventh Division of the PLA are suffering from various diseases.
Over 1,000 PLA fighters are suffering from various diseases in division headquarters Talband alone, a source at the seventh division said.
According to Bikram Bahadur Singh, assistant health worker at the division, most of the patients are suffering from viral fever. He said some 200 PLA fighters were visiting the outpatient department of the division in Talband every day for treatment.
The government had posted a medical team headed by a doctor in the seventh division last year. The team attends to PLA fighters lodged in different sub-divisions.
"The government clinic in the division headquarters doesn't even have cetamol tablets," Dr Hari Kumar Shrestha of the government clinic at the division said. He said he had asked the district public health office (DPHO) for medicines, to no avail.
"The DPHO said it is unable to send medicines to the camps as it didn't even have medicines to distribute to government-run health posts. The DPHO says the government hasn't released budget for procuring medicines this year," Dr Shrestha added.
He warned that the disease might take epidemic proportions if medicines were not made available on time.
The condition of 60 women fighters in Talband is worsening, Dr Shrestha said, adding, "More than 140 nursing mothers in the sub-divisions are suffering the most. The condition of mothers and babies is worsening due to lack of nutritious food. Most of them are suffering from viral fever," Dr Shrestha added.

With thousands of people affected by diarrhoea, malaria, gastroenteritis,
and even a mysterious disease that has claimed at least 135 lives, Uttar
Pradesh has sounded a statewide health alert. "Since January [2008], the
state has been grappling with diseases like diarrhoea, gastroenteritis,
measles, malaria and such. Taking this into account, we have sounded the
alert across Uttar Pradesh," I S Srivastava, director general (medical and
health), told Indo-Asian News Service (IANS).

Doctors in the state will now make door-to-door visits in the affected
villages. Fogging and spraying of insecticides will be undertaken on a
war-footing, he added.

The Kanpur division is grappling with a mysterious disease that has claimed
about 135 lives in 4 weeks, officials said Friday [29 Aug 2008]. This
disease has symptoms similar to that of malaria and is accompanied by high
fever.

Since January 2008, diarrhoea has affected more than 7000 people, of whom
94 have died. Likewise, out of the 570 people infected by gastroenteritis,
15 have died, say officials.

In east Uttar Pradesh, acute encephalitis syndrome (AES) has infected more
than 900 people, of whom 160 have died. Cases of jaundice and kala azar
have also been reported, according to health records. The worst affected
districts include Kanpur Dehat (Akbarpur), Muzaffarnagar, Maharajganj,
Siddharthnagar, Mau, Azamgarh, Balrampur and Gorakhpur.

"Directions have been issued to the chief medical officers to disinfect all
sources of drinking water, especially wells and hand pumps," said Srivastava.

Rodentborne disease: leptospirosis and hantavirus infection
------------------------------------------------------------
We fully agree with Dr T Jacob John, when he says that immediate
serological testing for leptospirosis is warranted. With the scarce
clinical and epidemiological information we have already, the presumptive
diagnosis of a Japanese encephalitis (JE) outbreak seems rather improbable.
Sudden epidemics of a "viral disease" in warm and moist regions with poor
sanitation should always evoke the possible diagnosis of leptospirosis,
particularly if they occur after floods.

The wild rat is here the prime suspect, being the main (but not the only)
reservoir for leptospirosis. However, the wild rat is also the worldwide
reservoir of at least one known serotype of hantavirus, being Seoul virus
(SEOV). Double infections by both leptospirosis and SEOV have already been
described. In previous ProMED-mail posts, we have already pointed out to
the distinct possibility of a novel hantavirus in India, discovered but not
isolated yet in patients presenting with symptoms suggestive for
leptospirosis. It should be remembered that in similar episodes in Orissa
and Mumba, the serodiagnosis of leptospirosis was worked out indeed, but
not a potential concomitant infection by a hantavirus. It is striking that
in most reports of leptospirosis outbreaks in South East Asia (and even in
South America), the number of seroconfirmed cases surpasses rarely 50 per
cent.

Moreover, the main clinical characteristics of what we know from the
current outbreak in Uttar Pradesh are fever and CNS involvement. Both can
occur also in leptospirosis, as well as in hantavirus infections. Aseptic
meningitis is, together with renal involvement, the most common
complication of leptospirosis, but encephalitis may also occur. The same,
perhaps to a minor degree is true for hantavirus and dengue (Clement J,
Colson P, Van Ranst M. Dengue versus hantavirus CNS infections. Lancet
2000; 355: 2163-4). Leptospirosis and hantavirus infections, although
resulting from totally different pathogens, have not only most clinical
symptoms, but also most laboratory anomalies in common. Both are announced
by thrombocytopenia, the severity of which is an indicator of the clinical
complications to come (Clement J, Maes P, Van Ranst M. Acute kidney injury
in emerging, non-tropical infections. Acta Clin Belg 2007; 62: 387-95).
Thrombocytopenia is not a consistent sign for JE. Early, marked, but
transient proteinuria is the most consistent sign however in both early
leptospirosis and hantavirus infections.

Even in the New World hantavirus infections, which are (wrongly) supposed
to target only the lungs, early proteinuria is found in > 90 per cent of
cases. With the restriction in mind for a minimal degree of proteinuria in
high fever cases (probably even more in infants), a detection of + to +++
degree of proteinuria is without doubt the quickest, simplest, and by far
the cheapest bedside test for a first tentative diagnosis of leptospirosis
and/or hantavirus infections. When urine dipsticks are not available or too
expensive, even the good old urine boiling test might be considered. The
fact that only (?) infants seem affected is the only serious argument
against the tentative diagnosis of both these rodent-borne infections, and
would point rather to a dengue hypothesis.

For reasons still ill understood, hantavirus cases under the age of 15 are
definitely rare in most series. But then, IF there is really a novel
hantavirus roaming around in India, it could as well be a form having the
brain in youngsters as main target organ. Let us not forget that nobody
expected to see emerging in the USA in 1993 a "new disease," affecting
mainly the lungs in healthy young adults, and later called "hantavirus
pulmonary syndrome." Health authorities in Pune tried virus culture to
track the elusive cause of the current outbreak, and expected "results
within 2 or 3 days of collection of the samples."

However, during JE outbreaks, virus isolation is rarely successful, because
the viraemic phase is probably over when CNS symptoms start to appear. The
same goes for hantavirus infections, at least for the milder (Puumala
virus-induced) forms in Europe and Russia. Moreover, hantavirus isolation
is always a tricky affair, needing several time-consuming passages on VERO
E 6 cells in labs with a BSL of at least 3. Finally, since hantaviruses do
not induce a CPE in culture, they can only be detected by selective
techniques such as IFA with specific antisera, or PCR with specific
primers. For a putative novel hantavirus, both are lacking.

Doctors team visit encephalitis-hit Gorakhpur

A team of doctors deputed by the Union Government, including 2 from the
United States, visited Gorakhpur district of Uttar Pradesh today to examine
patients suffering from encephalitis. The team, along with the 2 doctors of
the US Centers for Disease Control and Prevention, Atlanta (CDC), visited
Gorakhpur to examine patients and study the virus that causes the disease.

The fatal vectorborne disease spreads through viral infection and has
already claimed 171 lives during this season. "From 1 Jan 2008, records
shows that 851 people were admitted out of which 171 people have died,"
said Lalit Mohan, a doctor at the local BRD hospital to reporters. Most of
the patients fall in the age group of 6 months to 12 years. The death toll
due to disease is rising and experts have expressed serious concern.

The disease causes inflammation of the brain. The initial symptoms of the
disease are recurring fever and vomiting. Experts feel that parents as well
as the local doctors have failed to understand the seriousness of this mild
fever, which can prove to be fatal.

The disease is endemic to the region and experts say that immediate
measures are required to contain the virus from spreading.

because the affected locations in the former are "13 districts of eastern
Uttar Pradesh" while the location of the latter is Kanpur Dehat District
which does not belong to eastern Uttar Pradesh.

However, Part (2) of the former conveying Dr Jacob John's comments is most
probably related to the latter.

:tiphat: I see stories about diarreah,measles,polio,cholera,unknown fevers,malaria and disease from unclean water ALL ON THIS MAP! :surrender: A dam has broke in nearby Bihar and some flooding is also in these areas on the map, plus there has been rain. Animals are being vaccinated,athough i don't see poultry specifically mentioned,i do see a story of people cooking food next to animal waste and also people drinking river water in Bihar. I will try putting a map of the province where the story is from to try and help us keep the diseases straight.Some of the words don't translate at all and alot of the stories post the names and ages of the victims,so i will edit them out as they add to the confused state I already am in!..I have posted stories of drs not shoeing up ,lack of drs, lack of meds,l have posted stories of drs not showing up,meds not avaliable,lack of drs,lack of concern and nobody is certain of anything at all. Kanpur, Aug 29: A four-member team of experts from Delhi arrived here today to take stock of the spread of Malaria and viral fever in Kanpur Dehat.

According to information received here, Dr Sunil Gupta and Dr Jagbir Singh from National Institute of Communicable Diseases and Dr BR Thapa and Dr Anupam Prasad from National Institute of Vector Borne Diseases inspected some of the villages hit by Malaria and viral fever.

Speaking to PTI, Additional Director Health, Kanpur Dehat Dr SP Singh said, "In the last 20 days, 34 deaths due to Malaria and viral fever have been confirmed in 139 villages of the district and about 650 people have tested Malaria positive." The AD Health also added that some cases of Hepatitis (E) have also been detected in the district.

Meanwhile, the district administration has undertaken massive cleanliness drive to avoid further spread of the disease.

Aug. 31: It happens every year ? year after year.

Japanese Encephalitis (JE), the dreaded virus that claims hundreds of lives each year in eastern UP, has struck against. Over 190 persons, mostly children have lost their lives this year with four more deaths being reported on Saturday.

The victims included two children ? one each from Ballia and Ghazipur. They had been admitted to the Baba Raghav Das (BRD) Medical College in Gorakhpur earlier last week.

According to official sources, the number of patients afflicted by Japanese Encephalitis since January this year is 1,029, of which 190 have died.

Almost 169 patients, including 16 fresh cases, are in remission. The JE patients come in from Gorakhpur, Deoria, Maharajganj, Sant Kabirnagar, Kushinagar, Ballia, Gonda, Bahraich, Mau, Basti, Siddharthanagar, Azamgarh, Gonda and Balrampur districts of UP, with Gorakhpur being the JE epicentre in the region. The endemic afflicts eastern UP during monsoon every year.

Last year, the killer disease had taken almost 1,000 lives.


The state government has been undertaking a massive vaccination drive against the vector-borne disease every year but the continuing toll raises a question mark on the efficacy and penetration of the drive.

LUCKNOW, AUGUST 31: After the state Government?s failure to isolate the viruses responsible for Acute Encephalitis Syndrome cases in Gorakhpur, the Centre has sought USA?s help.
On the invitation of the Government, a team of two irology experts from Centre for Disease Control and Prevention, Atlanta, are camping in Gorakhpur from August 28 and collecting blood samples for testing in their own laboratory.

Scientists at Pune-based National Institute of Virology found that unlike Japanese Encephalitis, which is a vector-borne disease ? spreads through mosquitoes ? the AES cases were caused by enteroviruses, which spreads through the oral-fecal route.
There are thousands of enteroviruses, and those responsible for the problem in Gorakhpur could not be identified. Experts from the US are expected to help in the identification of the viruses and check whether the present line of symptomatic treatment given to the patients is correct.
The experts are supposed to compile their report in America and give it to the Centre along with their recommendations.
?The Indian Council for Medical Research (ICMR) has a long time understanding with CDC and they have been assisting us in various projects. Thus, we thought to take their help in this case as well,? said Dr M M Gore, a senior scientist from National Institute of Virology, Pune.
On Sunday, the experts visited the houses of some of the patients to see the environment in which most of the patients were living. They also visited the BRD Medical College, Gorakhpur, to check the treatment given to the patients registered under AES.
?We have high hopes from these experts. Year after year, we have been watching large number of children being affected with the virus and we have no other option but to provide them with symptomatic treatment,? said Dr Lalit Mohan, principal of BRD Medical College. ?We hope that they will help us in identifying the clinical spectrum of the disease and suggest if there can be any better line of treatment,? added Mohan.
