India reported its first confirmed case of monkeypox on Thursday, a 35-year old man with a history of travel to the Middle East, government officials said. The man, who travelled from the United Arab Emirates to the southern Indian state of Kerala on July 12, was in stable condition and had been isolated at a hospital, the state's health minister, Veena George told reporters late on Thursday.

"He is stable and all his vital signs are normal. We have asked all districts to be on alert," George said. Monkeypox, endemic in parts of Africa, is a viral disease that causes flu-like symptoms and skin lesions. Around 60 countries in which monkeypox is not endemic have reported outbreaks of the disease, with confirmed cases crossing 10,500.

George said the patient's parents and other primary contacts have also been isolated while 11 passengers who came in contact with him on his flight and some others have been told to watch for symptoms. The federal government said in a release that it was sending a high-level team to the state to work with local authorities and recommend necessary public health measures...

A suspected case of monkeypox was identified in Telangana. A 40-year-old person from Kamareddy who has returned from Kuwait on July 6 developed rashes on skin and fever.

The person has picked up fever on July 20, and rashes on July 23. The patient consulted a doctor at a private hospital in the district. Upon seeing symptoms of monkeypox, the doctor has directed the patient to Government District Hospital.

The patient was rushed in an ambulance from Kamareddy on Sunday night to be admitted at Sir Ronald Ross Institute for Tropical and Communicable Diseases at Hyderabad on Sunday evening. The government health facility is prominently known as Fever Hospital.

Superintendent of Fever Hospital K. Shankar said that the patient’s condition is stable. Five forms of samples would be collected from the patient, which includes blood, urine, from lesions and swab from throat. Director of Public Health G. Srinivasa Rao said that samples will be sent to National Institute of Virology (NIV) Pune for the tests.

The senior health official said that six people who were in direct contact with the suspected case were identified. “None of them have symptoms. However, they were isolated. People do not have to panic about monkeypox. This is not a fatal infection,” said Dr. Srinivasa Rao.

Monkeypox strain detected in India not linked to Europe outbreak
india news
Published on Jul 30, 2022 12:14 AM IST
To be sure, there is no evidence yet that suggests either of the strain may be more infectious or virulent.
By Soumya Pillai, New Delhi

Genome sequencing reports from two monkeypox patients in Kerala suggests they were infected with the A.2 strain of the virus, different from the one causing the monkeypox outbreaks in Europe, an analysis by scientists at CSIR Institute of Genomics and Integrative Biology (CSIR-IGIB).

This A.2 strain, which has largely been found in the US and Thailand, has not been linked to major cluster or super spreader events, unlike the B.1, which has been found in large parts of Europe.

To be sure, there is no evidence yet that suggests either of the strain may be more infectious or virulent.

Vinod Scaria, genome sequencing scientist at IGIB, said that genome sequencing data from two monkeypox patients from Kerala belong to the A.2 version of the monkeypox virus, in contrast to majority of the genomes across the world which belongs to B.1 lineage.

Of the four confirmed cases of monkeypox in India, these are the only two samples that have been sequenced for now, officials confirmed.

“Genome sequences have been deposited for two samples (EPI_ISL_13953610 and EPI_ISL_13953611) along with two re-sequenced genomes from isolates of one of the sample. Both the isolates were from early cases reported from Kerala and both cases have a travel history,” Scaria said.

Scaria said that around the world, there are only a few cases that belong to the A.2 strain and these cases, including the ones from India, seem to all have travel links to Middle East or West Africa.

“The earliest sample in the cluster from US is indeed from 2021 suggesting the virus has been in circulation for quite some time, and earlier than the European events,” he added.

The samples from the monkeypox patients were collected by the National Institute of Virology in Pune, and the genetic sequences of the monkeypox virus has been uploaded on a public database for researchers, GISAID—a global initiative for real time communication for disease prevention. ..

The National Institute of Virology (NIV) in Pune under ICMR has isolated monkeypox virus from the clinical specimen of a patient which can pave the way for development of diagnostic kits and vaccines against the disease, the apex health research body said on Wednesday.

With India isolating the virus, the Indian Council of Medical Research (ICMR) also invited expression of interest (EOI), proposing to hand over the strain to interested Indian vaccine manufacturers, pharma companies and in-vitro diagnostic industry partners for development of indigenous vaccines against monkeypox and diagnostic kits for detection of the disease.

ICMR isolates monkeypox virus; invites companies to develop vaccines, diagnostic kits

The development comes amid India reporting four cases of monkeypox -- three from Kerala and one from Delhi -- so far.

"The National Institute of Virology has successfully isolated monkeypox virus strain from the samples of infected Indian patients which may help in the development of diagnostic kits and also vaccines in future," Dr Pragya Yadav, a senior scientist at NIV, told PTI. "For smallpox live attenuated vaccine was successful for mass immunisation in the past. Similar approaches on new platforms can be tried for making vaccines against monkeypox. The virus isolation enhances India's capacity to do research and development in many other directions," she said.

5 monkeypox cases in Delhi without int’l travel history suggest under-diagnosis of infection: Report
The study recently published in the Journal of Medical Virology has noted that the detection of monkeypox cases in Delhi without travel history to affected areas suggests the undetected circulation of the virus in the community.

Newsguard

A new report has described five cases of human monkeypox infection detected in New Delhi without any international travel history, suggesting under-diagnosed monkeypox infection in the community. Researchers from Maulana Azad Medical College in New Delhi, Indian Council of Medical Research – National Institute of Virology in Pune and AIIMS, New Delhi have emphasised the need for active surveillance of monkeypox virus (MPXV) in the high‐risk population such as men having sex with men and female sex workers.

On July 14, this year, India reported its first human monkeypox case from Kerala. Since then, 20 confirmed cases of monkeypox have been identified from Kerala (6) and New Delhi (14) till October 13, 2022. The study, published in the Journal of Medical Virology in October, noted that the cases observed in Kerala had well-defined epidemiological linkage, with international travel history and contact with suspected cases. The detection of monkeypox cases in Delhi without a travel history to endemic or affected areas suggests the undetected circulation of MPXV in the community.

“The confirmed monkeypox cases from New Delhi have no travel history to monkeypox endemic or current outbreak areas,” researchers said further adding that monkeypox cases were mild and recovered completely. “This emphasizes the significance of timely clinical and laboratory diagnosis for early detection and patient management leading to recovery without secondary complications,” lead author Dr Vineet Relhan from Maulana Azad Medical College has said in the report...
