Muzaffarpur, Jeanan . Due to high heat and moisture for a long time this year, AES has taken a vivid form. The number of children suffering from this disease has increased over the last few years. So far, 132 children have died due to this disease whereas 440 children have been admitted in hospitals in the district, whose treatment is going on.

In the year 2014, 342 children were ill with this disease. Of these 86 children were killed. In 2012, 120 children died from this disease. However, 336 children were then ill

It is being said by the government that we have succeeded in protecting many children from the disease. This number is more than hundred. But, seeing the figures of the last 10 years, the rates of children suffering from the disease were reduced. In 2014, there was considerable hazard of the disease, 75% of the sick children were rescued.

At the same time, in the next two years in 2015 and 2016, about 85 percent of children survived. But, this figure decreased this year. So far we have saved about 75 percent of the children. Alerting this figure of sudden increase in death. Where was the lapse?

The strategy that Union Health Minister Dr Harsh Vardhan created in 2014 has been implemented for the next two years. Keep awareness campaign continuously. The protest took place outside the state-run Sri Krishna Medical College and Hospital (SKMCH), where Nitish Kumar arrived with Deputy Chief Minister Sushil Kumar Modi.

The protesters, annoyed over poor medical treatment and lack of facilities, shouted slogans like "Chief Minister wapas jao (Go back Chief Minister)".

Bihar government to bear treatment expenses of encephalitis patients

Patna (Bihar) , June 17 : Bihar government will bear the treatment expenses of all encephalitis affected patients, said Chief Secretary Deepak Kumar on Monday here.

"The ambulance facility will be free of cost. If someone comes to the hospital in a private vehicle, the fare will be reimbursed to them," he said, while addressing media persons.

People's displeasure was also about lack of facilities in the hospital. The family alleged that the children are not being treated properly. Nitish remained in SKMCH for about one and a half hours. There are seven ICUs in the hospital. The Chief Minister visited everyone and visited the children.

Along with my colleagues, I had investigated the so-called mystery disease in Muzaffarpur, Bihar, during its outbreak in 2012, 2013 and 2014. The local name for it was acute encephalitis syndrome, but we found that the disease was not encephalitis but encephalopathy. This distinction is important. Encephalitis results from a viral infection, unless proved otherwise. The pathology is primarily in the brain. Encephalopathy is a biochemical disease, unless proved otherwise. The primary pathology is not in the brain. Specific treatment is scanty for viral encephalitis, but encephalopathy is eminently treatable.

Hypoglycaemia (when the level of glucose in the blood falls below normal) is usually due to an overdose of insulin in children with diabetes. It is easily corrected with oral sugar or intravenous glucose. The easily available 5% glucose solution suffices. Hypoglycaemic encephalopathy, however, is different from simple hypoglycaemia.

This disease was reminiscent of the Jamaican Vomiting Disease, a form of hypoglycaemic encephalopathy. It is triggered when unripe ackee fruits are eaten. These fruits contain a substance, methylene cyclopropyl alanine, which blocks a biochemical process called fatty acid oxidation, or gluconeogenesis.


There are two essential steps: gluconeogenesis is turned on and is then blocked midway by methylene cyclopropyl alanine. The back-up molecules of the unfinished process are certain amino acids that are highly toxic to the brain cells. Ackee and litchi belong to one plant family. My toxicology colleague, Dr. Mukul Das, found generous quantities of methylene cyclopropyl glycine in litchi fruit pulp.

The disease affected only malnourished children between the ages of two and 10. A majority of them were from families camping in orchards for fruit harvesting. No child from the nearby towns fell ill. Children of well-to-do families never fell ill.

Litchi harvest usually begins by 4 a.m., which means that families are awake before that. They go to sleep early. If children go to sleep without dinner, parents usually do not wake them up and feed them. Litchis are collected in bunches and sent to the collection points, but single fruits fall to the ground. Children are free to collect and share the fruits with their friends.

With this information we made the hypothesis that the disease was hypoglycaemic encephalopathy. Along with my paediatric colleague, Dr. Arun Shah, we conclusively showed that the disease was indeed hypoglycaemic encephalopathy. With all the pieces in hand, we reconstructed the disease pathway.

We were unable to demonstrate aminoacidaemia in children with hypoglycaemic encephalopathy, but that was done by investigators from the U.S. Centers for Disease Control and Prevention (CDC). The only missing piece in our studies was filled in by CDC colleagues.

In 2015, all primary health centres were supplied with glucometers to check the blood glucose levels of sick children. Doctors were instructed to take a blood sample for glucose estimation and, irrespective of the results, infuse 10% glucose intravenously. To correct mild hypoglycaemia, 5% glucose is enough, but here the problem is not hypoglycaemia alone, but aminoacidaemia as a result of blocked gluconeogenesis. To prevent any further back-up amino acid from accumulating, the fatty acid oxidation process has to be turned off quickly. That requires raising blood glucose level to abnormally high levels so that insulin secretion is stimulated, and that in turn turns off the gluconeogenesis.

What Dr. Shah and I found was that if ill children are infused with 10% glucose within four hours of onset of brain dysfunction, recovery is fast and complete. If only 5% glucose is given, or if 10% glucose is not administered within four hours, recovery is unlikely. I do not have detailed information from the field, but there seem to have been some human slip-ups this time.

Glucometers have not been maintained well. Health education was not sustained. New doctors are not familiar with all the information. Instead of 10% glucose, 5% is given. Children are taken mostly to private clinics and are then referred to the Sri Krishna Medical College in Muzaffarpur city since ambulance services are free of cost and easily available. Ambulances take more than four hours to reach the city hospitals from many rural clinics. We might think each error is minor, but when all the errors add up they contribute to deaths that should have been averted.

Muzaffarpur (Bihar) [India], June 19 (ANI): In the wake of the growing number of deaths due to Acute Encephalitis Syndrome (AES) in Bihar's Muzaffarpur district, the State Health Society has deputed more doctors from neighbouring districts.

The death toll due to encephalitis rose to 112 on Wednesday, officials said. As many as 93 children have died at Sri Krishna Medical College and Hospital (SKMCH) while 19 others lost their lives at Kejriwal Hospital.

The State Health Society had on Tuesday ordered to delegate more doctors in Muzaffarpur from Darbhanga, Supaul, Madhubani, Samastipur and Sitamarhi districts.

Raipur: Chhattisgarh Chief Minister Bhupesh Baghel announced on Wednesday that tribals living in the forest regions of Bastar would soon get free nutritious food through gram panchayats and access to medical facilities.

Baghel said the state government would be launching two new schemes for tribals, related to malnutrition and healthcare facilities. These schemes will play a pivotal role in improving the health status of tribals.

The directive has come in the wake of reports that encephalitis is spreading in litchi growing areas of Bihar, including Muzaffarpur district and other parts of the country and that consumption of the fruit was among the factors for the spread of the disease.

Doctors and health officials have different views on the factors behind AES epidemic and the cause of the deaths. This confusion has further complicated the process to combat or control the seasonal outbreak of AES, which has been recurring every year.

Bihar Chief Secretary Deepak Kumar has publicly admitted that even the government is unclear about what exactly was causing the AES outbreak, which has been recorded regularly in Muzaffarpur since 1995.

"We are still not aware if the disease is caused due to some virus, bacteria, toxin effect due to consumption of litchi, malnourishment or due to environmental conditions such as high temperature and humidity," he said.

"Several researches have been conducted, including by a team of experts from the Centers for Disease Control and Prevention, Atlanta (US), but the finding is inconclusive".

SKMCH Chief Medical Officer S.P. Singh said the reason behind the outbreak of the disease was yet to be confirmed. "We don't know it and it was being ascertained medically", Singh said.

Union Health Minister Harsh Vardhan, himself a doctor, who had visited Muzaffarpur three days ago and examined several children in SKMCH, said the deaths could have something to do with electrolyte imbalance and the metabolic system.

AES in Muzaffarpur broke the record of death of 120 children in 2012 Five more children died in SKMCH Wednesday morning. So far, 125 children have died in the hospitals of Muzaffarpur. In Bihar, the figure has so far reached 145 this year. So far 473 children have been admitted in various hospitals. 336 were admitted in the year 2012.

Today only five children have died. Families of children have become aware, due to which the health of the children is recovering quickly now.

MUZAFFARPUR: Untrained doctors who don?t have the skill to handle critical equipment in intensive care units, and the lack of an awareness drive because of the Lok Sabha elections in April-May, could be behind the sudden spike in the deaths of children due to suspected Acute Encephalitis Syndrome (AES) in this district.

A seven-member team of experts from the All India Institute of Medical Sciences (AIIMS) at Delhi and Patna, which visited the Shri Krishna Medical College Hospital (SKMCH) here to study the high rate of deaths, blamed the hospital?s doctors in its report for not being able to operate equipment such as CT scan machines, ventilators and blood gas analysers.

Accordingly, eight additional advanced life support ambulances (ALS) have been pressed into service for transportation of critical patients under National Health Mission (NHM). The central teams of 10 pediatricians and five para-medics sent yesterday, have been deployed in the field and have started working in coordination with the state government. Dr. Harsh Vardhan further added that sixteen senior district officials and medical personnel have been sent to the vulnerable blocks for monitoring and early identification of cases and ensuring daily reporting. Their headquarters have also been shifted to these blocks also, he added[/FONT][/FONT] [FONT=&quot][FONT=&quot]Dr. Harsh Vardhan said that the District Collector (DC) has now got involved and is personally taking responsibility. He has deputed the Senior Deputy Collectors (SDC)/District level officers to lead various block teams. Surprise checks are also being conducted at the PHCs by the Senior Deputy Collectors (SDC). Additionally, the early morning inspection of Anganwadi Centres is also being undertaken by SDCs and Child development Project Officers (CDPOs). The ANMs and ASHAs have been directed to visit every house for raising awareness about AES/JE by distributing pamphlets and brochures. According to statistics released by the department, the total number of Acute Encephalitis Syndrome (AES) cases registered across the state since June 1 was 626.

Muzaffarpur district has bore the maximum brunt, accounting for 117 deaths so far. Besides, deaths have been reported from districts like Bhagalpur, East Champaran, Vaishali, Sitamarhi and Samastipur.

The department also said that among the various complications that are covered under the umbrella term AES, hypoglycemia or very low blood sugar level was the most prevalent as it has afflicted 520 children and accounted for 110 deaths.

After Bihar, now Chattisgarh has knocked on the acute encephalitis syndrome i.e. sperm fever.

The first case of death has been revealed in the Bastar district of the state with spinal fever. The child has died from this disease in Jagdalpur of Bastar. The name of the deceased child was Bhuvan, whose age was 4 years.


According to the information, the children of 4-year-old Bhuban were admitted to Dimarpal Medical College Hospital in Jagdalpur in Bastar district. Symptoms of Acute Encephalitis Syndrome were found in Bhubeni's Investigation Report. During treatment on Thursday, Bhuvan breathed his last. So far, samples of four children have been examined in the hospital, one of which has confirmed fever. It is being told that in addition to Bhavna, two more girls suffering from sperm fever were admitted to the hospital. In which one has been discharged after being healthy and one is undergoing treatment, whose condition is under control.


Here the hospital management has prepared for the prevention of this fever. In the state, there has been a stir in the health department after the death of a child born with a sparkling fever. A team of doctors from the capital Raipur has been sent to Bastar. It is worth noting that the fever of bark in Bihar has so far claimed the lives of 135 children.

The Jharkhand health department has swung into action after test report of a 35-year-old man in Dumka came positive for Japanese Encephalitis (JE), said officials on Thursday.

The patient, identified as Ramlal Sharma, is being treated at Rajendra Institute of Medical Science (RIMS) in Ranchi.

The state vector borne disease control programme (SVBDCP) nodal officer, Dr Vijay Nath Khanna, said out of two JE suspects admitted at RIMS, report of one patient came positive.

The Acute Encephalitis Syndrome (AES) has claimed the lives of 158 children in Bihar, plunging the state in a grim crisis. The deadly outbreak of the disease has gripped the children of the state since the beginning of this month. The worst-hit district is Muzaffarpur with the viral disease taking the lives of 122 children, followed by Vaishali where 17 children have lost the battle with AES. Another six children have lost their lives in Begusarai district, five in Samastipur, two each in Motihari, Patna and Bettiah and one each in Bhagalpur and Gopalganj.

Gorakhpur In the four districts of the Mandal, there were 16 deaths from Japanese Encephalitis (JE) and Acute Encephalitis Syndrome (AES) till June 19. Most six-six deaths have happened in Gorakhpur and Kushinagar. According to statistics, the highest number of patients were found in Kushinagar and Maharajganj. The number of patients in Deoria is low.

According to the official data of Additional Directorate Health and Medical Gorakhpur Division, between May and June 19 in Gorakhpur, 130 patients of JE, AES have been found. The number of people who died with the recruitment patient till 20, 21 and 22 June will also be told to the government soon.

80% of the patients found in four districts of Gorakhpur Division are being treated at BRD Medical College. According to the Medical College Administration, public awareness, cleanliness and vaccination have reduced the outbreak of JE, AES. The knock-out campaign is beginning on July 1. Under this, children will be vaccinated to JE. Vaccination campaign will run until July 25. It will also have positive results. Vaccination has reduced the number of patients in JE.
More danger after 15 days of rain
After the rain, the number of JE, AES grip, is increasing rapidly. There is no season yet, even though the patients have increased in four districts of the Mandal. There is a possibility of an increase in numbers after the rain. Healthcare is only on alert to deal with this. Chief Minister Yogi Adityanath will also review the preparations for JE, AES treatment in BRD Medical College on Saturday. A meeting of officers of Gorakhpur-Basti Mandal has been convened in this regard.
