On the brink of closure of department for rehabilitation of encephalitis patients in Gorakhpur

BY Manoj Singh ON 28/07/2017 ? Your Response

11 employees of the department for the treatment and rehabilitation of patients with encephalitis were not paid for 27 months; the salary, the three physicians and the stenographer left the job.

For the treatment and rehabilitation of physically and mentally challenged patients with encephalitis, the Department of Preventionive Medicine and Rehabilitation (PMR), made in BRD Medical College Gorakhpur, can be closed at any time as 11 medical personnel working here have not received salary for 27 months. is. Due to lack of salary, three doctors and stenographers have left the job.

The 'Wired' news published on July 20 of 378 medical workers who did not receive salaries and one year salary increase for four months from Encephalitis in BRD Medical College.
(see post #69 in this thread - Ro)
After this, the news of the medical personnel of PMR Department not getting salary for 27 months is a surprise. This condition is when Chief Minister Yogi Adityanath himself visited the medical college on July 9 and had prepared his own details of preparedness and deficiencies regarding the treatment and treatment of encephalitis.

The Central Health and Family Welfare Ministry gave fund to the establishment of PMR Department in BRD Medical College in February, 2010. This department was started in 2011. Given the large number of disabilities from encephalitis, it was decided to establish this department.

More than 30 percent of disability in Japanese encephalitis and acute encephalitis syndrome (JE / AES) is found. That is, more than 30 percent of those whose lives save from this illness get physically and mentally disabled. Multiple children are found to be multicolored. In addition to insufflation, there is a large number of disabilities in this area.

The establishment of this department was to get great help in the treatment and rehabilitation of patients with encephalitis, but soon the center and the state government started fluttering from this department.

37 posts were created for the department, including four doctors, but here only 17 posts were recruited. It includes a professor, an associate professor, a house surgeon, four therapists (physiotherapy therapists, auctive therapist, clinical psychologist and speech therapist), a stenographer, four orthorthist and prosthesist and five-fourth class employees.

This appointment was also done on contract. Appointment of staff nurses, technicians, medical social workers etc. has not been approved, whereas the approval of all the posts will be given only one year's salary of 1.48 crore.

After deduction of the posts, the Center and the State Government started having problems in paying salaries. The Central Health Department began to say that its work was only for the establishment of the department and to provide the equipment. Now run the state government department and pay salaries to medical personnel. The state government has not included the expenditure of this department in its budget till date.

This is the reason that after the appointment, the medical workers could not get the salary for nearly two years. It started disillusioning with the medical personnel, especially the Department of Doctors, and one year after three doctors left the job. After this, Steven Grafer left the job.

Somehow two year salary was mixed in March 2013. Then the salary of two years was found in March 2015. At present, there are 11 medical personnel (four therapists, two economists and provenists and five-fourths) working in the department. After this, 27 months have passed, they have not received the salary.

There is no annual increase in their wages, but the medical personnel of the PMR department working in Meerut and Lucknow Medical College are getting increment each year.

How important is this department, considering the number of patients coming to the treatment can be applied. In 2012, 1484, 1775 in 2013, 2294 in 2014, and in here 2015, 5750 patients came for treatment.

In seven months this year, 3550 patients have come for treatment. If all the posts are sanctioned and filled in the department and the medical personnel gets regular salary then the patients will be benefitted greatly and they do not have to go out of Gorakhpur for treatment or operation.

The serious encephalitis patient, 72 hours too difficult
Gorakhpur, Gorakhpur
Last updated: July 28, 2017 7:55 PM

Gorakhpur In the office reporter
Purvanchal, encephalitis remains a life-threatening killer. The doctors' drunkenness to save the innocent from death is proving to be a waste. The tragedy of this disease is that 67 per cent of patients who died in BRD could not spend 72 hours in the hospital.
The influx of encephalitis patients has increased in the BRD Medical College. About 100 patients were recruited in the last fortnight. 90% of these conditions are extremely fragile. Most patients are reaching the BRD after losing precious time of treatment. The reason for this is that the statistics of patients' deaths increased compared to the previous year.
GCS shows that the seriousness of the merge has
been set to standardize the risk of the life of an innocent person admitted to the BRS Encephalitis Ward. Doctors call it Glasgow Coma Scale (GCS). It involves the movement of innocuous eyes or pupils, its ability to speak, and strength in hands and feet to be recruited. Different points are set for this.
In 67 hours of recruitment of 67 percent of patients, died in
BRD this year, 98 patients have died from encephalitis. Of these, 67 patients came in those whose GCS was between three and seven. Such patients can not survive 72 hours after being admitted to BRD. 29 of the district have died during treatment in BRD so far. Of these 18 patients could not cross the 72-hour period.

Dr Kafeel Ahmad Khan, in charge of the innate bleeding encephalitis ward being kept on the ventilator said that GCS is of 15 marks. The number of children is more than eight. They are more likely to survive. Children with less than seven marks are at risk. They are placed immediately on the ventilator.
Zaolachap doctor
Dr Kafeel said that the reason for the death of innocents was a zodiac doctor due to encephalitis. In rural areas, they are treating the patient with a scream. Some of the patient's tigers have tweeted it. Poor patients are getting worse due to treatment.

Warning of stopping oxygen supplyWarning of stopping oxygen supplyLiquid Oxygen supply firm to Gorakhpur BRD Medical College warns of stopping supply
Gorakhpur

The firm supplying Liquid Oxygen to the BRD Medical College has warned against stopping the supply. The firm has owed 57 lakh rupees ($91,000 US - Ro) on the college. This is the reason for the firm's resentment.

Liquid Oxygen is supplied in many wards of Nehru Hospital of Medical College. About 300 beds, this type of oxygen is given through pipes. Pushpa sells its supply. In a letter sent to the principal, the company's official said that the college administration is continuously violating the contract. According to the condition, the college can get a loan of up to 10 lakh rupees. The amount will be paid within 15 days of submission of the bill, but now the loan has increased to Rs 57 lakh 44 thousand 336. The copy of the letter has been sent along with the Principal of the college, DG Medical Education and DM.

Need for research to check JE, AES

TNN | Jul 31, 2017, 09:25 AM IST
GUWAHATI: With 392 Japanese Encephalitis (JE) cases being reported out of a total 1328 Acute Encephalitis Syndrome (AES) cases so far in the state this year, the need for greater coordination between the medical fraternity and the community along with setting up of a full-fledged entomology directorate is being felt more than ever.
...
Till now, there have been 53 deaths due to JE and 110 due to AES this year. Thirty nine years since the first outbreak of encephalitis was reported in Assam's Lakhimpur district, vector-borne disease experts are still unable to capture the exact diagnosis of 60% of Acute Encephalitis Syndrome (AES) cases. From 2010 to 2016, 9,835 AES cases were reported from different parts of the state. As many as 1,675 AES deaths were reported in the same period.

Comment: Note how the article begins by treating JE cases as a subset of AES case numbers but, by the final paragraph, treats the deaths as additive. This should read 53 confirmed JE, 57 unconfirmed for a total of 110. - Ro

Encephalitis Treatment Center Live: Nowhere is a trend doctor
Hindustan Team, Gorakhpur Basti
Last updated: July 31, 2017 8:30 PM

102 deaths have been caused by encephalitis in the BRD Medical College of Gorakhpur. More than ten patients are reaching medical college every day, where two or two patients are being recruited on one bed for lack of doctors and resources. At present 78 more patients are admitted in Epidemic Ward. The number of innocent patients loaded on the shoulders of parents and the condition of the medical college is reaching like this: Encephalitis treatment center and other hospitals made in villages and towns are not proven to be effective.

On Monday, 'Hindustan' examined ETC centers in seven districts of the Gorakhpur-Basti division, it was discovered that there is no trend doctor, and there is no small facility available for medicines and treatment. Therefore, parents can not believe anywhere, and as soon as there is any symptom of brain fever, fever like fever, only some of their trust and reach reach the impression of the doctor. If there is no improvement in the condition of the child even after losing much time and money, then referring to BRD Medical College. But here, the condition of most children becomes such that it can be difficult to undergo 72 hours.
Gorakhpur- (Sardar ETC)

Trend Doctors are not the only way to treat how
most of the encephalitis is in Gorakhpur. In order to treat patients, 22 Encephalitis Treatment Centers (ETCs) are operated in the district, where patients' inflow is much lower than expected. Although the figure of patients recruitment is one and a half times as compared to the previous year. At some ETC, doctors have not been able to train.

'Hindustan' examined ETC on Sardar Nagar PHC on Monday. This Ph.C., running between a population of about one lakh, has all the resources for treatment. There is a lack of trained doctors only. There is an ETC of two beds. To operate this center 24 hours, there are three trained doctors. There is no pediatrician here. The first pedestrians posted here were transferred to the second district a few days ago.
Other doctors deployed in the hospital have not been given special training in the treatment of encephalitis. This reduction is being accepted by the hospital in-charge Dr. JK Sinha. There are medicines, oxygen cylinders and suction machines in this center. A total of 26 patients were admitted in the year 2016. So far 34 patients have been recruited this year. There are 12 suspected patients of AES.

-Manish Mishra report

ETC's worst, worst load on the district hospital,
Kushinagar is the most sensitive of the district encephalitis. Hundreds die every year, but with the help of incomplete preparations, war is fought with encephalitis. So far 11 people have lost their lives this year, while 142 patients have been admitted to government hospitals.
There are 15 ETC centers in the district, but almost all the centers are in the role of referral. Four-four beds are reserved for these patients for encephalitis patients, but the patients are referred to the district hospital as soon as the symptoms appear. This is the reason that the wards of ETC centers are vacant, then there is a waiting position in the ICU of the 10-bed district hospital. Alam is that when a patient gets little improvement in the condition, he is admitted to General Ward and the second is recruited. At this time all the beds of the ICU of the district hospital are full.

So far, 11 deaths have been reported from Kusinagar in Encephalitis, in which one patient has been killed in the district hospital and everyone else in Medakil College Gorakhpur. Most of the ETC centers in the district have treatment and beds, but the doctors send the patients to the district hospital instead of recruiting patients. In the district hospital, even a small condition worsens, then the medical college is referred to, but in the process the condition of the patient becomes very bad.
-
It is the ATC -
all the four beds reserved for encephalitis were vacant in the ATC ward on Monday morning. The oxygen cylinders, Ambu bag, Suction machine kept in the ward and running in the condition. There were enough drugs in the stock too. The ETC is on the second floor, due to which the chart of the Doctor and other staff will show the Chash with the mobile number near the Hospital Ward of Emergency Ward. Here are three pharmacists, Apart from four staff nurses, there are seven doctors, including a woman. The trained doctors of encephalitis are on departmental training at this time, due to which the patients presently treating patients are treated. Doctor and staff deployed on 12-12 hours duty, oversee the staffing of the ITC ward. A total of eight patients, High grade fever, were recruited in this center since April, three of whom were referred to the district hospital after receiving symptoms of encephalitis. After treatment of the remaining five, he was returned home.
-
discharged patients under the supervision of the hope
after the hospital superintendent treated by Dr. Mukesh Yadav home hope care Sent patients are put through Carykrtrion. Those who are recruited here are the elderly people of AYUSH, the elderly of the Ritesh Mahdi, Nandini resident Karjahan was referred to the district hospital.
--- The claims of the
responsible- is
the whole system of treatment of encephalitis. Patients are admitted in ETC and severe patients are referred from ETC to District Hospital. All necessary facilities are provided to patients admitted in ETC and District Hospitals.

-Z.A. Siddiqui report
.................................................. ....................................
Sant kabirnagar (natanagar etc)
Half of the bed reserved in
the district of fighting encephalitis is half incomplete preparation. The condition is that two beds have been reserved instead of four on many CHCs, including Nathnagar. Patients posted in the Emergency are responsible for supervising patients. A total of 40 patients have been found in the district since January. They include 39 AES and one Je patient. Seven of them have died. While the rest are healthy and living life. Nine ETC centers have been set up in the district along with Joint District Hospital.

In the district, the number of encephalitis affected villages was 402. After vaccination, things have improved in many villages, but the disease is not destroyed. The number of patients of Japanese encephalitis has decreased. While the number of people affected by AES is not taking the name of being reduced. To overcome the problem of contaminated drinking water, this disease is not being reduced even after spending crores of rupees on the maintenance of government handpumps every year.
In hospitals arrangements
Health Department to fight JE and AES on ETC's Nathnagar CHC has a system instead of four beds in hospitals, according to close two. Oxygen, Ambu Gas, is available. A trained physician for treatment but the responsibility of the doctor posted on the Emergency is done. The equipment and medication required for treatment are available. Despite everything, the lack of physicians is definitely coming down.
What is said is that the
necessary resources have been arranged on all ETC centers. From the recruitment of patients there are other necessary facilities.
Doctor Abhaychandra Shrivastav, CMO
-Tej Light Tiwari Report

settlement (Murtiya etc)

Drug equipment but lack of doctors
ETC Ward has been set up to facilitate the treatment of patients of AES at CHC of 13 blocks of habitation. The necessary equipment and medicines have been provided here but due to the lack of doctors, the patients are not getting benefit from it. Instead of ETC, patients prefer to be treated in district hospital or medical college.

11 AES patients have been reported from Banakati, Kudraharha, Saltouwa, Saaghhat and Vikramjot Chancellor. Most of them have been referred to the district hospital. The main reasons for the referee are being told that the doctor is not available for 24 hours in the hospital.
---
This is the center of the
ETC. All eight beds were lying vacant in the ETC ward on Monday morning's Murbalia CHC. In addition to the Gas Cylinder, Ambu bags and other equipment are found in the current condition. Medicines are also available in stock in adequate quantities. Following the rules, there is a mobile number in the duty chart of doctors and staff outside the ward.
Three pharmacists and four nurses were found on duty but there is no separate system for staff to stay posted on ETC. In the hospital, there is a deployment of two MBBS doctors in place of three. 12-12 hours duty has been imposed instead of eight. Because OPD is also responsible for them, they are on call duty after two o'clock in the day. When a doctor goes on leave, a doctor has to do twenty-four hours of duty.
--- On
seven
occasions recruited seven patients , MoIC Dr. Aftab Raza said that since January 2017, seven patients of Highgate Fever have been admitted in the ward. These were kept for a few days by the hopes of being converted to AES. A resident Shiva (6), daughter Ashararam was admitted to the ward on July 12 on the complaint of high fever. The home was sent to the fever control but the patient was looked after by hope. He told that the staff is available for twenty four hours but due to two doctors, doctors are on call duty after two o'clock in the afternoon.
---
30% of the patients are getting in Referral
district so far 71 patients of AES have been found. The Health Department says that about 70 percent of the patients have been treated in the ICU of the district and Kaili hospital. The department claims that the cases of JE have been reduced in the district. So far 9 cases have been reported and one has died. CMO Dr. JLM Kushwaha says that all ETCs have been instructed to recruit patients of Highgate fever. Only when the condition is fragile is the instruction to refer them.
-Sajjad Rizvi report
.................................................. ...............................................
Deoria- ( Rampur factory ETC)
Instead of ETC centers, directly reaching the
District Hospital, there are a total of 17 ETC centers in the district regarding the patient district hospital. Up to now, 7 of the 130 AES patients have died. In this, 6 patients have died in another medical college in the district hospitals. 43 patients of AES have been admitted at 16 Encephalitis Treatment Centers. When these were serious, 18 patients were referred to the district hospital. While 25 patients got cured after treatment. In 130, 23 patients were referred to Medical College. Since January, 611 patients of High grade Fever have been admitted on ETC. Most treatment facilities are available at ETC. In spite of being a night-day doctor in its district hospital, the patient's relatives feel better to be admitted here. There is no vacancy in the 10-bed ICU ward.
The roof is dripping - This ETC-
Primary Health Center has been built in a small room at Rampur factory. Two beds have been installed even after the place is low. But in the rain the roof is dripping in the room. The duty chart of the Doctor and name, mobile number etc. was not taken on ETC. It does not know which duty of the doctor is engaged. Four patients have been admitted in July on high fever. In it, on 7 July, Anjan of Ranjan and Dumri of Govindpur and Kartikeya recruitment of Shveta and Mundera of Sarasia on July 17. Other devices, including oxygen, Ambu bag, are found in the current condition. Sufficient quantities of encephalitis and high grade fever on the hospital are available for medicines and testing insects.
Similarly, 9 patients have been admitted till now on Rudrapur ETC. Two patients were cured of this and were discharged and 7 were seriously referred to the District Hospital. The doctor's charity charts, oxygen cylinders, amu bang, medicines, screening insects etc. are available on this four-bed treatment. There are four beds on the crest and there are 11 patients of high grade fever. Out of these Baloo Chapar's Krishna son, in the village of Petraulal, the hospital was referred to the symptoms of AES.
27 High grade Fever patients have been admitted to Gauri Bazar ETC. From this, Sirajam's Brijmohan was referred to the possibility of being a AES. 80 percent of encephalitis patients were treated in the district hospital and 20 percent were referred to medical college. Chief Medical Officer Dr. S.N. Singh said that the duty of the Health Officers has been imposed to remove the monitoring and deficiencies of ETC. He inspects and submits its report and accordingly, the shortcomings are being removed. Recognizing fever patients, hospitalization has been given to the health workers at the Gram Panchayat level.

As of July 31, 1,404 people in Assam have been diagnosed with acute encephalitis syndrome in Assam this year, 424 among them have tested positive for Japanese encephalitis. About 119 people have died of acute encephalitis syndrome of which 58 had Japanese encephalitis infections.

Bora, incidentally, had been vaccinated against Japanese encephalitis in February 2014. In fact, out of the 12 people who were detected to be infected by Japanese encephalitis in Bhogamukh, 11 had been vaccinated. And they are not the only ones.

Almost 28% of people in the state who have been infected by Japanese encephalitis had been vaccinated, said Dr Parashee Choudhury, senior medical and health officer at the state?s Infectious Disease Surveillance Program. ?There is definitely a problem there too. We need more research to say what exactly is wrong.?

Assam is the only state where children in all its districts are being given the Japanese encephalitis vaccine as routine immunisation, which is part of India?s universal immunisation program. The state began administering the vaccine to children for the first time in 2006 in Sibsagar district, and has since gradually expanded the coverage to other districts.

According to state government data, almost 80% of all children under 15 have been vaccinated as of December, 2016.
...
?The mortality rate has come down because of the vaccine,? said Dr Labnaya Baruah, a retired government doctor in Sibasagar district that neighbours Jorhat. Sibasagar has been a hotbed for Japanese encephalitis for decades.

?Also, if you notice the trend, more adults are getting affected than children,? said Baruah. ?That is because almost all children are vaccinated these days.?
