NAINITAL: After dengue, it's the turn of encephalitis to wreak havoc in the Kumaon region.

As many as 10 people have succumbed to encephalitis since August this year. Doctors say that the infection is beginning to assume the proportion of an epidemic with more than 100 suspected patients admitted in various government and private hospitals in Haldwani.

Nainital CMO L M Upreti said, "Preventive measures are being taken to curb the outbreak of the disease. Appropriate treatment of the patients and suspected cases is on."

However, the situation on the ground is different. Apart from the 10 deaths due to different kinds of encephalitis, patients suffering from the infection also have to contend with poor health infrastructure in the region.

7 more deaths ( aged 1,4,5,7,15,15 and 25 years old) reported from BRD hospital, Gorakhpur today. 8 new patients were admitted. 81 are currently under treatment. The cumulative toll for BRD is 1,209 patients of whom 260 have died. At least 46 deaths have occurred in other parts of Uttar Pradesh (see elsewhere in this thread for details). The state has now seen at least 306 fatalities this year.

Deaths from encephalitis less, vaccine out in November

Concerted efforts of the government towards its fight against Japanese Encephalitis and Acute Encephalitis Syndrome have started yielding results. Data reveals that for the first time cases and deaths of both JE and AES have gone down. To further prevent its spread, the government is planning to introduce adult JE vaccine from the November. (transmission peaks in Sep-Oct, but at least they will be protected for next year)

In 2015, till September 11, total 985 cases and 210 deaths have been reported due to JE (up from 907/199 resp. to Aug 31) as compared to 1089 cases of JE and 211 deaths which were reported in the corresponding period in 2014. Significantly, during 2015 the AES cases and deaths have fallen in the endemic areas.

According to government data, 1291 AES cases (up from 1,027 reported to Aug 31) have come forward in 2015 in Uttar Pradesh. However, during the same time in 2014 (incorrect - the figure quoted here is for the full year), 3329 AES cases were reported in UP. Bihar has seen a drastic improvement too with the endemic state reporting a mere 145 cases in 2015 (up from 105 reported Aug 31st) as compared to 1358 of AES cases reported in (all of) 2014. (The reduction from Bihar is mainly due to the fact that an atypical outbreak of AES from June of 2014 in the Muzaffarpur district affected a thousand and killed around 200, mostly children, did not recur this year.The causes of this outbreak have not yet been proven, but may relate to the litchi harvest which was poor this year. Even the number of deaths due to AES have decreased from 627 in (all of) 2014 to 174 in 2015 (up from 135 to Aug 31st, as of today this figure has risen above 300) Uttar Pradesh. Even in Bihar, the number of deaths have gone down drastically from 355 in (all of) 2014 to 53 deaths (up from 24 reported to Aug 31st) in 2015.

A similar story prevails for JE, as both ? cases of JE and deaths ? in Bihar and UP have come down. JE cases in Bihar (should read Uttar Pradesh not Bihar) have gone down from 191 in (all of) 2014 to 103 (up from 56 reported to Aug 31st) in 2015. In Bihar, 20 cases of JE have been reported in (all of) 2014 to 8 (up from 0 reported to Aug 31st) so far in 2015. The number of deaths have fallen too. While in (all of) 2014 34 people died due to JE in UP, the deaths in 2015 are 11 (up from 4 reported to Aug 31). In Bihar, two people died (in all of 2014) as compared to one death so far (up from 0 reported to Aug 31st). ?Ever since his visit to Gorakhpur in February 2015, the health minister has been regularly monitoring the situation,? said the official.

?Seven districts of UP have been identified for adult JE vaccine by an expert group as per the recommended criterion depending on the adult case load in the endemic districts,? said a senior official. Adult JE vaccination has been planned in 36 blocks of six endemic districts.

I'm disappointed with the conclusions made by the author of this piece. Japanese Encephalitis season peaks in both Bihar and Uttar Pradesh in September / October. When you compare pre-peak disease figures for these states to full season figures for last year of course they are going to be lower. To state that the figures quoted for 2014 are "during the same time" is flat out wrong and misleading. Looking inside the numbers, AES cases in UP rose by 264 in the 11 days from Aug 31-Sep 11, deaths rose by 39 during that period. In Bihar, 40 cases and 29 deaths were reported during the same period. So those two states alone were reporting an average 27 cases and 6 deaths per day around September 11. There's 50 days between September 11th and October 31st. That's peak season. Then there's a gradual decline in the 61 days from November 1st until the end of the year. We can sadly expect 1,500 more cases and 300-400 more deaths from Sep 11th to the end of this year. I don't think that this deserves a pat on the back.

Spectre of diseases haunts Gujarat

GANDHINAGAR: While malaria, dengue and swine flu continue to give health authorities in major Gujarat cities sleepless nights, cases of acute encephalitis syndrome (AES) and Congo fever are emerging as the new worry factors for state health authorities.

In the past five weeks there has been 45 suspected cases of AES and three places have reported Crimean-Congo haemorrhagic fever (CCHF) ? Lakadiya village in Bhachau, Vadala village in Mundra, both in Kutch and the third being Desai Purakampa village in Bayad in Arvalli district. This has already set alarm bells ringing in the state health department.

22 new patients and 4 more deaths (aged 6 months, 3, 4 and 27 years old) reported at BRD, Gorakhpur on October 6th. 93 are under treatment. 1,249 encephalitis patients have been admitted to BRD this year. 270 have died. The state wide toll is now at least 317.

An attempt to distribute & study the effect of insecticide treated mosquito nets in certain districts has ceased. A research group attempted to distribute nets only to households with children under 15 years of age. Popular and political pressure was put on the group to distribute nets to all households. The distribution has ceased for now.
