Japanese encephalitis toll at 52, number expected to rise

As of Monday, the death toll of victims of Japanese Encephalitis (#JE) rose to 52 in the state, with lower #Assam being the region worst affected by the disease.
The total number of JE positive cases registered in Assam on Monday was 323. Additionally, the number of deaths caused by Acute Encephalitis Syndrome (#AES) is at 160 while 947 AES infected cases have been registered in the state. AES is a group of diseases that may or not pertain to JE.
The #Barpeta district has remained the worst affected district in the state as the death toll rose to eight from five deaths within one week and confirmed positive cases jumped from 41 to 54 by Monday.
The rural area of the #Kamrup district recorded 34 cases with four patients succumbing to the virus. The district also recorded 90 positive AES cases that led to 13 deaths. #TheSentinelAssam

It is for the first time that lower Assam has recorded so many positive JE cases and the department is yet to ascertain the exact reasons behind the rapid spread of this infection.

The Union Health Ministry has sought a report from the West Bengal government on the encephalitis deaths in north Bengal where over 100 persons have died so far since January.
The ministry has sent a letter asking the state Health Department what measures have been taken to contain the disease and the arrangements that have been made at the North Bengal Medical College and Hospital (NBMCH) to tackle the epidemic-like situation.
Dr Harsh Vardhan, Union health minister, also confirmed that they have sought a report on this matter.

A team of experts also reached NBMCH on Friday. They will tour the affected areas in Jalpaiguri, Cooch Behar and Darjeeling.
NBMCH has received 200 encephalitis test kits from Pune and has started an MRI unit to tackle the flood of patients.

The encephalitis toll in seven north Bengal districts rose to 105 in last 15 days with six more deaths reported since Monday, a senior government official said Tuesday. He said, at least 80 children affected with the disease have been admitted to hospitals — 45 at North Bengal Medical College and Hospital (NBMCH), the rest at Jalpaiguri district hospital and other centres.
“As of now, 105 people have died in the region since the outbreak about three weeks back. In NBMCH, 67 patients affected with encephalitis died this month. Of another 45 detected with encephalitis syndrome, seven are very critical. Two patients died Tuesday, while we lost four patients on Monday,” said Amarendranath Sarkar, superintendent of NBMCH.

There are vaccines for swine flu but we have no knowledge of any drug used in India to vaccinate pigs to arrest encephalitis," he said.

Rise in encephalitis cases: govt. to conduct entomological survey

With the State grappling with the rising number of encephalitis cases, the West Bengal government has decided to conduct an entomological survey in north Bengal and directed the municipalities to move the piggeries located in towns outside human habitation.

Speaking to The Hindu on Tuesday, B R Satpathi, Director of Health Services, West Bengal, said the government had roped in the Centre for Research in Medical Entomology, Madurai, to conduct a survey in the affected areas in north Bengal.

Scientific investigations into the outbreak of acute encephalitis syndrome (AES) in West Bengal, which has claimed 105 lives since June, have revealed a pesticide has been found ?above minimum safe limits? in the chemical analysis of litchi samples from Malda, but asserted that it was too early to ?confirm its toxicity in humans?.
Tests of litchi orchards in Bihar?s Muzaffarpur, the other epicentre of the yet unidentified disease causing a similar AES, conducted over decades have never identified any toxin, experts said.
According to an expert from NVBDCP, ?Pesticide Alpha Cyphermethrin was found above minimum safe limit in litchi samples. We tested the fruit samples because the villages are surrounded by litchi gardens, and since no pathogen was found in blood and other samples of patients, we started looking for toxins in surroundings.?

Meghalaya sounds alert on Encephalitis

Meghalaya today sounded an alert in four districts bordering Assam after outbreak of Encephalitis in the neighbouring state, health officials said here.

Even as there was no confirmed case till date, at least six suspected cases of encephalitis have been detected in Ri-Bhoi district, they said.

The four Meghalaya districts where alert was sounded by the state health ministry are Ri-Bhoi, parts of West Khasi Hills, North Garo Hills and West Garo Hills.

Dr Harsh Vardhan writes to Ms. Mamata Banerjee on neo-natal deaths due to AES and JE. Offers Central support

The Union Health Minister Dr Harsh Vardhan has written to Ms. Mamata Banerjee, Chief Minister of West Bengal regarding neo-natal deaths in the state due to AES and JE and offered help of the central government.

In his letter, the Health Minister has stated that the flow of reports from West Bengal on neonatal deaths and fatalities from AES and JE had prompted him to send specialist teams to the affected areas during the current month. The Minister states that there are infrastructure issues that need to be addressed as we are only a year away from the deadline for meeting the Millennium Development Goals on MCH parameters. Regarding the deaths due to JE-AES in north Bengal, the Health Minister states that the Ministry has stepped up immunization drive against JE and cleared the installation of urgently needed hospital infrastructure in the affected areas. He notes that he has himself flagged off a mass immunization drive in June in Bihar.

Dr Harsh Vardhan has assured the Chief Minister of West Bengal of his personal interest in extending all facilities to help her tide over the crisis. He has also recommended, for a start, intensive and extensive consultations between the state and central officials on the steps to be taken on an emergency basis, and also on the permanent measures that would be necessary to avoid such tragedies in the future.

GUWAHATI: Japanese Encephalitis (JE) toll in Assam stood at 59 on Wednesday with 364 positive cases. Till now, 167 people have succumbed to Acute Encephalitis Syndrome (AES) with 1043 total positive AES cases. Sivasagar registered a total of 58 AES-positive cases with 23 casualties.
Alarmed by the spread of JE, chief minister Tarun Gogoi on Wednesday directed senior health officials to curb spread of JE and AES in new areas and map areas that are prone to water-borne diseases.

Gogoi, besides stressing on the importance of a comprehensive action plan in combating JE and AES, asked the authorities concerned to set up for Intensive Care Units (ICU'S) in each and every district hospital for proper treatment of emergency cases. He asked them to ensure that there is adequate supply of vaccines and medicines to hospitals.

Guwahati, July 23: Chief minister Tarun Gogoi today reviewed the health scenario in the state with Japanese encephalitis (JE) and acute encephalitis syndrome (AES) claiming 59 and 167 lives respectively in Assam so far.

Gogoi directed health officials to conduct awareness campaigns by involving panchayats, to intensify measures to control the diseases from spreading to newer areas and carry out intensive fogging in the affected areas and localities situated close to them.

The directive came as another person, Chitra Rabha, 45, died of the disease at Singra-Palahapara village under Boko police station in Kamrup district last night, taking the toll from the disease to four in the village.

Fever clinics have been set up in all district and sub-divisional hospitals of the region. The State government had issued a red alert in seven districts of North Bengal and cancelled the leave of hospital staff.

With six more deaths due to encephalitis reported in the North Bengal Medical College and Hospital (NBMCH) over the past 48 hours, the death toll in North Bengal has touched 83 this season.

According to Amarendra Nath Sarkar, the NBMCH Superintendent, more than 65 patients died in the medical college hospital in July.

The health experts examined sources of drinking water, treatment infrastructure, nutrition levels of infants and mothers and looked for chicken and pig farms in the region to ascertain the reason behind the outbreak. They also collected mosquitoes and blood samples of affected patients for laboratory test.

At the end of the meeting, ICDS project manager Debdas Biswas said last year children ranging between six months to six years of age were vaccinated which is why this age group has not been affected this year.

?This AES is an unhelpful terminology ? it represents a basket of possibilities,? said Thekekkara Jacob John, a senior virologist formerly with the Christian Medical College, Vellore, who was invited by the Bihar government last year to investigate AES in the state and has recommended that the term should be abandoned.
Experts have come up with varied reasons for AES but no one has been able to pinpoint what are the causes for what is labelled as AES.
The symptoms associated with Acute Encephalitis Syndrome, or inflammation of sections of the brain, can happen because of a number of conditions ? viral infections such as JE, or infections caused by enteroviruses, or bacterial infections, or typhoid.
The precise cause of the disease is important to prevent future outbreaks, public health experts said.
?We?re missing the elephant in the room ? whatever be the cause, the high mortality is unacceptable,? said Manish Kakkar, a clinical microbiologist and faculty member at the Public Health Foundation of India, New Delhi.

According to Amerendranath Sarkar, the NBMCH superintendent, around 25 children have been admitted with acute encephalitis syndrome in the past month and five have tested positive for JE.
?So far, eight children have died, of whom seven had acute encephalitis syndrome and one had tested positive for Japanese Encephalitis. Most of the patients affected by Japanese Encephalitis are adults. At present, 42 patients are admitted at the hospital with symptoms of encephalitis and 10 of them are children,? Sarkar said.

There late in the evening she died during treatment. The child's death has caused mayhem in the family. Also Kanhya fever (3) Khurhnd town resident, Anshoo (3) Bisnda, Hribabu (2) Pista, sun (2) Mrdnnaka, Sukhbir (8 months) Bisndi, Munni (3) grams Lhureta, an old Umakant etc. have been admitted to the district hospital. Other public and private hospitals are being admitted suffering severe fever. Similarly, a girl to go out on Monday has died of meningitis.

Suspected Chandipura outbreak toll rises to 8

VADODARA: Five more children have died in the suspected Chandipura virus outbreak in central Gujarat in the last two days, taking the total number of deaths to eight since Sunday.

While four children, all below age of 10, died at Sir Sayajirao General Hospital (SSGH) throughout Tuesday. Earlier on Monday, a five-year-old boy Vikram Pagi from Vadagam in Lunawada taluka of Mahisagar district had been rushed to Modasa for treatment where he died on Monday.

Three children from Bara village in Devgadh Baria taluka in Dahod had died of the virus on Sunday.

Jalpaiguri: An outbreak of Japanese encephalitis and suspected encephalitis in North Bengal has claimed over 100 lives in seven districts, with the majority deaths coming in the last fortnight. The government has said it has stepped up measures to control the spread of the disease.

But settlements like the Haricharan Bihta village in North Bengal's Jalpaiguri district are examples of how the government's measures to tackle encephalitis may not be working. A lot of these people for instance prefer going to private doctors as they say government doctors are mostly unavailable at primary health care centres.

The state health department has stepped up measures to tackle the situation with intensive fogging initiated in the affected areas and awareness campaigns launched by involving the local panchayats, a health department official said.

Measures are being taken to ensure adequate supply of vaccine and medicine to big hospitals, district hospitals and other health centres and procure laboratory testing kits for quick diagnosis of cases.

The Public Health and Engineering Department (PHED) has been asked to speed up conversion work of shallow tube wells into deep tube wells, specially in the affected areas, to bring down high incidence of AES cases.

Enterovirus suspicion in encephalitis zone

July 24: Experts from Pune?s National Institute of Virology who have toured parts of north Bengal have told district health authorities that they suspect enterovirus outbreak to be the reason for the high death count.

In the North Bengal Medical College and Hospital, where most patients with symptoms of Japanese Encephalitis are being treated, one more person died today. The tests done on 50-year-old Hasan Eman, who hailed from Chopra in North Dinajpur, had not confirmed Japanese Encephalitis, according to Amerendranath Sarkar, the NBMCH superintendent.

Eman?s death without confirmation of what virus took his life has been the dominant pattern to emerge from this outbreak. This is one of the reasons why the NIV experts suspect that the cases being classified as Acute Encephalitis Syndrome (AES) could be related to some enterovirus ? any virus that could be contracted from water, food, saliva and sputum, among other agents.

The NIV experts who toured Mainaguri and Dhupguri in Jalpaiguri district also found that many people were drinking water from contaminated sources.

The Pune experts have decided to send the samples of over 190 patients, all from Jalpaiguri and Alipurduar districts, to their laboratory in Pune and also another lab in Mumbai to test for enteroviruses...
