The canker Encephalitis in the last three and half decades Purvanchal Gorakhpur division, this time is the most devastated. Treatment is mainly due to the surge of deaths from encephalitis Bdintjami. Most of the districts are not recruiting patients at public health centers. They treat patients who have arrived is almost nil. District hospitals are just formalities being completed. The whole system will fail. Forget night, a day after two hours at most health centers are not doctors. If you have already received encephalitis patient is referred by looking at ambulances.

Only ten per cent of these patients come to the OPD of the Medical College. Eighty per cent of the private physicians revealed that they were in critical condition after the show when you arrived. Only ten per cent are referred to the district hospitals or health centers. Experts say that if you get timely treatment, deaths are largely preventable.

My latest count for fatalities in Uttar Pradesh is 240. There have also been two fatal cases reported from Karnataka, one from Jharkhand and four suspected Chandipura fatalities reported from Gujarat. All told, 795 total fatalities to date.

Gorakhpur: Encephalitis deaths and continues to recruit patients. Twenty-four hours the past fourteen new patients were admitted to the Medical College while two were killed. Nehru 141 patients are being treated in hospital.

Gorakhpur, Sep 12 (UNI) The death of two more children overnight due to vector-borne Japanese Encephalitis (JE) disease at Baba Raghav Das Medical College in this district has taken the toll to 251.

The children were from Deoria and Sant Kabir Nagar districts of Uttar Pradesh.

Since January 1, as many as 1,143 cases have come to Baba Raghav Das medical college and other district hospitals for treatment, of whom 251 died. The deceased included 30 patients of Bihar and one patient of neighbouring country Nepal.

from Blocks/PHCs Naharani, Tengakhat, Lahowal, Khowang, Barbaruah &
Panitola, District Dibrugarh in 27th week (ending on 7th July 2013). From 8th
to 29th July, 2013 further, 94 cases and 30 deaths occurred. Out of 183 CSF
& serum samples collected & sent to AMCH, Dibrugarh 82 samples were
positive for JE.

Sivasagar ii. AES/JE 105 cases 36 fatalities 08/02/13 Under surveillance
In the 27th week (ending on 7th July 2013), an outbreak of Acute Encephalitis
Syndrome (43 cases & 17 deaths) was reported from all BPHCs of District
Sivsagar. Further, 62 cases and 19 deaths occurred till 30/07/13. Out of 105
CSF/Serum samples collected, 41 samples were positive for JE.

Tinsukia iii. AES/JE 81 cases 23 fatalities 15/01/13 Under surveillance
41 cases and 10 deaths due to Acute Encephalitis Syndrome were reported
from BPHCs of District Tinsukia in 27th week (ending on 7th July 2013).
Further, 40 cases and 13 deaths occurred till 29th July 2013. Out of 81
CSF/serum samples collected, 34 samples were positive for JE.

'Polluted water, malnutrition are root causes of encephalitis'
After a two-day public hearing, National Commission for Protection of Child Rights today said contaminated water and malnutrition "appear" to be root causes behind the outbreak of encephalitis in Gorakhpur region.

Since January this year, the vector-borne disease has claimed over 200 lives in the region.

Speaking on the concluding day of the two-day public hearing on encephalitis at the BRD Medical College here, NCPCR chairperson Kushal Singh asked all the concerned departments and authorities to take immediate action to prepare a strategy to prevent further outbreak of the disease and suggested several ways.
Besides Singh, the public hearing was attended by NCPCR members Vandana Prasad and Yogesh Dubey, the commissioners of Gorakhpur and Basti divisions, district magistrates of the seven districts, senior officials of different departments, experts and relatives of the JE/AES victims.

NCPCR members stressed the need of utilising front-line service providers like accredited social health activists and auxiliary nurses in early detection and timely referral of children having symptoms of encephalitis.

Encephalitis menace continued unabated in eastern UP where six more children died, taking the death toll in the disease to 285 this year.

The six children died in the last two days at the BRD Medical College Hospital, health officials said today.

As many as 12 people suffering from the disease were admitted to the hospital today, they said, adding 158 patients are being treated at BRD-MCH and different government hospitals in the region.

This year, 1374 persons suffering from the disease were admitted to these hospitals, of which 285 died, they said.

Encephalitis claims 6 more lives, 8 dengue cases reported

Encephalitis claimed six more lives in the last 24 hours as dengue also made its presence felt in the district with the arrival of eight patients at Gorakhsnath Chikitsalay.

Krishna (4), Reena (7) and Anil (3) of Maharajganj, Sunita (13) and Asha (8) of Gorakhpur and Pintu (16) of Bihar succumbed to encephalitis.

For two-three months before the monsoon, when the weather is hot and dry, when fruits are ripening on the mango and litchi trees, and when villages are preparing for the coming showers?layering roofs of their thatched huts with more bricks and straw?parents in north Bihar's Muzaffarpur district wake up each morning with dread.

Every year, around this time, 900 and 1,200 children between the ages of two and five are afflicted with a mystery illness. It starts with lethargy and a light fever, but shows the first signs of worsening towards the morning. Many of the children are dead before they can reach a hospital.

The district saw 44 deaths in 2011, 121 in 2012 and 39 this year. Bihar Health Secretary Sanjay Kumar admits they don't know the reason for this decline. "The cases have dropped this year simply because God has been kind," he says. "We don't know what we have done right. Right now, we are all groping in the dark."


The latest to draw a blank is a team from the Centers for Disease Control (CDC) in Atlanta, US, which was roped in by the National Centre for Disease Control (NCDC) to test samples from this year. A report submitted to the state and Centre by the NCDC in the first week of September said the results have been negative for several known viruses, including Japanese Encephalitis (JE), enteroviruses, West Nile virus, dengue, Chandipura and Nipah.

"Since the disease was not found in patients, we decided to look for it in the vectors?mosquitoes which carry the JE virus from hosts such as pigs, water birds or other livestock to humans," says Dr Pradip Das, director of RMRI. However, no known vector of JE was found on the scene. "Now we have eliminated the theory of JE completely," Dr Das says.

The RMRI and NCDC conducted similar tests, but to no avail.

Admits Dr Das: "There are several characteristics which make Muzaffarpur's disease different. JE is a post-monsoon disease, while all the cases here are seen before the rains. Here, in case of fever, sponging with cold water and removing all clothes helps, unlike in JE where patients are advised to cover up. In JE, affected children are up to 15 years of age, while here it's kids till the age of five-six who bear the brunt."

There are other differences: in Gorakhpur, deaths occur within 48-72 hours of convulsions, while in Muzaffarpur, mortality strikes within a few hours of the first convulsions.

The CDC Atlanta examined MRIs of 10 children and reported no signs of inflammation in the brain tissue, according to preliminary findings published in an NCDC quarterly newsletter.

"In fact, there were no signs of any infection at all in the CSF. This throws up a huge question. We don't know if any pathogen is involved at all. That could mean our search for viruses as possible causative factors has been in the wrong direction," Dr Das says.

But this also led to inconclusive findings. "Liver function tests and kidney function tests on patients to test for toxins have all been negative, with no evidence of absorption of any poisonous agent from fruit or otherwise," Dr Singh says.

Noting that "no infective organism or its antigen or antibody was found in any of the samples tested", the authors say, "overall picture of these cases was suggestive of heat stroke".

Other clinicians however discount this theory, stating that episodes of heat strokes cannot be seen in the wee hours of the morning?the time when most children take a turn for the worse.

The CDC team has given the Muzaffarpur illness an even broader name?Acute Neurological Syndrome. "We are trying to capture all possibilities since there was definitive brain involvement. Many patients did not have fever so we are not sure of an infectious agent or any pathogen," a member of the CDC team visiting Patna said.

But there is one one common factor that binds all the children. Crowded into the three main hospitals of one of India's most backward regions, they all show low sugar levels (hypoglycemia) and elevated sodium levels (hyponatremia).

Investigators say these blood indicators are a "no-brainer"?the children are all malnutritioned.

Abstract:Japanese encephalitis (JE) is an important cause of morbidity, mortality, and disability in the countries of
Asia. Prospective, hospital-based study for AES/JE in all the age group was undertaken for 5 years (January 2008–
October 2012) in lakhimpur district of Assam. A total of 217 (Two hundred and Seventeen) nos of AES suspected
cases were found from different areas of North Lakhimpur District of Assam out of which 54 (fifty four) nos of
patients expired. Serological diagnostic test revealed that 66 (Sixty six) nos of cases were found to be positive for JE
IgM antibodies. 12 (Twelve) no of JE positive patients expired during last five years. Most of the AES cases were
observed between the age ranges of 6-30 years. Males were highly affected with AES as compared to females. A
positive correlation was found between the nos of occurrence of AES/JE cases and monthly rainfall. The prevalence
trends of AES/JE cases were found to an unsteady pattern.

CONCLUSION:
From the study it was concluded that JEV strain is
still circulating in this part of Assam due to many factors like
existence of piggery firm, increase n nos of domestic pigs,
frequent migration of people, environmental condition, rice
field near to house, unhygienic living condition and some
other factors. Significant progress has been made in the effort
to controlJE and the incidence has declined.However, it was
observed that nos of AES/ JE cases have been decreasing in
this year due to the implementation of the SA-14-14-2
vaccine among the children in February, 2012 in Lakhimpur
district of Assam. Vaccine schedules, surveillance of adverse
events, and setting up effective vaccination programmes are
important issues that need addressing to maximize the
impact of the available vaccines.
