Change in clinical profile alerts docs

Maulshree Seth

Posted: Fri Dec 02 2011, 04:56 hrs
Lucknow:

Certain changes in the clinical profile of encephalitis patients in eastern UP this year have become a new cause of concern for the doctors. Apart from the usual high fever and convulsions, the doctors have noticed that most patients are now taking longer time to recover, having rashes all over their body and also heart inflation.
As encephalitis patients are given symptomatic treatment, changes in the clinical profile have been noticed by the doctors at Gorakhpur’s BRD Medical College, which receives patients from all over eastern UP. They are now planning to bring out a clinical paper on the subject and also study the subject.

Patients coming to us this year are more serious as their illness has become prolonged now. Till last year, the average stay of an encephalitis patient at the hospital used to be two week, but the recovery time has increased to three weeks this year. Symptoms like high fever and convulsions continue for three weeks now. Besides, children are coming with rashes all over their body and inflation of the heart,” said Dr KP Kushwaha, head of the Pediatrics Department at BRD Medical College.

...

Denying any possibility of mutation in the virus, Dr MM Gore, senior scientist at National Institute of Virology, who is also in charge of the NIV field laboratory at Gorakhpur, said: “It is too early to come to any conclusion, because even doctors are still observing the clinical changes”.

He added: “As far as I have come to know, symptoms of JE patients remains the same and it is the other forms of encephalitis where doctors have observed changes in the clinical profile.”

The problem with the experts is that apart from the JE virus, the viruses affecting majority of the patients have not yet been identified. For this reason, all non-JE patients are grouped together in a category called acute encephalitis syndrome (AES). The lack of identification of AES viruses makes it difficult to ascertain the causes behind these clinical changes.

The similarity of this illness to "encephalitis lethargica" or Japanese B encephalitis (JE) is striking, and presumed JE cases with negative serology should be further investigated to rule out NMDARE.

The article posted by Emily gives some interesting statistics;

"A recent study has provided insights into the likely burden of "encephalitic illness." Of 203 patients with an encephalitic illness, 42% had an infectious cause (including 19% with Herpes simplex encephalitis, 5% with Varicella encephalitis and 5% with Mycobacterium tuberculosis), 37% were of unknown cause and 21% had immune-mediated encephalitis (IME). Of the last category, 11% were diagnosed as acute disseminated encephalomyelitis (ADEM), while 9% had other autoimmune causes. Among this subgroup, 1% of the patients were diagnosed with anti-NMDAR encephalitis, [3] a figure similar to that observed by Dalmau et al., who also found only six cases in a large series of 505 patients (1%). ?Mysterious? disease still baffles government
Nishant Sinha, TNN | Dec 2, 2011, 08.32PM IST

PATNA: Although, every year encephalitis hits north and central Bihar areas of Muzaffarpur, Gaya and adjoining districts, the state government has not been able to take effective steps for saving the lives of innocent children. In fact, months after the outbreak of the mysterious disease in Muzaffarpur, the government is still clueless on whether it was encephalitis or Japanese encephalitis (JE).

Health secretary, Sanjay Kumar, admits it, "As far as Muzaffarpur goes, we are still groping in the dark on whether it was JE or encephalitis. As far as Gaya is concerned, by and large experts have said it is JE."

Bihar has reported 386 cases of encephalitis and Japanese encephalitis this year, with the death toll being 89, according to the government. (Bihar has actually reported 821 cases and 197 fatalities - Just another example of poor fact checking in Indian medical reporting - Ro)

Civic body told to ban rearing pigs in residential areas
SATURDAY, 17 DECEMBER 2011 00:58 RAJESH KUMAR | NEW DELHI

With fear of Japanese Encephalitis (JE) looming large in the Capital, the Delhi Government has directed the civic body to immediately stop rearing of pigs in residential areas. While there are slaughterhouses for other animals, there are no slaughterhouses for pigs. A proposal has been moved by Directorate of Health Services which suggested that Delhi Government should impose municipal laws banning rearing pigs in residential colonies and areas as it was applied in the case of banning cows and buffalos in the capital.

It has also suggested regulating their stay in Delhi for a minimal period for their slaughtering at designated places. Sources say the Urban Development department will have to direct MCD to immediately stop rearing of the pigs in residential areas or regulate their stay for a minimal period for their slaughtering at designated places.

?The pigs are being reared all over Delhi in slums and resettlement colonies. In addition, pigs are being brought from the other states in the Capital and there is no slaughter house for pigs. These are being slaughtered in various places,? the proposal said. A senior official of health department said on the condition of anonymity that there is a possibility that Japanese Encephalitis may be an endemic by next year if it was not controlled or checked. The note says JE infection has been reported from neighbouring state UP and Haryana though virus infection in pigs could not be established initially.
