Poor coordination between Centre, UP hits encephalitis programme

GORAKHPUR: Lack of coordination between the Centre and state government seems to have affected the encephalitis treatment programme. The disease has gripped the eastern part of Uttar Pradesh for the past 33 years, and neither the state, nor the UP government appears to be serious in providing infrastructure for management of the patients.

Two more children died of encephalitis, while 38 new cases were admitted to the Baba Raghav Das Medical College on Saturday. With this, the encephalitis death toll has touched 433 since January 2011, while the total number of encephalitis cases reached 2,482.

Alarmed over the situation, the ministry of health and family welfare has decided to hold a meeting of all officials on Monday October 17. Doctors, from BRD Medical College will also attend the meeting to submit a status report.

Sample this: In 2009, the union ministry of heath and family welfare had sanctioned 136 posts for a 50-bedded acute encephalitis syndrome treatment facility in BRD Medical College, Gorakhpur. It gave funds to the tune of Rs 5.88 crore for the unit and stated that Rs 2.53 crore should be spent on sustaining the work force. However, the Centre had not made it clear whether the grant would be a one-time charity, an annual feature or a short-term grant. In 2010 as children continued to die, no grants came in. And in August 2011, the principal of the medical college was directed to trim the work force to just 36.

Thirteen-year-old daughter of Surendra Lights Regional Rural Trkulwa Bhatganwa resident was suffering from JE. He was admitted to Medical College in Gorakhpur, where he died on Friday during treatment. The news is panic in the village. The villagers demanded that the spraying.

With six more deaths, Japanese encephalitis, a brain infection, has killed 68 children in Bihar's Gaya district in the last 50 days, an official said on Monday. Six more children died in the last three days at the Anugrah Narayan Medical College and Hospital (ANMCH), about 100 km from Patna, hospital deputy superintendent NK Paswan said.

"About 50 children are still battling for life in the hospital," he said.

Most children belong to Mahadalit families -- the poorest of the poor in the state.

Gorakhpur: has been a string of deaths from encephalitis in Purvanchal. In the past two days, 44 new patients admitted to BRD Medical College, killing six. Still admitted to Nehru Hospital, 278 patients are being treated different wards.

Among those who died six years Sunny in Gorakhpur, Deoria light of four years, thirty years of Bihar Arun confluence of four years, eight years of Smita and Deoria Babu Nandan are eighty-five years.

Twelve of the 44 people recruited in Gorakhpur, Kushinagar ten, seven of Deoria, Bihar and Sntkbirnagr three - three, five and Siddharth Mhrajagnj four patients.

According to information received here from the dead in Bihar Arun encephalitis autopsy was conducted. According to sources Priwarijnon say any medications that the patient had become ill from eating at the Medical College was admitted as a patient of encephalitis.

This is a very significant development. Patna has a population of almost 6 million. If AES/JE gets a foothold in the city, the consequences will be disastrous. This article does not give any fatality figures, but one can assume at least 50 fatalities have already occurred from the 250 cases.

Patna, Oct. 17: Japanese encephalitis ? a form of brain fever ? has hit the state capital and its neighbouring areas. At least 10-12 cases of the disease are being reported at Patna Medical College and Hospital (PMCH) daily over the past few days.

Dr Nigam Prakash Narayan, a senior doctor at the paediatrics ward of the hospital, said: ?About 250 patients of encephalitis have been admitted to the hospital so far. Of them, around 100 are down with Japanese encephalitis. This is apart from high number of cases registered at the government medical college in Gaya. The mortality rate of Japanese encephalitis patients is about 20-25 per cent. Thus, the disease has taken an epidemic form in the state.?

Dr Narayan said: ?Most of the patients coming to the hospital are from Patna, Hajipur, Muzaffarpur and Chhapra districts. Several of them are in critical condition.?

With the number of suspected Japanese encephalitis cases on the rise, the PMCH authorities are finding it tough to cope with the pressure of patients. The health hub is running short of beds as well as medicines.

?Many Japanese encephalitis patients are lying on the floor as beds are not available. We are also running short of required medicines. Worse, there is no special arrangements for such patients though new cases are pouring in every day,? a senior officer of the hospital said.

Encephalitis claims 9 more lives in UP

Gorakhpur: Nine more people succumbed to encephalitis, taking the death count in the brain fever to 465 so far this year in Uttar Pradesh's eastern region, health officials said here on Wednesday.


Four encephalitis patients from Gorakhpur, two each from Kushinagar and Deoria and one from Mahrajganj succumbed to the illness in the last two days, they said.


With this, the death toll due to the viral disease rose to 465 this year, they said.

As many as 301 encephalitis patients are undergoing treatment at BRD Medical College.

UP: 10 more children succumb to Encephalitis

In UP, ten more children have succumbed to deadly encephalitis during past 36 hours in BRD Medical College Hospital in Gorakhpur. With this, the total death toll due to Japanese and other forms of Encephalitis has mounted to 465 this year.

AIR Gorakhpur correspondent reports over 2950 encephalitis patients have been admitted this year in different government hospitals. Over 321 encephalitis patients are being treated in various government hospitals of the region.

Meanwhile, the state government has decided to launch another round of Japanese Encephalitis Vaccination Campaign in the affected districts.

The state Director General (Health) Ramji Lal said in Gorakhpur that the upper age limit for vaccination has been increased to 25 years. Till now, children up to 15 years were being administered JE vaccine.

BHUBANESWAR: The state government has sounded an alert on Japanese encephalitis, the viral disease that has gripped Uttar Pradesh, Bihar and Delhi. The government has asked all district headquarter hospitals and medical colleges to examine blood samples of "brain fever" patients for Japanese encephalitis (JE).

Angul district, which witnessed hundreds of dengue cases and 25 deaths recently, has also reported a suspected JE case. Like dengue, JE virus is also transmitted by mosquito. A 40-year-old businessman, who was admitted in SCB Medical College and Hospital, Cuttack, had tested positive for JE in the Elisa test conducted at the Regional Medical Research Centre. But it remained a suspected case because findings of Elisa test are not confirmatory. The patient was discharged and no confirmatory test was done on him, a scientist of Regional Medical Research Centre said.

The RMRC here has started testing samples for JE besides other viruses. "We have tested over 310 samples so far," said Dr Kar, adding except for the one suspected case JE was ruled out in others. However, the RMRC virology laboratory had found Herpes simplex virus (HSV) 1 in 12 samples and HSV2 in eight samples in the past few months for the first time in the state. These viruses also cause encephalitis

Meanwhile, mosquito-borne diseases continue to take a toll in the state as 58 persons died of malaria, also spread by the winged monsters, so far (as on September 30) this year, a government statement said. The government said deaths due to malaria had declined as 162 persons had died by this time last year.

BRD Medical College this year so far 2562 patients have been enrolled, of which 253 are being treated in different wards. 455 people are dead in the cheek of time.

LUCKNOW/GORAKHPUR: Ten-year-old Rajat was lucky to recover from encephalitis. However, the son of an agricultural labourer from Kushinagar has become short-tempered and at times even hurls abuses at people and beats them.

Describing the change in his son's behaviour, Ram Sakha, said, "My son has become very short-tempered. He throws things here and there most of the time. Gets abusive and goes on to beat people irrespective of who the person is. He ran away from home a couple of times in a spate of anger."

Rajat is suffering from is a behavioural problem that is seen in about 35-50% children who survive encephalitis. "Children develop irritability, become short-tempered and easily lose calm mainly because their thinking and understanding capacities reduce drastically. Poor literacy rate doesn't allow parents to understand the condition of their children and they too lose patience. Such circumstances are not in the interest of the children and the society as well. Imagine what will happen when these children will grow up," said Prof K P Kushwaha, head of paediatric department at Baba Raghav Das Medical College, Gorakhpur. He explained that residual problems in entero-viral encephalitis (EE) were more severe as compared to Japanese encephalitis (JE). Even the hospital stay in EE (12-15 days) is higher than in JE (3-5 days).

However, the boy is still better, as a large number of children develop loco-motor problems due to paraplegia. "Children usually suffer from hemi (or half) paralysis. So, their hands or legs or one hand or one leg lose function," said Dr RN Singh, a local paediatrician working against encephalitis for the last three decades. "Such children need to be trained, so that they can live with the disability," Singh added.

Physiotherapy and training does help. Sanju, a teenager of Holiya village that has not seen any encephalitis case for the last 22 months, turned hemiplegic about seven years ago, is an example. Over the years, she has learnt to use her left hand which is stronger of the two to manage household chores. She can now cook, write (though with a lot of effort); tidy her hair besides other small tasks. Equally difficult is the life of children who lose sight or sense of hearing. "Our studies have shown that 3-12% children lose sight between six to 12 months of recovery," said Prof Kushwaha. Worst are the children who turn mentally challenged, their proportion being 18-20%. Such children usually have multiple problems with loss of speech (loss of flow of language, stammering and so on).

Lack of coordination between state and central government also comes into picture. When locals wrote letter in blood to the President and Prime Minister asking them to do something about this annual carnage, the union ministry of health and family welfare (MOHFW) released Rs 55.5 lakh for setting up a department of physical medicine and rehabilitation at the medical college. Soon after, funds were sanctioned. But nothing has happened in this direction.

When asked to comment, commissioner, Gorakhpur division, K Ravindra Naik said, "We have sent a proposal to the state education department for opening a special school for the survivors under Sarva Shiksha Abhiyan. The centre is meant to assess the extent and type of disability in the children and then help them accordingly." The said proposal is pending with the state government as of now. However, a school may not be enough for the children, who hail from far flung villages in different districts. Social workers, counsellors, special trainers need to be roped in and a module of training needs to be devised.

KEY POINTS:

Close to 30-35% children suffering with encephalitis, die.

Survival comes with disability and behavioural problems.

Disabilities relate to functionality of the limbs, senses with vision and speech in particular and even mental retardation.

The disabilities may also be overlapping. For instance, one-fourth of the mentally retarded children have speech related problems.

A behavioural problem, usually irritability and short-tempered attitude, is seen in about 35-50%children.

New Delhi, Oct 20 (PTI) The central team visiting Gorakhpur in Uttar Pradesh to review the outbreak of encephalitis has attributed the recent deaths to the consumption of contaminated water in the affected areas. More than 300 children have succumbed to the viral infection in Gorakhpur and Basti Mandal in Uttar Pradesh even though the figure is less than the mortality reported from the state's vulnerable spots in earlier years. Alarmed by the reports of deaths, Health Minister Ghulam Nabi Azad and AICC general secretary Digvijay Singh are on a visit to the poll-bound state today. Experts, however, made it clear that UP has always accounted for a majority of encephalitis burden in India. Of the 4185 cases of Japanese Encephalitis (JE) reported across the country in 2009 and almost as many in 2010, 71.5 per cent (2,996) were in UP alone. Sources say the recent problem in UP is due to enterovirus which transmits through water. "People are consuming contaminated drinking water which is one of the main reasons for the deaths in Uttar Pradesh, besides poor sanitation," said A S Dhariwal of the National Institute of Communicable Diseases (NICD). NICD experts have also found hand pumps in seven affected districts of Gorakhpur and Basti Mandal as the major source of polluted water and people in the area are using these hand pumps as a source of drinking water. The Centre has, meanwhile, asked Uttar Pradesh government to take immediate steps for setting up a department of Physical Medicine at BRD Medical College, Gorakhpur for speedy rehabilitation of Japanese Encephalitis patients who have suffered permanent neurolgical disorders.

Comment: I'm sceptical about all of this. According to the NVBDCP UP recorded 3,073 AES cases in 2009 (556 fatal) and 3,540 AES cases in 2010 (494 fatal). I can tell you right now that these cases did not all test positive for Japanese encephalitis and certainly there were not 2,996 positive cases of JE in UP in 2009. For example in 2010 only 417 of 3,303 patients with AES treated in Gorakhpur's BRD medical college tested positive for JE (see post #101). So the powers that be continue to fudge reality. They blame past outbreaks on Japanese encephalitis, and current outbreaks on an unidentified enterovirus. In most cases the diagnosis is symptomatic.
The enterovirus has not been identified, but according to the article above NICD experts have found hand pumps that are the source of the contamination. Quit the bluffing!!!!!! If you don't know what's causing it, you can't fix the problem!
Again, the article above indicates that more will be done to treat the permanent effects of encephalitis, but lack of attention to the cause will ensure a constant flow of new victims for those wards.

The Indian media needs to inform themselves better of the problem and challenge the lines being fed to them by government officials.

Gorakhpur: continue the brunt of encephalitis in Purvanchal. BRD Medical College in the last twenty-four hours twenty three new patients were admitted and two died.

The two people died and twelve years of Gorakhpur them seven years of aspiration and Siddharth are Golu. Still admitted to Nehru Hospital, 253 patients in various wards are being treated.

Azad was in Gorakhpur to take a stock of encephalitis situation in eastern UP. Encephalitis has claimed more than 461 lives in the region this year while 2,55 are still under treatment at the BRD Medical College where special arrangements have been made to tackle the annual epidemic.

Accompanied by senior Congress leader Digvijay Singh and Union minister of state for road transport and highways, RPN Singh, who is also an MP from the neighbouring Kushinagar, Azad was shocked to see the heavy rush of attendants and patients in the epidemic and encephalitis wards of Nehru Chikitsalaya of the college. Seeing two-three patients sharing a bed, Azad reportedly said that following the standard norms of public health was a far cry here.

Taking a round of the hospital, the minister found that cleanliness was pathetic in the hospital and patients were exposed to a perennial risk of hospital-borne secondary infections. Huge piles of garbage, including biomedical wastes, were littered on the premises. A big dumping ground, barely 20-30 meters from the epidemic wards, caught the attention of the minister. Also, stray animals like cows, bulls and dogs were found roaming in and around the wards.

After inspection, the minister held a meeting with the health officials in the college auditorium. Head of the pediatric department, BRD Medical College, Prof K P Kushwaha, briefed the minister on the situation. "Acute encephalitis syndrome (AES) is just broad categorization of the disease that kills scores of children each year. Both viruses and bacteria could cause AES but the ratio of viral with bacterial encephalitis is 98:2. Japanese encephalitis affects about 6% children is form of viral encephalitis. JE is almost preventable with proper vaccination. Encephalitis caused by several strains of entero virus is the chief cause of disease and death these days. It affects about 44% children and has no cure. It can, however, be avoided by ensuring clean drinking water and total sanitation. Other viruses, that cause encephalitis in the remaining lot, are yet to be isolated," Prof Kushwaha reportedly maintained.

Some reports released today on earlier fatalities in UP. Note vaccination status. When "result: AES" is given I assume that means tested negative for JE.

Government Announces Steps To Check It
Gorakhpur, Oct 21: With the encephalitis toll in eastern Uttar Pradesh mounting to 474 this year, the Centre today announced setting up of a coordination committee of different Union ministries and said it was mulling short, middle and long-term plans to control the epidemic.
"A coordination committee comprising union ministries of Health, Water Resources and Irrigation, Social Justice and Women and Child Welfare would be formed after discussion with the Prime Minister. The committee will make a coordinated effort to tackle the problem," Union Health Minister Ghulam Nabi Azad said.

Azad said the short, middle and long-term plans to control the epidemic will be worked out soon.
Terming the deaths in Gorakhpur Medical College Hospital as unfortunate, he said, "It seems that vaccination drives are not carried out in a transparent manner from time to time or else the situation would have been better."
He said cooperation from all quarters was necessary to check spread of the epidemic and expressed concern over the shortage of para-medical staff.
The Union minister said the central government has also sanctioned 44 nursing schools in the state to overcome the problem. "Japanese Encephalitis cases in eastern UP have come down from 36 percent in 2005 to 10 per cent this year, due to repeated vaccination drives but the cases of water-borne encephalitis are mounting," he said.

Gorakhpur: a series of deaths from encephalitis in Purvanchal is not unending. BRD Medical College in the last twenty-four hours in the 21 new patients were admitted, while five others were killed. Still admitted to Nehru Hospital, 245 patients are being treated different wards.
Among the five people died and five-year-old Anu of Gorakhpur, Deoria yearn for seven years, four years of Mhrajagnj pleasure, fifty-five years of Kushinagar Sntkbirnagr Rani and Vaishnavi are two-year period. Six in 21 people admitted to Kushinagar, Deoria, five, four of Gorakhpur, Mhrajagnj two and Bihar, Mau, Azamgarh and Basti - a patient. Overall, from January 2606 until the last patient arrived for treatment of whom 462 have died.

In the BRD medical college in Gorakhpur, 500 children have died of encephalitis in the last 2 months, 10 in the last 48 hours alone.

Could the health minister of the country Gulam Nabi Azad, seen here visiting the hospital last week reacted a little earlier to the crisis, been more pro active in saving India's dying children?

Yes, if this report from the National Disaster Managemnet Authority is to be believed.

On August 6 this year, the authority conducted a workshop with officials from easten UP warning of the crisis and asked Ghulam Nabi Azad to visit the area immediately.

The ministry and the minister SAT on the report for 2 months, ignoring the suggestion of a visit not once but twice. First on September 15 and then on October 3. Busy with Telangana, Azad found time to go to the encephalitis epicenter only on the insistance of Rahul Gandhi.

Japanese Encephalitis cases on decline
Monday, October 24th, 2011 With the rains over, cases of Japanese Encephalitis (JE) have begundeclining in poverty-ridden eastern Uttar Pradesh, where the mosquito-borne killer disease has claimed the lives of 507 children in the past four months. Two people are dying of the virus everyday, a health expert said Monday.

The recent visit by union Health Minister Ghulam Nabi Azad and Congress general secretary Digvijay Singh failed to provide any succor, say doctors.

“Even though the change in the weather has now reduced the daily inflow of patients to the nodal JE centre at B.R. Medical College here from 40 to about 25, the killer virus was still killing at least two persons every day here,” K.P. Kushwaha, head of the Encephalitis programme at the medical college told IANS.

Gorakhpur: encephalitis in eastern Uttar Pradesh, the situation remains serious. Recruitment of patients and has been a string of deaths. In the last twenty-four hours in BRD Medical College admitted 31 new patients, while eight others were killed.
Among the eight people were killed and three years Rvindu Shamim two years of Gorakhpur, Kushinagar Ramu and three years of Alexander's four-year, 65-year-old Amina Devi and two years of Mhrajagnj Rajan and 13 years in Bihar Durgesh and 12 years, including the baby.
Nine of those recruited to Kushinagar, Gorakhpur and Bihar, seven - seven, Mhrajagnj, Sntkbirnagr and Deoria two - two and a Ghazipur and Ballia - including a patient.
238 patients admitted to different wards of Medical College still going to treat. At the Medical College since January 2661 has come to treat patients of whom 472 have died.

Encephalitis toll climbs to 488
Gorakhpur (UP), Oct 25 (PTI)

With 10 more persons succumbing to encephalitis, the death toll in the viral disease in eastern Uttar Pradesh has climbed to 488 this year, health officials said today.


Ten persons suffering from the disease died in various hospitals in the region in the last four days, they said.

Over 280 people suffering from encephalitis are undergoing treatment at the BRD Medical College here besides in some other district hospitals.

As many as 3088 patients were admitted to government hospitals in the region this year of which 488 have died, they said
