In a potential epidemic, at least 61 people have died and 662
hospitalized due to encephalitis in the last 10 days in the district,
around 125 kms [78 mi] north-east of Lucknow.

The chief medical superintendent in-charge of the district hospital,
Dr RS Baranwal said encephalitis in the area was caused by
[transmitted by] mosquitoes that breed in stagnant water.

Encephalitis is most often caused by a viral infection and exposure to
viruses can occur through mosquito, tick, and other insect bites,
contaminated food or drink, breathing in respiratory droplets from an
infected person and skin contact. The disease caused by the herpes
simplex virus is the major cause of fatal cases in all ages, including
newborns, especially during or after monsoons in India. Last year
[2010], a large number of deaths due to encephalitis were reported in
Bihar and UP [Uttar Pradesh). "Therefore, it is important to maintain
cleanliness with an aim to prevent the disease from spreading," Dr
Baranwal said.

On Monday [13 Sep 2011], 3 more children died due to the disease
[encephalitis], taking the death toll to 61 in the district. 10 new
patients have also been admitted to the district hospital. The number
of patients suffering from acute fever has crossed 662. Of them the
condition of 6 children is stated to be critical. The blood and serum
samples of 13 patients have been sent to Lucknow medical college for
examination.

The paediatrician of the district hospital Dr.K.K.Verma said that it
was for the 1st time in 2 years that the patients in such a huge
number had been coming to the hospital. He said that last Tuesday 350
patients had visited the Out Patient Department (OPD), which was a
record in itself. Earlier, the maximum number of patients to OPD were
150 to 175.

Dr RS Baranwal said that considering the increasing number of patients
in the hospital a letter had been written to the government for more
medicine and resources. A new lot of medicine would arrive in a day or
2, he added.

Due to the increasing number of encephalitis patients in the hospital,
new patients are now lying outdoors on the ground and hoping to get
medical treatment.

for the numerous cases in the hospital. Previous recent reports of
encephalitis in this region indicated that Japanese encephalitis virus

Gorakhpur: Purvanchal in the recruitment of patients and deaths has been a string of encephalitis. BRD Medical College in the last twenty-four hours in four new patients were admitted, while six were killed. Among the six people were killed 7-year-old line of Gorakhpur, Karan 12 years, two and a half years and 13 years of smiles, Renu, Ritesh Mhrajagnj 14 years including 3 years Shivani. Different wards in medical colleges are still admitted to Nehru Hospital, 284 patients are being treated. Overall in the Medical College since January 1816 have come to treat patients of whom 303 have died.

PIL on encephalitis in Allahabad: HC seeks counter-affidavit

Allahabad, Sep 22 (PTI) Hearing a PIL raising concern over loss of over 15 lives here due encephalitis and lack of adequate medical facilities for its diagnosis and treatment, the Allahabad High Court today sought a reply from the Chief Medical Officer and the principal of city-based Swarup Rani Nehru Hospital.The order was passed by a Division Bench comprising justices Yatindra Singh and Dinesh Gupta on the PIL filed by a social activist Pervez Rizvi.The court directed the authorities to file their counter-affidavit within three weeks.In the PIL, it was alleged that 18 people have died in the district this year on account of encephalitis and that most of the patients hail from villages far away from the city where health centres and trained medical personnel are not available.Moreover, it was alleged in the PIL that even the city does not have a single lab where the facility of serum testing, which helps early detection of the deadly disease, is available.

UP govt has failed to check encephalitis: Cong

Lucknow, Sep 22 (PTI) With toll mounting to 319 in Japanese encephalitis in Uttar Pradesh, the state Congress today accused the Mayawati government of failing to take effective steps to check the disease. "There have been over 300 deaths in the state this year and it is a matter of shame for the state government which has not yet taken note of it," Congress spokesman Virendra Madan said. "The BSP government is most insensitive and has not yet initiated steps for checking the disease," he said, adding hundreds of patients have been admitted in hospitals in Gorakhpur, Azamgarh, Maharajganj, Kushinagar and Deoria and now even the central districts like Lucknow have come in the grip of the disease. At a time when parts of the country are reporting a surge in cases of Japanese Encephalitis, four persons in Delhi have been diagnosed with the disease caused by the mosquito-borne virus. This has been confirmed by officials of the Health department who said the four persons were in different hospitals. No JE case had been found in Delhi in recent times.

Of the four, one is from Gol Market in Central Delhi and is admitted at the RML Hospital. The other three are from the North West Delhi areas of Bawana, Pooth Khurd and Jahangirpuri...

Pediatrician is the unending stream of patients suffering from fever. District hospital is left in place for patients to enroll. From remote rural hospital, laying the mat on the floor of the children are undergoing treatment.

Treated at district hospital on Friday Shravasti Shravasti district, village resident Dinesh Pyarelal son was killed. They were suffering from high fever. To seven new patients were admitted. Meningitis which is not confirmed but the symptoms of meningitis in these patients are very obvious.

In the last two months of the district known in the surrounding districts, there is an outbreak of meningitis. So far seven deaths have been over. The Health Department is accepting only a dozen deaths. Hospital records show that so far 80 people have been confirmed meningitis. Most affected by meningitis Nepal border areas and flood affected areas. With the problem of where people have not been able Jlbrav. But in remote rural areas is not the Anti-larval spraying said Faging nor the machines are visible.

Among the six people died and nine years in Gorakhpur, Rahul, Pankaj six years, the consecration of a half years, two years of Mhrajagnj shekinah, Deoria and Bihar in the thirteen-year-old Sonu, including a year Bittu. Different wards in medical colleges are still admitted to Nehru Hospital, 273 patients are being treated. BRD Medical College since last January for a total of 1893 patients had been treated of which 318 have been killed.

Confirming four cases of Japanese Encephalitis (JE) in the city, officials on Friday said preliminary inquiries had established that the patients did not travel to endemic areas in the recent past and appeared to have contracted the virus in Delhi itself.

The state government had directed MCD and NDMC to conduct inquiries about four persons who had tested positive for JE in serological tests conducted by the National Centre for Disease Control (NCDC).

Of the four, three are children from Bawana and Jahangirpuri in North West Delhi while the fourth is an HIV positive woman from Gole Market in New Delhi.

Dr V K Monga, chairperson of the MCD public health committee, said: ?After the JE virus enters the body, it only takes between 5-15 days for the symptoms to manifest. This is known as the incubation period of the virus. No patient appears to have travelled to any endemic area in the last month.? He said though two patients had travelled to UP in February and July, the virus was contracted in the last 15 days.

The vector-borne disease is carried by a female mosquito of the Culex tritaeniorhynchus species. ?The virus is found in pigs and cattle. A mosquito which bites any infected cattle becomes a vector and can spread it to human beings. It has a very high mortality rate of 20-40 per cent. This is the first time that cases have been confirmed in Delhi, so we are worried,? Dr Monga said.

Both MCD and NDMC said they have taken immediate measures to control the spread of the virus. ?We have carried out intensive fogging and anti-larval treatment activities within a 2 km diameter of the area of residence of the patients in North West Delhi on Friday,? Dr Monga said. NDMC health officer Dr P K Sharma said similar measures had been taken in the Gole Market area.

The Municipal Corporation of Delhi (MCD) has suddenly woken up to the health and hygiene of pigs. Not because Delhiites have taken a huge liking for pork, but because cases of Japanese encephalitis ( JE) have surfaced in the city.
Now one might ask what pigs have got to do with Japanese encephalitis. The answer is that domestic pigs play host to the virus.

The MCD has also raised questions about pigs being brought to the city from areas where Japanese Encephalitis virus is common.
"This virus doesn't occur in Delhi," Dr V. K. Monga, chairman of the MCD's health committee, said, adding that pigs seem to be the reason for this disease.
"Piggeries are common in Delhi and there is a fair possibility of pigs being imported from areas such as Gorakhpur in UP where the disease is common. We will direct the pig owners to maintain proper hygiene in pigsties and not bring pigs from areas which are prone to this disease," Monga said.

With three more reported deaths in the last 24 hours at the Baba Raghav Das (BRD) Medical College, the cumulative death toll of Encephalitis in Purvhanchal and adjoining regions of Uttar Pradesh has reached 332 this year.

According to additional director health, Gorakhpur Mandal,


Diwakar Prasad, the three deaths in the last 24 hours include a child each from Gorakhpur, Kushinagar and Deoria districts.
"Out of 2,246 Encephalitis patients since January this year, 332 have succumbed to the disease," he said.

In disease season, some new steps, one old obstacle

The encephalitis season has peaked again in eastern Uttar Pradesh and Bihar, the Centre has stepped up research and the UP government has sanctioned funds for a long-term treatment plan. Yet the original problem remains ? identifying the exact virus behind a syndrome that sees patients testing negative for Japanese encephalitis, but which has been killing thousands, mostly children, year after year since 2006.


The season usually continues till the middle of October. Around 300 patients, 260 of them children, have already died in UP due to what has come to be known as acute encephalitis syndrome, or AES. Over 2,100 cases of patients with such symptoms have been registered from eastern Uttar Pradesh; 125 of these later turned out to be of Japanese encephalitis, which has caused 14 of the 300 deaths. Bihar has had 130 cases with 33 deaths. There have been 800 cases and 130 deaths of patients from Assam, too, but about half those cases have been of Japanese encephalitis, which is preventable by vaccination. Ro's note - officially Bihar has recorded 288 cases and 54 fatalities from AES - this does not include an additional 31 fatalities from a recent outbreak in Gaya. Assam has officially reported 1,308 cases and 246 fatalities (up to Sep 13th).

Experts of the National Institute of Mental Health and Neuro Sciences, Bangalore, have taken samples of cerebral spinal fluid from AES patients from all three states.


For UP, Chief Minister Mayawati has announced the construction of a separate, 100-bed special ward for Japanese encephalitis at BRD Medical College in Gorakhpur, sanctioning Rs 18 crore. For now, an 80-bed ward at the hospital has been reserved for AES/JE cases, eight ventilators worth Rs 2 crore will be acquired, and Rs 2 crore has been sanctioned for strengthening of the pediatric ward, with diploma seats in the medical college?s child health department raised from three to ten. The government has also ordered installation of more hand-pumps for clean water in affected areas.


The hosptal?s pediatric ward is overcrowded, with about 470 patients on 180 beds till the weekend. The proportion of lab-confirmed Japanese encephalitis cases is less that six per cent of the total that have been registered as AES.


Till 2006, Japanese encephalitis had been taken to be the primary killer. After the introduction of a vaccine in 2006, JE appeared to have been brought under control but patients continued to show encephalitis-type symptoms, the syndrome often leading to death, though very few of them tested positive for JE.


There is no specific medicine yet for AES. Preventive vaccination is available only for JE and doctors continue to give symptomatic treatment to AES patients against convulsions and kidney failure, and to help keep their respiratory track clear since most patients are admitted in a semi-conscious state.


?All experts now feel that the need is to bring about changes in the lifestyle and basic hygiene among people because a majority of the patients are affected with entero-viruses, which are contagious. Installation of some hand-pumps will not help because the population coverage would remain small. Still, a large population depends on contaminated sources of drinking water and defecate in the open. Until these practices are stopped, it would be difficult to control the spread of the disease,? said K P Kushwaha, head of the pediatric department at the medical college. He has been handling encephalitis patients for three decades.


The health department has deputed five extra doctors at the medical college to manage just encephalitis cases. ?The NIMHANS experts have told us that they have developed some new technique, where they can test a sample for over five-six viruses at a time within eight hours. We are hopeful that some results will come this time,? Kushwaha said.


Dr R N Singh, a private practitioner who has written letters in blood to the President and the Prime Minister on the subject, stressed the need for a national eradication program for encephalitis. With Assembly elections approaching, he said he would urge parties to include this in their agendas.


A senior state health official said the percentage of deaths is down slightly this year and the positivity of JE cases has decreased. ?The best preventive measures are being planned and we hope they will be effective. An awareness drive has been taken up, even pradhans have been educated about basic hygiene; new handpumps have been provided. Chlorine tablets are being distributed and fogging is taking place.?


However, some experts say that such efforts start after the peak season begins in the end of July, and are not continued round the year.


From Bihar, state surveillance officer A K Tiwari said, ?Central officers have visited affected districts like Gaya, Aurangabad and Nawada and collected some samples. But they have been doing similar exercises for many years without any results. We cannot be sure about the preventive measures that we can take because we are not sure about the virus.?

2007: New terminology


Acute encephalitis syndrome, or AES, became an expression widely used by doctors. Not all cases, it was found, were caused by JE virus. As per WHO, ?Clinically, a case of AES is defined as a person of any age, at any time of year with the acute onset of fever and a change in mental status (including symptoms such as confusion, disorientation, coma, or inability to talk) or new onset of seizures (excluding simple febrile seizures).?


2008: New laboratory


NIV established a field laboratory on premises of BRD Medical College, Gorakhpur. NIV senior scientist Dr M M Gore researching there since. The same year, AES cases showed rising presence of entero-viruses that enter through oral-faecal route; research began to identify the exact entero-virus causing this from among hundreds.


2009: Vaccine controversy


State refused to use about 16 lakh doses of encephalitis vaccine meant for routine immunisation, saying their efficacy was doubtful. The previous year, 6 lakh doses had been wasted over allegations that they were ineffective after heat exposure. Dr R N Singh, private practitioner, wrote in blood to the Prime Minister to draw his attention to the outbreak.


2010: Fresh round


This time, it was vaccines meant for a special campaign. Centre asked UP to use previous year's leftovers, UP refused. Later, Centre agreed to send 25 lakh fresh doses and special vaccination took place seven districts ? in November 2010, after the JE season had ended.


2011: No let-up


In eastern UP so far, over 2,000 cases, over 300 deaths; only 125 confirmed JE cases and 14 such deaths. Of 1,925 deep-bore hand-pumps to ensure clean water, promised in Gorakhpur, Mahrajganj, Deoria and Kushinagar, only 375 installed.

The outbreak of Japanese encephalitis in and around Ranchi has assumed pandemic proportions, with authorities at Ranchi Institute of Medical Sciences (RIMS) reporting 12 child deaths since August 16 till date.

Moreover, 36 among the 81 children admitted to the state-run hospital have been confirmed to be suffering from the mosquito-borne viral disease. All 36 are children, between the age group of five and 11 years.

Japanese encephalitis, marked by high fever, headache, neck stiffness, disorientation, coma and seizures, strikes mostly younger children, as teenagers and adults are largely immune to it.

Though authorities are not willing to come on record on this, the last casualty seems to be of Latehar?s Priyanka Kumari (11) on September 24. Priyanka was admitted to RIMS on September 20.

Confirming the broad figures of patients under treatment, RIMS director Tulsi Mahto said: ?A total of 81 patients suffering from high fever came to our hospital. We sent blood samples of 51 suspected cases to the department of microbiology. Tests confirmed 36 were Japanese encephalitis-positive. Eleven tested negative and the results of the four are awaited.?

He added they had adequate medicine stocks, but the severity of the disease was such that some children succumbed. ?We are getting patients from across the state and are trying to accommodate them. At the paediatrics and neonatology department, there are 90 beds but patients inflow is high,? Mahto added.

A.K. Sharma, head of department of paediatrics and neonatology at RIMS, said they had been successful in saving the lives of most children. ?We are doing our best. Till now, we have been able to save 90 per cent of the patients. In many cases, people have admitted their children here after visiting private hospitals,? Sharma claimed.

?The death toll stood at 12 till Tuesday,? he added.

On the reasons behind the disease, Sharma said: ?Poor sanitation and unclean drinking water. Cases are mostly coming from rural pockets.?

Experts say that Japanese encephalitis, caused by Culex mosquito, which breeds in rice fields, marshes, waterlogged drains and pig farming regions, is a poor man?s disease.

Not surprisingly, most cases at RIMS are from city outskirts and rural areas.

An unconscious Vikas Kumar (6) in bed No. 4,019 at the Intensive Care Unit (ICU) of RIMS is one of them. His parents, Ramdev Ram and Rukmani Devi, from Hazaribagh district, admitted the boy on Sunday evening.

?We found out this Japani fever affects the brain,? Ram, a daily wage earner, said, seeking comfort from the doctor?s assurance that his son would be fine.

An unconscious Urmila Kumari (7) of Chatra district was admitted on September 19. ?Her condition is very serious,? said father Bhokta Jati.

Meanwhile, confirmatory tests will start in the less affected East Singhbhum district from Tuesday at Mahatma Gandhi Memorial Medical College and Hospital.

Across the district, 58 suspected cases have been reported at MGM and the premier Tata Main Hospital. But the district was waiting for Elisa test kits to arrive from the renowned National Institute of Virology (NIV), Pune.

One kit, able to test 90 cases, arrived on Saturday, delayed by a few days. Authorities, seeing the situation was under control, had continued with symptomatic treatment.

Suspected brain disease kills 51 kids in Bihar

Suspected encephalitis, a brain infection, has killed 51 children in Bihar's Gaya district in the last seven weeks, with three more kids succumbing to the disease, an official said on Saturday.

"Three more children died on Friday," an official said. Most of the children belong to Mahadalit families - poorest of the poor.


According to district health officials, all children have died at Anugrah Narain Medical College and Hospital (ANMCH) in Gaya, about 100 km from Patna.
The children reported high fever, followed by bouts of unconsciousness and convulsions.

AK Ravi of the hospital said that till date, more than 220 children with suspected encephalitis were admitted for treatment. "Most of the children were from rural areas of Gaya and neighbouring districts," he said.

"Over two dozen children are still battling for life," he said.

A district administration official said that suspected encephalitis hit Gaya in 2009, 2007 and 2005 and killed dozens of children.

Two months ago, 55 children died in Muzaffarpur district but the state government is yet to confirm these as encephalitis deaths. (but they have been included in the NVBDCP's national tally - Ro)

Last month, Union health and family welfare minister Ghulam Nabi Azad informed the Rajya Sabha that clinical and epidemiological data suggested that it was an outbreak of acute encephalitis syndrome, resulting in 150 cases and 55 deaths, mostly among children.

In a written reply, the minister said these cases were reported from early June to mid-July from Muzaffarpur and its bordering areas in Bihar.

The eight people died and half years for them to Dilip Gorakhpur, Kushinagar Bittu's three and a half years, forty years Ramsurt, eight-year-old patient of tonality and three years, seven years of Mhrajagnj Naseem, Deoria Shivam eight years, Rahul and includes one year of settlement.
Twelve of the 43 patients enrolled in Kushinagar, Gorakhpur Deoria eight of the eleven, Mhrajagnj six, and township and Siddharth Sntkbirnagr one of the four - including a patient. Still Nehru Medical College Hospital, 287 patients admitted to different wards are being treated. Overall the BRD Medical College since January 2126 have come to treat patients of whom 287 have died. (The real figure for UP is around 360 fatalities the 287 seems to be a repeat of the number currently hospitalized.-Ro)

More and more often, reports are including mention of this second water borne disease. Often the pathogen is referred to as an enterovirus. To my knowledge no single bacterium has yet been identified. - Ro

UP: Encephalitis claims eight more lives


Gorakhpur (UP): With eight more deaths reported in the past 36 hours, the cumulative death toll due to encephalitis in eastern Uttar Pradesh has risen to 376 this year. Of the persons who have succumbed to the disease, four are from Kushinagar and one each from Mahrajganj, Deoria, Basti and Gorakhpur districts, said Diwakar Prasad, Additional Director Health, Gorakhpur division.



They were undergoing treatment at BRD Medical College here and other government hospitals in adjacent districts, he said. According to data available, as many as 44 patients have succumbed to various kinds of encephalitis in the region in the past 10 days.



Prasad said that out of 2,480 encephalitis patients admitted in the government hospitals in the region this year, 376 have succumbed to the disease. At present, 321 encephalitis patients are undergoing treatment at BRD Medical College and hospitals in adjacent districts. 43 fresh patients of encephalitis have been admitted in BRD MCH and district hospitals of the region during the past 36 hours, Prasad said.



Experts say two mainly kinds of encephalitis are prevailing in the region -- Japanese encephalitis, which is caused by mosquito bite while the other type of encephalitis occurs due to consumption of unsafe drinking water. Both are vector borne diseases, they said. (PTI)
