Two people succumbed to encephalitis here, taking the toll in the viral infection in eastern Uttar Pradesh this year to 516. Health officials said on Saturday that the two died at the Baba Raghav Das Medical College Hospital here yesterday. The victims hailed from Gorakhpur and Sant Kabrinagar.

"Nine patients suffering from encephalitis have been admitted to the hospital on Friday," Additional Director (Health) U K Srivastava said.

"As many as 103 patients are presently undergoing treatment at various government hospitals in the region," he said.

"The vaccination drive to contain Japanese Encephalitis is on in 24 development blocks of Gorakhpur, Deoria, Kushinagar, Mahrajganj, Basti, Siddharthanagar and Sant Kabir Nagar," Srivastava said.

Twenty eight children have died due to encephalitis in Mahrajganj district of Uttar Pradesh in the last two months, health department officials said in Mahrajganj on Sunday.

"In October-November, 28 children have died due to encephalitis in Mahrajganj ," Additional Director Health, U K Srivastava said.

"Since January, 359 encephalitis patients were admitted for treatment, of which 49 children died...

UP's spat with Centre over JE vaccine ends

LUCKNOW: Six months' long dispute between Union and state governments over unusable vaccines to prevent Japanese Encephalitis finally ended with a special vaccination drive. The drive aims to cover 67.46 lakh children of the age group 1-15 years across seven districts of east UP. Officials said that the drive that started last week is expected to gain full momentum in the week starting December 6.

The drive will continue for 15 days in districts of Gorakhpur, Maharajganj, Kushinagar, Basti, Deoria, Sant Kabir Nagar and Siddharthnagar. With the aim of examining the levels of immunity provided by the vaccine, a seroprevalence study will also be conducted. The study is a joint effort of UP government, National Institute of Virology and PATH, a non-profit organisation.

Special JE drive extended in four affected eastern districts

The special immunisation drive for Japanese Encephalitis (JE), which was to conclude on Wednesday in six out of seven earmarked eastern part of the state, will now be extended for another three days.


As of now, while almost 77.5 per cent immunisation has been achieved in these districts, the districts which are extending their drives till December 19 is expected to take the figure up to at least 85 per cent.

A total of 67.9 lakh children, between the age group of 1 to 15 years, were to be vaccinated in these seven worst-effected districts. Till Thursday, Gorakhpur ? with the highest target population of 14.58 lakh ? has shown the highest immunisation result of up to 84.6 per cent. Sant Kabir Nagar and Basti have reported 78.4 and 77.4 per cent immunisation respectively. as of now, the lowest immunisation report has come from Siddharthnagar ? 71.7 per cent.


?Most districts have asked for an extension in the last date of the drive, as the aggregate result is currently 77.5 per cent,? said Dr V S Nigam, Joint Director of Vector Borne and Communicable Disease (Health Directorate), Uttar Pradesh. ?With the drive continuing for three to five more days, we expect the figure to reach between 85 to 90 per cent,? he added. According to the data of the last 15 days, an average of 5 to 7 per cent target group is being immunised every day in these districts.

The Gorakhpur region in eastern Uttar Pradesh (UP) is a paddy (rice)
cultivating area. That means the combination of Culicine mosquitoes
that breed in rice fields and water birds that develop Japanese
encephalitis (JE) viremia. Thus, it is a JE affected eco-type area; in
2006 and since then some JE vaccination (Chinese live SA14-14-2) has
been going on. In 2006, when a mass campaign was conducted, the
prevalence plummeted. Since then the EPI [Expanded Program on
Immunization] mode of vaccine delivery is beset with many problems, so
JE continues relentlessly. The National Institute of Virology [NIV]
(of Pune, Maharashtra, an ICMR [Indian Council of Medical Research]
Centre) has established a field station in Gorakhpur for
investigations. That is how we know that there are JE cases and non-JE
cases.

The non-JE "acute encephalitis syndrome" could be one or more
conditions. Enterovirus(es) have been proposed since both the field
station and the Lucknow Medical College virologist have isolated many
enteroviruses including a rare isolate from CSF. The NIV people
discount the enterovirus theory but the Lucknow people insist it is an
enterovirus disease.

It cannot be an enterovirus syndrome since it is annually recurrent
and geographically confined. Enteroviruses spread widely. My guess is
that it may not even be a viral syndrome -- virologists look only for
viruses. It could even be a non-infectious problem (such as the
'hepatomyoencephalopathy' we have described in western UP -- it was
called Saharanpur encephalitis for over 2 decades and investigated by
NIV repeatedly. Our systematic studies showed it to be _Cassia
occidentalis_ poisoning of children who ate lots of the beans. A
Google search on Saharanpur encephalitis is a way to search or look
for hepatomyoencephalopathy, or else I can send a series of
publications.

As Dr John indicated, clearly a health landscape view ("healthscape")
is essential to determine the etiology and epidemiology of the cases
of encephalitis that have been going on in this area of Gorakhpur.
Until that information is in hand, prevention and control will not be
possible. Vaccination to prevent Japanese encephalitis virus
infections will be helpful, but for less than half the reported cases.
Another (other) etiological agents that are endemic in the area must
be involved in the cases termed acute encephalitis syndrome. Dr John
mentions possible involvement of the plant _Cassia occidentalis_ (or
_Senna occidentalis_) with the common name of coffee senna or
coffeeweed. It contains toxins in all parts (seeds, roots, leaves,
fruits). The symptoms of intoxication include diarrhea, tremors and
dark brown urine if plant parts are consumed in large quantities. The
toxic compounds in it include anthraquinones, emodin glycosides,
toxalbumins, and alkaloids.

LUCKNOW: With 512 children succumbing to the dreaded Japanese Encephalitis/ Acute Encephalitis Syndrome (AES) in Gorakhpur Medical College alone this year, the demand for a National Encephalitis Eradication Programme gets louder in eastern UP.

The deadly virus has claimed more than 10,000 lives in the belt since 2006. And, even as the state health department attributes the casualties to a different strain every year, the recurrence, despite a vaccination drive, is leading to much anxiety in the 18 affected districts of Purvanchal .

Already one lakh 100,000letters, 1,000 out of them written in blood, have been sent to Sonia Gandhi, Rahul Gandhi Prime Minister and the union health minister to bring the point home . HFM informed that 5149 Acute Encephalitis Syndrome (AES) cases and 677 deaths were reported in 2010. Of these, there were 565 cases and 110 deaths due to JE in 11 states.
