Gorakhpur, Sep 5 (ANI): At least 220 are feared dead following an outbreak of encephalitis in Uttar Pradesh?s Gorakhpur District.

Doctors said that children between the ages of six months to 15 years have been the worst affected and most victims are poor people from rural areas.

Hundreds of children and malnourished patients have been admitted to hospitals for treatment, exposing the rickety medical facilities in the region.

?The situation is not good. Till now, at least 220 people have succumbed to the disease, as per government reports. However, the actual toll could be much higher. The situation is bad,? said Brajesh Singh, a relative of one of the patients.

?This is because the preparations to tackle the outbreak are unsatisfactory. There are not enough medicines and doctors are overburdened. That is why they have not been behaving with the patients properly,? he added.

The overflow of patients, many of whom are in a critical condition, has raised fears among the medical fraternity, who are battling an acute shortage of doctors and infrastructure support.

?In the last few hours, at least 17-18 children have died. Many more have been admitted and their condition is set to be critical. Doctors and nurses are not available for treatment, and neither is any bed vacant for serious cases,? said Kamlesh Kumar, a local.

According to doctors, the death rate due to encephalitis is 28-56 percent.

Many survivors are left mentally or physically handicapped. (ANI)

At the BRD Medical College in Gorakhpur, the paediatrics department emergency ward is overflowing with patients ? two-to-a-bed is the rule, many beds have even three. There is no other way to accommodate 240 patients on 108 beds.

This is the season of the ?mastishk jwar? which ravages Gorakhpur and ten adjoining districts in eastern UP from mid-July to October every year.


About 250 children have died this year. Many who survived have been disabled by the virus that attacks the brain. Last year, 556 died. In 2008, the count was 495; in 2007 it was 525; and, in 2006, 476. In no other place in India does a disease kill so many children every year.


Until 2004, there was uncertainty about the nature of the disease. But in 2005, when an outbreak killed 1,200 children, medical experts across the world started looking at Gorakhpur closely.


The Atlanta-based Centre for Disease Control and several others conducted studies and concluded that the main killer was Japanese encephalitis.



Despite reservations, the Centre and the UP government ensured extensive JE vaccination. But children kept dying and it turned out that a large number did not test positive for JE. All cases were classified as AES (acute encephalitis syndrome).


Two years ago, the Pune-based National Institute of Virology set up a field laboratory in Gorakhpur to identify the cause of the disease. It came up with the finding that the culprit was a class of enteroviruses. Work is still on to make the research more specific.


Dr M M Gore, the scientist in charge of the NIV lab, said they installed equipment to speed up work. Samples have been sent to the Centre for Disease Control and the results are awaited.


While the research will take time, a few things are clear. Both JE and enteroviruses are killing children. Though it remains a problem, the JE mortality rate is declining. The main killer is the new class of enteroviruses; the disease these cause is, for want of a more specific term, still recorded as AES.


Like JE, the new viruses attack children. Like JE, these attack the brain, and other organs. Like JE, these have no medicine and patients have to be given symptomatic treatment. Like JE, these have a high death rate, in fact higher.


Unlike JE, the new enteroviruses have no vaccine; that will have to wait until the nature of the viruses is fully understood. Unlike JE, which is spread by mosquitoes, the enteroviruses spread via contaminated water through what medical experts call the ?faecal-oral route?.


But like JE, AES is preventable. If fogging, larvacidal sprays, removal of piggeries (hosts to the virus) and preventing stagnation of water can counter JE, hygiene and sanitation can tackle AES.


Dr K P Kushwaha, Head of Paediatrics Department at the BRD Medical College, said: ?We can only take measures to provide life support; recovery is natural in these cases. In AES, multiple organs are affected and we can only give symptomatic treatment. It will be difficult to eradicate the disease unless there is a public movement for preventive measures.?


To take a look at the conditions where the disease breeds, one only has to visit Rajdhani, 25 km from Gorakhpur city in Brahmapur block. Seven-year-old Khushi Prajapati has returned from BRD Medical College ? two other children were not so lucky.


Khushi was hospitalised after she had convulsions and high fever.Though she has recovered, she can?t move her hands properly and has a speech problem. Her father Rajendra Prasad Prajapati, who works in Gorakhpur city in an electric shop, has built a two-storied house minus a toilet. The family of 12 defecate in the open. There is stagnant water all around. There are also four piggeries in Bairagi Tola.


Residents say this has been the setting here for decades. Their children have been dying of ?mastishk jwar? but no one ever told them that the deaths could have been prevented if some simple steps had been taken.


Dr M M Gore, who has been travelling through villages in the last two years, said: ?Gorakhpur has a high water table which makes it prone to contamination. Many people here defecate in the open and use mud near handpumps to wash their hands. Eating utensils are washed at the same place. Villagers have constructed concrete houses but not toilets.?


?If such habits are changed, basic hygiene is taught to people, steps taken to ensure sanitation, and a proper drainage system put in place, it will go a long way in solving the problem,? said Gore.


Gorakhpur district has 13 primary health centres and nine community health centres but not one has equipment like a ventilator. So sick children have to be sent to the medical college. The Health Department has 21 fogging machines, but only five work ? the other 16 have been undergoing repairs for a year now.

Five more persons died due to encephalitis here taking the death toll in Uttar Pradesh's eastern region to 254 this year, a health official said today.

"All the deaths occurred in BRD Medical College Hospital," additional director (health) UK Srivastava said.

Of the five dead, two belonged to Kushinagar and one each from Deoria, Santkabir Nagar and Basti, he added.

Of the total 254 deaths, 238 occurred in BRD hospital, while the remaining took place in other government hospitals of the region - Gorakhpur, Siddharthnagar, Kushinagar, Maharajganj, Basti and Deoria districts, he said.

Meanwhile, 30 new patients have been admitted in BRD Medical College Hospital, while six were admitted at other government hospitals of the region, he said.

As many as 325 patients are presently undergoing treatment at the BRD hospital and various other government hospitals of the region, he said.

NEW DELHI, Thursday 9 September 2010 (Bernama) -- A deadly virus outbreak that attacks the brain has killed nearly 230 people, majority children, while another 400 people have been warded in hospitals in Uttar Pradesh state.

Initial reports revealed that a large number of victims from poor villages succumbed to the Japanese Encephalitis (JE), but doctors now worry that the dangerous enterovirus could be the main culprit.

"Two major causes are Japanese Encephalitis and the more complicated is the enterovirus, which is linked to polio-viruses and Coxsackie that affects the whole body, especially the brain," Dr K. P. Kushwaha, head of Paediatrics Department in BRD (Baba Raghav Das) Medical College, based in Gorakhpur district, told Bernama today.

Uttar Pradesh, one of India's most backward states lacking in proper healthcare facilities and clean drinking water, compounded the medical distress since July, at a time when the monsoon also arrived in the northern region.

The Indian Express reported that BRD Hospital's emergency centre was brimming with patients, where two or three sick people had to share a single bed, and villagers were living in "fear and panic".

People in at least 10 districts were exposed to the virus, the worst-hit being the Gorakhpur district in the state.

Last year, 556 people were reported dead in Uttar Pradesh, but 2005 was a most tragic year for the villagers when the outbreak killed 1,200 children in Gorakhpur, said the newspaper.

"Here, we do not have proper healthcare facilities. There are no toilets and safe drinking water. It is a complex problem.

"Children suffer inflammation of the whole body and most of them come from poor background. No single drug is available to treat patients, we have to prescribe multi anti-viral drug to treat different affected organs," said Dr Kushwaha.

Seven more deaths due to encephalitis in UP

Mahrajganj (UP), Sep 12 (PTI) Seven more children have succumbed to encephalitis at Baba Raghav Das (BRD) Medical College Hospital here, while 35 fresh cases of the deadly disease were admitted in adjoining district hospitals since yesterday, health officials said here today.

With this, the toll due to the deadly disease has mounted to 281 since January 2010, they said, adding more than 1,814 children suffering from encephalitis had been admitted in the medical College at Gorakhpur and other hospitals since the beginning of the year.

He said a total of 314 patients of suspected encephalitis are at present being treated at various district hospitals in the region.

Gorakhpur (UP), Sep 19 (PTI) Six more persons succumbed to encephalitis at a hospital here, taking the toll in the deadly disease in eastern Uttar Pradesh to 312 this year, Health officials said today.

The six deaths have been reported since yesterday in Baba Raghav Das Medical College hospital, where 25 new patients were admitted today.

Additional Director (Health) U K Srivastava said about 90 per cent of total deaths have occurred due to acute encephalitis syndrome, caused by enterovirus present in contaminated water.

Rest were due to Japanese encephalitis, he added.

Srivastava said of the 312 deaths so far, 292 took place in BRD Medical College hospital, while others were reported from government hospitals in Gorakhpur, Siddharthnagar, Kushinagar, Maharajganj, Basti and Deoria districts.

Brain fever death count touches 358

Gorakhpur, Sep 27 (PTI) Thirty-nine people succumbed to encephalitis in the past one week in eastern Uttar Pradesh pushing the death count from brain fever in the region this year to 358.

Eleven of the 39 fresh deaths occurred in the past two days, Additional Director (Health) U K Srivastava said.

The vector-borne deadly Japanese Encephalitis (JE) toll in eastern Uttar Pradesh today reached 361, following the death of three more children in the last 24 hours.
Official sources said so far, 2,354 JE patients had reported at the Baba Raghav Das Medical College here and other district hospitals in the division, of which 361, including 38 from Bihar and one from Nepal had died.
Since yesterday, 31 fresh JE cases had come to light. Almost 272 JE patients were still convalescing at the various hospitals.
The fresh deaths included one each of Gorakhpur, Kushinagar and Bihar.
JE patients mostly trickle from Gorakhpur, Maharajganj, Sant Kabirnagar, Deoria, Kushinagar, Basti, Siddharthnagar, Azamgarh, Gonda, Bahraich, Mau, Ballia and Balrampur districts of UP.
Gorakhpur has the dubious distinction of being the JE epicentre in the region.
The endemic afflicts eastern UP during monsoon every year and continues to take people under its jaws till the onset of winters.

The mosquito-borne Japanese encephalitis has claimed several lives in Uttar Pradesh this year but there are other infectious agents - called enteroviruses - that are believed to have caused over 200 fatalities, health officials say.
Only 18 people have died due to Japanese encephalitis Jan 1-Sep 9, says V.S. Nigam, joint director of the encephalitis eradication programme (Uttar Pradesh), but enteroviruses have been blamed for 234 deaths, which includes many children.

The deaths that took place in various districts, including Gorakhpur, Kushinagar, Siddharthnagar, Basti and Sant Kabir Nagar, have been recorded under Acute Encephalitis Syndrome (AES), which experts say could be spurred by enteroviruses.

“Ongoing research indicates that enteroviruses could be responsible for the AES cases. But we are yet to understand the nature of the viruses completely,” Kushwaha said.
Significantly, officials of the field laboratory in Gorakhpur set up by Pune-based National Institute of Virology (NIV) two years ago have also blamed enteroviruses for the deaths of people showing encephalitis symptoms but didn’t tested positive for JEV.
According to experts, the high-mutation rate of enteroviruses makes them difficult to study and therefore there’s no specific medicines or vaccination available to treat these infections.

Unlike JEV that spreads from mosquitoes, enteroviruses spread through contaminated water and enter the body through the faecal-oral route.

Gorakhpur (Uttar Pradesh): Four persons succumbed to encephalitis at Baba Raghav Das Medical College hospital in Gorakhpur during the last 24 hours, taking the death toll to 414 in the eastern region of Uttar Pradesh, a health official said on Wednesday.

Additional Director (Health) U K Srivastav said the dead include one each from Gorakhpur, Mahrajganj, Gonda and Ballia districts.

A total of 34 new patients have been admitted to this hospital during the same period, he said.

Of the total 414 deaths so far, 386 deaths occurred in BRD MCH while the remaining were reported from other state-run hospitals of the region - Gorakhpur, Basti, Deoria, Siddharthnagar, Kushinagar and Maharajganj districts.

As many as 290 patients are presently undergoing treatment at the BRD MCH and 19 in other government hospitals for the disease, he said.

Imphal, October 12 2010: Hindrances came in the way of taking up Japanese Encephalitis Vaccination Programme in the state when underground activists snatched away 'auto disable syringes' to be used in the programme.

The snatching away of the syringes led to the deferment of the vaccination programme which was scheduled to be launched from today.

Family Welfare Department today issued a notification regarding that postponement of the launching of the programme citing the reason as 'due to an unavoidable circumstance'.

A highly reliable source said that the programme which targeted mass vaccination of children for prevention from Japanese Encephalitis was set to be launched today in four valley districts and one hill district of the state.

Vaccine for the programme was already with the family welfare department.

The auto disable (AD) syringes to be used in the programme was procured from New Delhi.

However, the truck transporting the syringes has been hijacked by suspected UGs on the way after crossing Mao for Imphal on NH-39, somewhere at Tadubi area in Senapati district.

The matter of the hijacking came to light when the truck failed to arrive at Imphal even as officials of the family welfare department were informed that the truck was heading towards Imphal.

Official searched for the trucks for three days.

So far, there are no reports of a complaint having been lodged with the police regarding the missing truck.

Apart from this, it could not be confirmed whether the truck had actually been released.

The AD syringes was imported from China by the state family welfare department through an NGO in consultation with the Union health ministry.

A source from the family welfare department said that the responsibility of delivery of the item from New Delhi to Imphal lies with the Central government and the NGO importing the item.

The department is to receive it only at Imphal.

They are not the authority for lodging a complaint with the police about the hijacking, it maintained.

The AD syringes is not similar to that of ordinary syringes in use.

The WHO defines it as syringes that are automatically rendered unusable after having delivered a prescribed dose of vaccine.

With Manipur identified as one of the states in the country with a high prevalence of Japanese Encephalitis, the mass vaccination programme was being taken up.

The programme was to be taken up first in five districts, Imphal east, Imphal west, Thoubal, Bishnupur and Chandel district.

The programme is targeting to vaccinate 50,000 children across these five districts.

Hijacking of vehicles transporting life saving drugs and healthcare related kits procured from outside the state is not a new phenomenon on NH-39 .

In recent times too, there was an instance of hijacking three vehicles carrying life saving drugs belonging to NRHM (National Rural Health Mission) on this highway.

The deadly Japanese Encephalitis (JE) toll in Uttar Pradesh has reached 424 with the death of six more children in the last 24 hours.
According to sources today, the inflow of JE patients continued unabated at the Baba Raghav Das Medical College here.
The fresh deaths included two each at Maharajganj and Kushinagar, while one each died in Gorakhpur and Siddharthnagar districts.
Sine January 1 this year, a total of 2861 people -- including 34 fresh cases -- sufferring from JE have been admitted at the medical college here, of whom 424 have succumbed. The deceased included one of Nepal and 43 of Bihar.
As many as 303 people were still under treatment.
Last year, JE had claimed more than 1,000 lives in UP alone.
The patients belong to Gorakhpur, Deoria, Maharajganj, Sant Kabirnagar, Kushinagar, Basti, Siddharthnagar, Azamgarh, Gonda and Balrampur district of UP. Some cases also pertain to the adjoining districts of Bihar.
The endemic afflicts eastern UP during monsoon every year.

Four die of encephalitis

Gorakhpur (UP), Oct 16 (PTI) Four people suffering from encephalitis succumbed to the deadly brain fever at Baba Raghav Das Medical College Hospital here in the last 24 hours, taking the death toll due to the disease to 428 in the eastern part of Uttar Pradesh, a health official said today.Thirty-one new patients with acute symptoms of encephalitis have been admitted to the BRD Medical College Hospital during the period, he said.The deceased include three hailing from Gorakhpur and one from Mahrajganj, Additional Director (Health) U K Srivastava said.As many as 268 patients are presently undergoing treatment at the BRD hospital and 24 in various government hospitals of the region, he said.

IM behind hijacking of anti-JP vaccination kits

Imphal, October 15 2010: The hijacking of 'auto disable syringes' for Japanese Encephalitis Vaccination Programme was the handiwork of the NCSN (IM) outfit which is under a ceasefire trust with the government of India.

Mention may be made that due to the missing of the truck carrying the AD syringes the mass vaccination programme due to be launched by the state family welfare department from October 12 was postponed indefinitely.

The syringes had been dispatched to Manipur through a Kolkata based farm.

The farm has intimated the state government about the dispatching of the syringes.

On investigation, it was found that the truck was detained by the NSCN(IM) on the way to Imphal before entering Manipur territory on NH-39, a highly placed source said.

The hijacking is reported to be linked with extortion demands but it could not be confirmed whether the demand is made to the company transporting the item or to the Manipur government.

The state government has not apprised the centre of the hijacking since transportation of the item is not the responsibility of the state government.

Apart from this, the transporter is yet to furnish documents relating to the dispatchment of the AB syringes even though they had informally informed the state authority about the dispatchment of the said item.

Another highly reliable source said that the truck with syringes worth around Rs four lakh has not crossed into Manipur territory.

There were no entry records of a truck carrying such items in the record books of any of the police check posts on the stretch of the highway in the Manipur section.

However, the truth of the NSCN (IM) hijacking the truck has reportedly reached the ears of the centre and the centre is understood to be negotiating with the outfit to release the truck and ensure its reaching Imphal at the earliest, the source disclosed.
