But I think we have to take all precautions so that in case if it is a risk in Hong Kong, we can find out as early as possible.

Please note how SFH is calling 'another case' and not uses a different wording such as 'other sporadic cases', or ''other cases'.

I think the words choice is not casual and the next 24/48 hours will be critical to see whether this additional case will materialize or not.

Hong Kong: Steering Committee on Avian Influenza updates avian flu precautionary measures (11/18/10)

The Chairperson of the Steering Committee on Avian Influenza and the Secretary for Food and Health, Dr York Chow, this morning (November 18) convened an inter-departmental meeting on avian influenza following the confirmation of a case of Influenza A (H5N1) infection by the Centre for Health Protection (CHP) yesterday (November 17).

The meeting reviewed the latest situation and preventive measures being taken by various parties against avian flu.

The inter-departmental meeting was attended by representatives from the Department of Health (DH), the Hospital Authority (HA), the Agriculture, Fisheries and Conservation Department (AFCD), the Food and Environmental Hygiene Department (FEHD), the Education Bureau, the Social Welfare Department, the Home Affairs Department, the Tourism Commission and the Information Services Department.

Dr York Chow said the Serious Response Level under the Government's Preparedness Plan for influenza pandemic had been activated.

To prevent the spread of the virus, the inter-departmental group had decided to take a series of actions according to the contingency plan for the Serious Response Level.

Accordingly, the CHP under DH has stepped up surveillance by testing all severe pneumonia cases for H5.

A telephone hotline 2125 1111 has been set up to answer public enquiries.

The CHP has written to all medical practitioners alerting them of the development and has liaised closely with HA to step up surveillance.

The department has also sent letters to institutions, elderly homes and schools to remind them to be vigilant against avian influenza.

The World Health Organisation and the Ministry of Health of China have been informed of the case.

For public hospitals, the HA had activated the Serious Response Level to implement a series of measures to enhance surveillance and infection control. The HA will closely monitor all suspected as well as confirmed cases and enhance the existing report mechanism.

Furthermore, high risk patients attending Accident and Emergency Departments and general out-patient clinics will be triaged according to stipulated clinical guidelines, and appropriate tests will be performed when it is clinically indicated.

Under the Serious Response Level, more stringent infection control measures will be enforced in public hospitals, which include no visiting at isolation wards unless on compassionate grounds.

For general acute hospitals, visiting time is limited to not more than two hours a day and two persons at a time; for infirmary/convalescent hospitals, it is limited to not more than four hours a day and two persons at a time.

Visitors to public hospitals and clinics are required to wear surgical masks and ensure hand hygiene before and after visiting patient areas.

The FEHD has also taken a series of corresponding measures. They include stepping up inspection at public market stalls and fresh provisions shops, enhancing surveillance and random checking of any incompliance with the prohibition against overnight stocking of live poultry at the retail level and stepping up surveillance, cleansing and disinfection work at public markets, especially Yan Oi Market in Tuen Mun, as well as enhancing surveillance on live poultry at the import level. Cleansing and disinfection work has also been carried out by the staff of the Department at the patient's apartment as well as the common parts of the building concerned.

In addition, the AFCD will complete the inspection of all 30 chicken farms within these two days. The department will step up the inspection of the poultry wholesale market and the Yuen Po Street Bird Garden to remind traders to stringently implement biosecurity measures. The department will continue its wild bird monitoring and surveillance and collect swab samples at various locations for H5 testing. The department has also reminded poultry farmers, wholesalers, pet bird shop owners, Mai Po Nature Reserve, Hong Kong Wetland Park and Ocean Park, etc. to remain vigilant and strengthen precautionary measures against avian influenza.

The Government will step up publicity efforts. The Information Services Department will increase the broadcast of Announcements in the Public Interest (APIs) on avian influenza prevention on television and radio to further advise the public on precautionary measures. In addition, the Education Bureau has issued a letter advising schools to stay alert in preventing the spread of avian influenza on school premises. A letter from the CHP is enclosed for schools' reference.

The Committee also requested departments, including the Social Welfare Department, Tourism Commission, Home Affairs Department and other relevant organisations to take vigorous precautionary measures to prevent the spread of avian flu.

Latest laboratory results received by the Department of Health (DH) today (November 18) revealed that all eight close contacts of the H5N1 case were negative for Influenza A (subtype H5).

The eight, three family members and five other patients who shared a cubicle with the H5N1 patient in a Tuen Mun Hospital ward, were being quarantined at Princess Margaret Hospital.

The H5N1 patient is also in Princess Margaret Hospital and is still in serious condition.

Meanwhile, three more contacts who shared a meal with the H5N1 patient on November 11 have been identified. None of them has fever. Contact tracing continues.

As at 5pm today, a total of 18 public enquiries have been received on the DH hotline set up yesterday.

A spokesman for DH said a special edition of Communicable Disease Watch featuring the H5N1 case, among others, has been published at the Centre of Health Protection website.

【An informed interaction on.cc East CHP Dr Thomas Tsang said the bird flu in the families of the quarantine and observation in hospital, the hospital has conducted tests of their samples of this, I believe there will be 1 to 2 after the preliminary results, No other suspected cases. Confirmation of a case of human influenza A (H5N1) infection in Hong Kong

The Centre for Health Protection (CHP) confirmed a case of human influenza A (H5N1) infection on November 17, 2010 affecting a 59-year old lady living in Tuen Mun.

She enjoyed good past health.

She presented with runny nose since November 2, 2010, fever and cough with blood streak sputum since November 5.

She consulted doctors with medications given.

Her symptoms persisted and she attended the Accident and Emergency Department of Tuen Mun Hospital (TMH) on November 12 with chest X-ray (CXR) showing left middle zone haziness.

She was given a course of Augmentin.

She had increasing haemoptysis and persistent fever and was admitted to general ward in TMH on November 14.

Her condition deteriorated and CXR taken on November 16 showed consolidation of entire left lung field and right lower zone haziness.

She was then transferred to intensive care unit for close monitoring.

She was treated with oseltamivir and was given 100% oxygen but did not require ventilatory support.

She is currently in serious condition in Princess Margaret Hospital.

Her nasopharyngeal aspirate was tested positive for H5 influenza A virus by PCR on November 17, 2010.

She travelled to Mainland China (Shanghai, Nanjing and Hangzhou) with her husband and daughter from October 23, 2010 to November 1, 2010.

She had history of visiting a wet market in Mainland China during the incubation period but had no history of direct contact with poultry or birds.

She came back to Hong Kong on November 1.

The source of infection is being investigated.

Her 60-year-old husband had history of running nose and productive cough but has recovered.

Her close contacts are put under quarantine. Chemoprophylaxis was offered to close contacts and respiratory specimens were tested negative.

In response to the confirmation of this case, the Government has raised the influenza response level from ?Alert? Response Level to ?Serious? Response Level under the Framework of Government?s Preparedness Plan for Influenza Pandemic on the same day.

CHP has also enhanced surveillance for avian influenza, apart from the existing statutory notification system for suspected Influenza A (H5, H7, and H9) cases.

Doctors are requested to refer patients fulfilling the following criteria to the nearest Accident and Emergency Department under the Hospital Authority for further management:

  • A person presented with influenza-like illness (fever >38oC and either sore throat or cough); AND
  • History of visiting wet market or contacting poultry in Shanghai, Nanjing or Hangzhou within the incubation period (i.e., 7 days before onset of symptoms).

This was the first confirmed human influenza A (H5N1) infection recorded in Hong Kong since 2003. Previously in 1997 and 2003, Hong Kong recorded 18 and 2 cases of human infections of H5N1 avian influenza respectively. They were 10 males and 10 females with ages ranging from 1 to 60 years. Ten (47%) children were aged below 12 years. Seven (33%) of the 21 cases died and the others recovered. The cases in 1997 coincided with an outbreak of high pathogenic avian influenza caused by a virtually identical virus in poultry farms and live markets and the outbreak stopped after mass culling operation. The two cases in 2003 were imported cases affecting a 33-year-old man and his 9-year-old son who had travelled to Mainland China.

According to the latest update from the World Health Organization (See Map above) (October 18, 2010), worldwide there were so far a total of 507 confirmed cases (including 302 deaths) of human infection with the H5N1 avian influenza virus.

【On.cc Oriental interaction an informed one of the 59-year-old woman living in Tuen Mun yesterday confirmed the infection of H5N1 avian influenza. Department of Health said the latest test results show, the families of patients with 3 and 5 the same room patients infected with H5N1 avian influenza were not, they are in the Princess Margaret Hospital for isolation. The Princess Margaret Hospital were still present in serious condition.

In addition, patients with 3 people on Thursday who were dining, they do not have fever. The Administration indicated that it will continue to track those who had contact with the patient. Hong Kong, China (CNN) -- The first human case of bird flu in seven years has been diagnosed in Hong Kong, health officials said Thursday, urging the public to take precautions.

A 59-year-old woman was hospitalized in serious condition after a trip to mainland China, the Hong Kong Department of Health said.

Her illness was diagnosed as influenza A (H5), a variant of bird flu. It was unclear where she had contracted the disease. She did not have any contact with live poultry and did not visit farms while in China, Hong Kong health officials said.

Her 60-year-old husband developed possible symptoms of bird flu, but has recovered. (and likely tested negative - alert)

Despite some surprise reflected in news outlets after the discovery of this human infection with A(H5N1) avian influenza virus in Hong Kong, this virus is currently circulating widely in other part of the world, such as Egypt, Vietnam, Indonesia with sporadic human cases.

The occurrence of H5N1 in humans is no a new phenomenon and this situation will not change in the foreseeable future as the animal dispersion of the pathogen in a large geographical area.

For the current health condition of the closest contacts of the 59 year old female Hong Kong patient, there were several confirmation of preliminary tests results from authoritative sources:

Avian influenza – situation in China, Hong Kong Special Administrative Region (Hong Kong, SAR)

19 November 2010 -- </SPAN>The Centre for Health Protection (CHP) of the Department of Health (DH), Hong Kong, China, has reported a new confirmed human case of H5N1 infection on 17 Nov 2010. The case is a 59-year-old female. She had onset of symptoms on 2 November and was admitted to hospital on 14 November. She was diagnosed to have pneumonia and is now in a critical condition.
The case had a history of travel to Shanghai, Nanjing and Hangzhou. She reported no history of contact with live birds and poultry in the days before showing symptoms. Her close contacts are under surveillance and are isolated. All remain asymptomatic to date. Nasopharyngeal aspirates of all close contacts were negative for H5. Investigations into the source of her infection are ongoing. Evidence so far indicates a sporadic case of human influenza A(H5N1) infection without secondary spread.

Hong Kong residents experienced an unwelcome blast from the past on Nov. 18. A 59-year-old woman in the city contracted H5N1 avian influenza ? the first confirmed case in Hong Kong since 2003. It's not clear how she became sick. She traveled to China Oct. 23 to Nov. 1, visiting several cities ? and according to the Hong Kong government, she spent time at a live poultry market in mainland China and ate chicken while she was there. She started showing flu symptoms the day after she returned to Hong Kong, and was admitted to the hospital with a high fever on Nov. 14, and she's in serious condition.

On November 17, 2010, Hong-Kong health authorities reported a human case of influenza A(H5N1). The case is :

  • 59 year-old woman ;
  • She presented 1st symptoms on November 2nd ;
  • Hospitalised on November 14, she is in critical condition ;
  • She travelled to Shanghai, Nanjing (Jiangsu province) and Hangzhou (Zhejiang province) in mainland China (cf. Figure 1) with her husband and daughter between October 23rd and November 1st.
  • Contacts with sick/dead poultry have not been documented but she visited market.

  • Hong-Kong has implemented a comprehensive influenza surveillance system.
  • The last human cases in Hong-Kong were reported in February 2003; These 2 cases were certainly infected in Fujian province, mainland China.
  • The last epizootic reported in Hong-Kong occurred in the 1st trimester 2009 (wild birds). The last epizooty among poultry was notified in 2008.
  • In mainland China,
    • The last human case in mainland China was reported in June 2010 in Hubei province.
    • The last epizootic was reported was in the 3rd quarter 2010 (wild birds) in Tibet (Xizang).
    • Veterinary surveillance and reporting in Mainland china is suboptimal, it is also likely that A(H5N1) human cases are under detected and/or under reported.

Comments

According to available information this case detected in Hong-Kong is very likely to have been imported from mainland China.

  • To date, the origin of contamination is unknown.
  • The occurrence of cases without documented contacts with poultry in a country where few human and animal cases have recently been reported is worrying. This also raises questions regarding actual A(H5N1) situation in mainland China .
  • Hong-Kong health authorities further investigate. The situation is followed up carefully.

Following is the transcript of remarks made by the Secretary for Food and Health, Dr York Chow, at a stand-up media session after attending the Bone Marrow Donor Award Ceremonyheld by the Hong Kong Bone Marrow Donor Registry today (November 20):

According to the reports from the hospital, the patient is in a stable but still considered as serious condition. She is still receiving oxygen, although the oxygen concentration has decreased from 100% to 70%, which indicates her lung is actually improving a little. So we regard that her condition is stable and also slightly improving. According to the work that is being done by the Agriculture, Fisheries and Conservation Department, they have gone through 30 chicken farms in Hong Kong in the last few days, and also obtained cloacal swabs for testing. So far the tests from all these swabs were negative. The tests are still going on, but so far we feel that the risk of avian influenza in Hong Kong is still slim, and also there is no indication of human to human transmission.
