The Centre for Health Protection (CHP) of the Department of Health (DH) received 27 reports of institutional outbreaks of influenza-like illness (ILI) affecting 25 schools, a residential home for the disabled and a hospital ward involving 197 people today (March 11).

A CHP spokesman said ILI may be caused by influenza or other respiratory viruses.

CHP's officials have visited all these institutions and provided health advice to the staff accordingly.

The spokesman called on members of the public to consult medical advice before taking Tamiflu which is an anti-viral drug to treat influenza.

He said Tamiflu was a drug in the Third Schedule of Pharmacy and Poisons Regulation. It must be prescribed by a doctor and dispensed under the supervision of a pharmacist. Drug stores are also reminded to observe the relevant legislation strictly. Anyone who is found guilty of an offence under the Regulation will be liable on conviction to a maximum fine of $100,000 and imprisonment for two years.

To prevent influenza, members of the public are urged to maintain good personal and environmental hygiene.

They are urged to build up good body immunity by having a proper diet, regular exercise, and adequate rests, reducing stress and avoiding smoking.

Government adopts further measures to prevent and control influenza

The Secretary for Food and Health, Dr York Chow, today (March 11) convened a meeting with relevant bureaus and departments, including the Department of Health (DH), the Hospital Authority (HA) and the Education Bureau (EDB), to discuss the latest situation of seasonal influenza in Hong Kong and to step up preventive and control measures.

In the meeting, members discussed the possible developments of the current seasonal influenza peak.

They also considered and put forward a number of possible contingency measures to enhance the prevention and control of influenza.

Dr Chow has decided to set up an expert group under the relevant Scientific Committee of the Centre for Health Protection (CHP) of DH to thoroughly investigate the three recent fatal cases involving children amid the recent seasonal influenza peak.

The expert group is chaired by Professor Yuen Kwok-yung.

Experts of microbiology, clinical medicine, forensic medicine, pathology and toxicology will be invited to join the study where necessary.

Regarding the recent case of a seven-year-old boy who died today (March 11) after admission to Tuen Mun Hospital with respiratory and neurological symptoms, DH has conducted an epidemiological study on the primary school where the boy had studied.

Taking into account the severity of the case, the occurrence of suspected influenza cases in the boy's school, and the need to facilitate further investigation and disinfection work, the Government has required the school to advance its Easter holidays to March 12 until March 25.

The EDB and CHP have sent staff to meet with the school??s management to learn more about the latest situation of influenza-like illness among the students, discuss the arrangements of advancing the Easter holidays and advise on measures to prevent and control the spread of influenza in the school.

DH and EDB would maintain close liaison during the school leave period and keep in view the latest situation to decide whether the school can resume class as scheduled. The school campus will remain open during the leave period.

Students, if need be, could go to school as usual.

The school will arrange teachers to take care of students who go to school.

The school will make subsequent arrangements to defer its summer holidays.

The CHP and EDB will continue to closely monitor the influenza situation in all schools and adopt appropriate measures according to the actual situation.

EDB has issued circulars to schools on guidelines and advice to prevent and control influenza.

EDB and DH will hold four briefing sessions for schools from tomorrow to Friday (March 14) to provide more detailed information concerning influenza to teachers, students and parents.

CHP will continue the enhanced surveillance over influenza outbreaks in schools and introduce further measures in light of latest developments, including whether to suspend classes, to prevent the spread of influenza.

On public hospitals, the headquarters of HA and different clusters will continue to closely monitor the daily hospital occupancy rate, attendances and admissions at Accident and Emergency Departments (AED) to facilitate the deployment of manpower to ensure the provision of necessary services in public hospitals.

Contingency measures include recruiting and deploying additional healthcare staff to support the operations of AED and reduce elective surgical operations.

"The Government will continue to stay fully alert, closely monitor the peak of seasonal influenza, put in place preventive and control measures in view of further developments and to ensure the effective implementation of such measures to protect public health," Dr Chow said.

Members of the public are urged to build up good body immunity by having proper diet, regular exercise and adequate rest, reducing stress and avoiding smoking.

Children, elderly people and those with chronic diseases should consult their doctors for medical advice promptly and take more rest at home if they have influenza-like symptoms.

The cause of the boy's death has not been determined. Officials have said that some of his classmates were believed to be suffering from the flu, and the government closed the school earlier this week.

No cause has been determined in the two other deaths that occurred last week _ girls ages 2 and 3 _ officials said.

The deaths came amid a series of flu outbreaks in Hong Kong over the past several days. Health officials have confirmed a total of six outbreaks at schools, a hospital and a nursing home for the elderly since March 6. SARS infected 1,755 people in Hong Kong and killed 299.

None of the cases have been linked to bird flu, which was recently detected in birds in Hong Kong.
Bird flu remains difficult for humans to catch, but scientists fear the virus that causes it could mutate into a form that spreads easily among humans and trigger a pandemic that some say could kill millions.

The school closures are the first in Hong Kong since the 2003 outbreak of the flu-like SARS, or severe acute respiratory syndrome, which killed 299 people, infected 1,755 and led to widespread panic across the former British colony.

Concerns over the current outbreak, which also led to the death of a girl, 3, on March 1, has seen the return in schools and hospitals of the face masks that were ubiquitous during the 2003 crisis.

Government officials have also appealed to people to stop stockpiling the drug Tamiflu, which is used to counter avian flu. There is no evidence yet of any infections involving bird flu.

Emergency units at hospitals, meanwhile, said they are struggling to cope with the volume of parents bringing in children with flu symptoms as fears about the flu outbreak mount.

Chow said Wednesday that the current outbreak appeared to be worse in primary schools and told reporters, 'We will monitor the situation in the coming days to see if there is a real increase, a surge, in the influenza virus in our community.'

Chow suggested the government might leave secondary schools open and order primary schools to close. 'If we have to adopt a closure policy, it would affect mostly the primary schools,' he said.

There is anxiety in the school, for students and parents, as well as teachers and the management. So we feel this is appropriate for us to advise the school to close down for a period of resting so the people can go home. We will follow up on those infected cases to see whether they have any unusual features. After the clean-up of the school, when students come back perhaps in two weeks' time, everything can start afresh. We will look at similar arrangements if there are any unusual elements or unusual appearances of infected cases in other schools. Whether we need to close down all the schools, higher schools and primary schools alike, it seems that the primary schools are being more affected than the higher schools. If we have to adopt a closure policy, it would affect mostly the primary schools. Whether the primary schools should close down, we have to see for a few more days. We will monitor the situation to see whether we have a real surge in influenza virus in our community.

Speaking to reporters this morning, Dr Chow said under normal circumstances, only the coroner can disclose or explain the causes of deaths referred to the court.

However, if necessary, the Government may consider seeking the coroner's permission for early disclosure due to public health and safety reasons.

Noting five other STFA Ho Yat Tung Primary School students also came down with symptoms and have been admitted to hospitals, Dr Chow said the Government has decided to close the school temporarily for a detailed investigation. The five students are now stable, without any complications, he added.

Overall assessment

He said the most important thing to do at the moment is to grasp a clear picture of the outbreaks and whether they are spreading.

All schools must report to the Education Bureau the number of students on sick leave due to flu and the number admitted to hospitals because of the illness.

The Hospital Authority will also provide relevant figures and information to the Department of Health for follow-up. If necessary, the Government will step up its measures.

Demand surge

Dr Chow did not rule out the possibility of closing schools, but said any such decisions should be made after considering the overall situation.

Due to the peak flu season, there has been a rise in the number of emergency ward visitors, ranging from 6% to 10%. The Hospital Authority will deploy more resources to cope with the surge, he said, adding the authority is able to handle the situation.

Dr Chow reiterated the Government will continue to monitor the development closely and more flu tests will be conducted to assess the situation.

No common factors between the deaths have been identified at this stage, according to the site.

``We're not considering shutting down the entire school system,'' Matthew Leung, press secretary to Chow, said in a phone interview today. ``Each case has to be determined separately. We don't rule out any possibility.''

A seven-year-old boy died yesterday at the city's Tuen Mun Hospital and the school he attended will close early today for the Easter holiday break amid the flu outbreak, Radio Television Hong Kong reported.

Hong Kong's health department late yesterday said it had received 27 reports of institutional outbreaks of illnesses with flu symptoms at 25 schools, involving 197 people. The department is giving daily updates.

The government in the city of about 7 million people has formed a group to investigate the deaths of the children, RTHK said, without citing anyone.

The deaths will be investigated by a team headed by Yuen Kwok-yung, a University of Hong Kong professor in microbiology, the South China Morning Post said today.

More than 30 students have flu-like symptoms and one of them died on Tuesday.

Two other young children have died in separate cases from flu-like illnesses in recent weeks.

An expert team is to assess whether the cases are linked to either bird flu or to Sars, the severe acute respiratory syndrome that hit Hong Kong in 2003.

'Very concerned'

Seven-year-old Law Ho-ming was admitted to hospital semi-conscious with fever and flu at the weekend.

He died on Tuesday from serious encephalitis and swelling of the brain.

Five of his fellow students from the Ho Yat Tung Primary School remain in hospital and more than 30 others are displaying flu-like symptoms.

Government medical officers said they were "very concerned" about the outbreak and had brought forward the Easter holiday at the school by 10 days.

The school closure is the first since 2003, when almost 300 people died from Sars.

Concerns have also been raised about two other recent child deaths from flu.
