- Takanabe-cho, Koyu-gun, Miyazaki:
- On 30 January 2011, poultry was tested positive for influenza A with rapid test at a farm in Takanabe-cho.
- Virus was identified as H5 with RT-PCR on 31 January and as H5N1 on 3 February.
- Mandatory movement restriction has been enforced within 10 km around the affected farm since 30 January 2011.
- All of the poultry at the farm was destroyed by 31 January.
- Takaoka-cho, Miyazaki city, Miyazaki:
- On 1 February 2011, poultry was tested positive for influenza A with rapid test at a farm in Takaoka-cho.
- Virus was identified as H5 with RT-PCR on 1 February and as H5N1 on 5 February.
- Mandatory movement restriction has been enforced within 10 km around the affected farm since 1 February 2011.
- All of the poultry at the farm was destroyed by 2 February.
- Miyao, Oita city, Oita:
- On 2 February 2011, poultry was tested positive for influenza A with rapid test at a farm in Miyao.
- Virus was identified as H5 with RT-PCR on 2 February and as H5N1 on 7 February.
- Mandatory movement restriction has been enforced within 10 km around the affected farm since 2 February 2011.
- All of the poultry at the farm was destroyed by 3 February.
- Takachiho-cho, Nishiusuki-gun, Miyazaki:
- On 4 February 2011, poultry was tested positive for influenza A with rapid test at a farm in Takachiho-cho.
- Virus was identified as H5 with RT-PCR on 5 February.
- Mandatory movement restriction has been enforced within 10 km around the affected farm since 4 February 2011.
- All of the poultry at the farm was destroyed by 5 February.
- The outbreaks in Nobeoka city and in Kawaminami-cho reported as H5 in follow-up report No. 6 were confirmed both as H5N1 on 2 February.
