"Hepatitis A outbreak: Ashland restaurant served potentially contaminated food

...Oregon health officials said Evo's Coffee Lounge was using Townsend Farms Organic Antioxidant Blend in its Radically Free smoothie, potentially contaminating customers.

"They were still using the product when the FDA called them on Wednesday," said William Keene, senior epidemiologist with the Oregon Public Health Division. "So people may have been exposed as recently as this past week."

Costco pulled it from its shelves and sent out robocalls to members about the outbreak but a number of people have said they didn't get a call.

Evo's sold about a half dozen of the smoothies a day between May 17 and June 12...

...The Food and Drug Administration informed Oregon health officials about potential exposure at Evo's on Friday. FDA officials have been calling bulk purchasers of the berry blend who might have dished up the product, as Evo's did, to unsuspecting customers..."
