March 13 (Bloomberg) -- The U.S. may add GlaxoSmithKline Plc's vaccine booster to shots made by Sanofi-Aventis SA, enabling more people to be protected against a possible pandemic sparked by bird flu, a Glaxo executive said.

A so-called adjuvant developed by London-based Glaxo has been shown to increase the potency of vaccines in human tests, and would allow the U.S. government to immunize more people with existing supplies, David Stout, the company's chief operating officer, said in an interview yesterday.

Adjuvants are substances used to enhance the ability of an antigen, the key ingredient in vaccine, to stimulate the immune system. Glaxo's adjuvant-enhanced vaccine is about 25 times more effective in protecting against the H5N1 avian influenza strain than shots made by Paris-based Sanofi. Glaxo has held talks with U.S. officials about buying its product, Stout said.

``We think the value is in the adjuvant, not in the antigen,'' he said.

Antigens are foreign substances in the body that trigger the immune system to form infection-fighting antibodies.

About 30 companies worldwide, including Novartis AG and MedImmune Inc., are working on pandemic vaccines and other treatments as world health officials track the spread of H5N1.

Worldwide, there is enough capacity to produce about 300 million doses of seasonal flu vaccine within six months, David Fedson, a former director of medical affairs in Europe for Sanofi, said in an interview in July. The amount of pandemic flu vaccine that could be produced will depend on the amount of antigen required per shot, he said.

More Power?

Glaxo's proprietary adjuvant, also used in its experimental Cervarix vaccine against cervical cancer, might help the U.S. restore some power to its vaccine stockpile.

The Department of Health and Human Services said in November that one of five shots in its storehouse had begun to lose their effectiveness as they sit on the shelf. The reduction in potency means that the stockpile now contains enough vaccine to fully immunize 3 million people against H5N1, down from 3.75 million, HHS said then.

The Sanofi shots in the U.S. stockpile contain 90 micrograms of H5N1 antigen. Two shots are needed for full protection.

When used with the company's own adjuvant, the Glaxo vaccine provides protection with two doses each as small as 3.75 micrograms. The smaller the dose, the more shots can be made from a given supply of viral antigen, Stout said.

Glaxo charges from about $6 to $11 worldwide for its seasonal flu vaccine, and prices for its H5N1 vaccine will be close to that range, Stout said. He wouldn't say how much the company would charge the U.S. for adjuvant alone.

Antiviral Drugs

Governments are stockpiling vaccine to prepare for the possibility that H5N1, a virus that has spread in birds to at least 56 countries in Asia, Africa, the Middle East and Europe, will start spreading quickly in humans. Of the 278 people known to have been infected since late 2003, 168 have died, according to the World Health Organization.

In addition to vaccine, the U.S. and other governments are stockpiling antiviral drugs such as Roche Holding AG's Tamiflu pills and Glaxo's inhaled Relenza.

Glaxo has stopped development of injectable forms of Relenza, which health officials had said might help treat severely ill avian-flu patients, Stout said.

``We asked people if they were going to buy any and they said `No, we want it to be there just in case,'' he said. The company faced ``huge, expensive clinical programs without any buyer.''

``It's just on hold,'' he said. Orders for the available, inhaled drug have slowed, he said.

Anthrax Scare

The U.S. has stretched its vaccine supplies before to prepare for possible disease outbreaks, Stout said. After five people were killed by attacks with anthrax through the mail in 2001, health officials called on Glaxo and other drugmakers to discuss their ability to make supplies of smallpox vaccine, he said.

Experiments later showed that existing government supplies of stored vaccine could be diluted to make enough for a possible attack, he said. Governments may be seeking to stretch stockpiles of pre-pandemic flu vaccines in a similar way.

``Now they're buying antigen at this 90 microgram dose, figuring `They can't charge me much for a single dose and if I get 90 micrograms then I can buy GSK's adjuvant and use that down at 3.75 micrograms,''' he said.

Glaxo, Novartis AG, and U.S.-based Iomai Corp. received contracts in January to design and develop treatments for pandemic flu including those using adjuvants.

Novartis, based in Basel, Switzerland, got a $55 million contract. Glaxo's is worth at least $63.3 million and Iomai won a $128-million contract to develop a dose-sparing patch to be used with flu vaccine.

New Studies Indicate GSK's Pre-Pandemic Influenza Vaccine Can Protect Against Different Strains Of H5N1

GlaxoSmithKline (GSK) announces clinical trial data from two new studies, which show, for the first time, that GSK's candidate pre-pandemic split antigen H5N1 vaccine, formulated with GSK's proprietary adjuvant system, provides a substantial level of cross-immunity against a 'drifted' (diverse) strain of H5N1. It is hoped that the immune response elicited with this vaccine, could help prepare or 'prime' the immune system to rapidly respond against variants of the H5N1 strain and therefore protect the vaccinated population in the event of an H5N1 human pandemic.

In the first study presented at the IX International Symposium on Respiratory Viral Infections (ISRVI), data demonstrated GSK's proprietary adjuvanted candidate pre-pandemic vaccine, containing very low levels of the Vietnam H5N1 antigen (3.8?g), elicits a strong cross-immune neutralizing antibody response in humans against the Indonesian strain of the virus. The neutralizing antibody seroconversion factor at Day 42 was shown to be 25 times greater when the vaccine contained the adjuvant system compared with that observed in the control group immunized with a non-adjuvanted vaccine (77.1% vs. < 3%).

GSK's proprietary adjuvant system also displays powerful antigen-sparing properties as the strong neutralizing response in this study was obtained with only two doses of 3.8?g HA antigen administered 3 weeks apart. This could mean, in effect, that by adding GSK's proprietary adjuvant system vaccine manufacturing capacity could be increased more than tenfold. The vaccine was shown to have an acceptable safety profile when compared to the control group. As expected, reactogenicity (injection site tenderness) was slightly higher in the vaccine group due to the use of the adjuvant system.

In a second study presented at ISRVI, the data showed that GSK's proprietary adjuvanted pre-pandemic vaccine could protect against two diverse H5N1 flu strains, again at very low levels of antigen. The in vivo data from the pre-clinical studies demonstrated that GSK's adjuvanted vaccine, containing the Vietnam H5N1 strain, was not only able to protect against challenge with the vaccine virus strain but it also provides 96% (22/23) cross-protection against a lethal challenge with the drifted Indonesia strain of H5N1, giving an additional boost to hopes that pre-pandemic vaccination is a viable strategy for inclusion in pandemic preparedness plans.

Jean St?phenne, President, GlaxoSmithKline Biologicals, the vaccine division of GSK, commented, "I am extremely encouraged by the new trial data that has been reported today on GSK's candidate pre-pandemic influenza vaccine. The data confirm that our pre-pandemic influenza vaccine has the ability to recognize and kill an H5N1 strain that is different to the one contained in the vaccine. This means that proactive administration of our pre-pandemic vaccine before or just after the start of the pandemic could help to substantially slow down the spread of disease."

Pandemic influenza poses a serious global public health threat with substantial estimated morbidity and mortality. The exact strain that may cause an influenza pandemic cannot be accurately predicted. Many experts, however, believe that the avian H5N1 strain, now endemic in many bird species across the world, is the most likely candidate pandemic strain.

"We believe this vaccine will provide governments with a new option to help protect their populations against the threat of a future human influenza pandemic," commented David Stout, President, GSK Pharmaceuticals.

Trial Study Designs

First Study

Cross-reactive immunity with an adjuvanted H5N1 candidate vaccine

This pivotal observer-blind, randomized study was conducted in 400 adults (18 to 60 years) to evaluate the reactogenicity and the immune response of GSK's pre-pandemic candidate vaccine administered at four antigen concentrations (3.8?g, 7.5?g, 15?g or 30?g HA of H5N1 clade 1* A/Vietnam/1194/04 split virion) either non-adjuvanted, or in combination with GSK's novel proprietary adjuvant system, against the drifted clade 2 A/Indonesia/5/05 strain. Two doses were administered 21 days apart and blood samples collected on Days 0, 21 and 42. The conventional 4-fold increase in neutralizing titers, before and after vaccination, was used to assess seroconversion.

Second Study

Protection against challenge with a variant H5N1 strain in an animal model with a low dose adjuvanted split H5N1 vaccine

Four animal test groups (N=23) were immunized intramuscularly with 15, 7.5, 3.8 or 1.7?g of H5N1/A/Vietnam/1194/04 (clade 1; NIBRG-14) vaccine containing a novel proprietary adjuvant system. The 2 control groups were immunized with either the adjuvant alone or the non-adjuvanted H5N1/A/Vietnam/1194/04 vaccine (15 ?g). Animals were vaccinated on days 0 and 21 and challenged on day 49 with 105 TCID50 (50% Tissue Culture Infective Dose*) of H5N1/ A/ Indonesia/5/05 (clade 2).

The quantity of a cytopathogenic agent, ie such as a virus, that will produce degenerative changes (or a diseased condition) in 50% of the cell cultures inoculated.
