Germany is going to chance its strategic plan in novel influenza pandemic.

The Robert Koch Institute has published a new report on how to change Germany?s strategic plan according to the evolving epidemiological situation.

There had been an early stage of the pandemic spread in Germany with only a few cases from foreign countries and now there comes a second stage ?of a permanent increase of cases? and more in-country transmissions.

Aim of the strategic plan for stage 1 would have been "containment and delay".

Aim for stage 2 now will be the reduction of the total number of cases ("mitigate/protect").

The literature on different models of prognoses and effects of measures to mitigate the pandemic spread of the influenza pandemic are discussed. The impact of different types of preventive measures on the public health system during the pandemic are reviewed. At a certain point of the epidemic curve the burden for the public health service for containment efforts and case tracking has to be reduced. Legal requirements of notification will be changed. New "epidemiological trigger points" will be defined. Diagnostic approaches will no longer be made in all suspected cases of novel influenza but will be focused on serious cases. Surveillance then will focus on the data of the numbers of serious cases, numbers of hospitalisations, fatal cases and special laboratory results (e.g. resistance tests).
