Genetics might explain why some people get bird flu

SINGAPORE, May 4 (Reuters) - People who have been infected with the H5N1 bird flu virus might be especially susceptible to avian viruses because they are genetically predisposed to them, leading disease experts suggested on Thursday.

Of the 205 reported cases of human infections since late 2003, there have been many family clusters involving blood relatives, such as father and children, mothers and daughters. Of the total infections, 113 people died in nine countries.

"There have been family clusters. So there has to be certainly a genetic aspect to it," Robert Webster of the St Jude Children's Research Hospital told a bird flu conference organized by the Lancet medical journal in Singapore.

Another leading expert Hiroshi Kida, who has spent more than three decades working on viruses, has long harboured the same theory.

"There has not been a single case of infection involving husband and wife," Kida said told Reuters in an interview. Kida is with the department of diseases control at Hokkaido University in Japan.

Kida explained that people infected with H5N1 have a carbohydrate receptor on cells lining their throats. The receptor -- called alpha 2,3 -- is predominantly found in birds and avian influenza viruses like to bind to this class of receptors to replicate and cause disease.

Human influenza viruses, however, prefer to bind to another receptor called alpha 2,6, which is dominant in humans.

"I think people who are infected with avian strains are special. They must have alpha 2,3 receptors," Kida said.

Although humans have some amount of alpha 2,3, Kida said alpha 2,6 was by far more "dominant" in most people.

DANGEROUS

Kida is now trying to look for H5N1 survivors in Vietnam and Thailand to verify his theory, and if it proves to be true, it could mean that most people simply cannot catch H5N1 easily -- unless the virus mutates.

"If it changes receptor specificity, then it must be dangerous," Kida said.

Many experts see H5N1 as possibly triggering an influenza pandemic that is long overdue. But that could only happen if it mutates sufficiently to become easily passed among people -- something that has not happened.

Most of its victims contracted the virus directly from sick birds. And there have not been any proven cases of human-to-human transmission.

Although very little is known about the virus, much work has been done to find out how it is transmitted and even why so few people have been infected and why it hasn't yet become infectious among people.

A group of researchers recently postulated the reason why it has not been as infectious as feared is because the virus lodges itself deep in the lungs, and not in the upper respiratory tract where it could more easily dislodge itself, get out of the body and spread.

I relayed this article to my genetics-loving 20 YO daughter.

She thinks the virus may pass only between blood relatives because the virus is doing what virus do - rapidly adapting to a new host. So after one person is infected, it mutates to adapt to a particular set of parameters that would be carried mostly by close blood relatives.

This might account for the transmission among cousins in Turkey.

If she is right, transmisson might be slowed or stopped by having caregivers NOT be related.

It's current mutation speed may be what is interfering with efficient transmission beyond close relatives. :D

Abstract (provisional)

Background
Cytokines play important roles in antiviral action. We examined whether polymorphisms of IFN-gamma, TNF-alpha and IL-10 affect the susceptibility to and outcome of severe acute respiratory syndrome (SARS).


Methods
A case-control study was carried out in 476 Chinese SARS patients and 449 healthy controls. We tested the polymorphisms of IFN-gamma, TNF-alpha and IL-10 for their associations with SARS.


Results
IFN-gamma +874A allele was associated with susceptibility to SARS in a dose-dependent manner (P<0.001). Individuals with IFN-gamma +874 AA and AT genotype had a 5.19-fold (95% Confidence Interval [CI], 2.78-9.68) and 2.57-fold (95% CI, 1.35-4.88) increased risk of developing SARS respectively. The polymorphisms of IL-10 and TNF-alpha were not associated with SARS susceptibility.


Conclusions
IFN-gamma +874A allele was shown to be a risk factor in SARS susceptibility.

Human Genetics May Play a Role in Avian Flu Cases (Update1)

May 11 (Bloomberg) -- Some people are more susceptible to bird flu than others, suggesting human genetics may play a role in the viral infection that's killed more than 100 people in the past three years, virologist Robert Webster said.

"There are a few humans that are extraordinarily sensitive,'' said Webster, 74, the Rosemary Thomas professor at St. Jude Children's Research Hospital in Memphis, Tennessee. "This seems to be in families on the maternal side,'' he said in an interview in Singapore last week.

Scientists are trying to increase their understanding of the disease in humans and of the lethal H5N1 strain of avian influenza, which has the potential to evolve into a pandemic form that may kill millions of people.

The H5N1 virus has killed at least 115 of 207 people known to have been infected since late 2003, the World Health Organization said on May 8. Some of the human cases involved multiple family members.

The human genome contains an estimated 30,000 genes. One hundred or more genes may be involved in the human response to influenza infection, Webster said.

"Which one matters for H5N1 more than another?'' Webster said. "We don't know enough about cell biology, let alone enough about virology to put these two together.''

The number of humans infected by H5N1 has increased this year after 34 countries across three continents reported initial outbreaks in birds since February. In Asia, almost 200 million domestic fowl have died or been culled to contain the spread of H5N1, costing countries more than $10 billion, the World Bank said in January.

Family Members

This year, 39 fatalities have been reported, almost as many as the 41 deaths recorded in the whole of 2005. Cases in which multiple family members were infected occurred across Asia and Eastern Europe, including in Azerbaijan, Vietnam and Indonesia.

"The question of genetic susceptibility to H5N1 has certainly been raised in the past,'' said Maria Cheng, a WHO spokeswoman in Geneva. "It may have some credence, given the millions of people that have likely been exposed to sick birds and the relatively small number of confirmed cases.''

Current data provides too little information to draw any firm conclusions, Cheng said.

"Hopefully, as we get more information once more sophisticated clinical networks are set up in countries with human bird flu cases, we will have information that helps us distinguish between exposure issues and host susceptibility factors,'' she said. "Until then, this remains only a theory.''

More Lethal

Variants of the H5N1 virus have become progressively more lethal over the past eight years in experimentally infected chickens and mice, the WHO said in February.

The virus is also becoming more capable of causing disease in mammals, the Centers for Disease Control and Prevention in Atlanta said in March.

"These are all worrying things,'' said Webster, who has been studying influenza for more than 40 years. "This highly pathogenic virus has been about for 10 years now and it keeps magnifying. Some of my colleagues are saying if it was going to go human-to-human, it would have happened now. You don't know.''

What this means is that we should not have mothers. That will reduce the risk. This is a strong argument for male supremacy and quick dispatching of all mothers. I guess this is timely. This weekend is Mother's Day. Yours, MCP.

Yes, it must be the genetics. Webster is off and remains so off I actually think he should retire and stop talking.

The uncle is sick. He's not the son of the mother. He's the husband of the daughter. His genetic line must suck eggs just like his wife's must.

Webster has stopped being an investigator and is now more like an internationally smuggled parrot. This is horrific. The guy who sits in the king's chair, for a long ago idea well thought, continues to be incapable of rational thinking. It's frightening.

If I were insulting, I'd say much worse. This is merely a fact reconfirmation.

BTW, genetics does count, but imo inconsequentially. This theory is grasping at straws, imo. But of course Webster and WHO really know. Right?

(I have "fixed" this so it's clearly my opinion. It's not mammal, right? It's not human, right? It's in the genetics and that's why the husbands don't get sick, right? And when they do, well they too had bad genes.
This is farsical. There's no reason not to look, but 2 + 2 = 4; it doesn't mean matrilineal or whatever the tight backwardslooking scientists hope will resolve the issue. I'm gonna eat a lot of infected crow if I'm wrong and I'll receive zero credit when I'm right. It's ok. I know truth and to me, the line of reasoning is analogous to smuggled birds and dirty human poultry inspector boots. It's just bullWHO.)

I wonder if Webster is thinking about the body of research surrounding genetics, the plague of the 1350's called Bubonic plague, host cell topography, and why offspring of some people who survived that plague are not susceptible to AIDS.

I remember I wrote about a PBS special on that kind of anthropological forensic genetics somewhere...

GR, I can't judge Webster as harshly as you do. He's a 3-D person like you with his own interests, detailed knowledge and expertise. You need to remember that scientists like to look at all aspects of the science that interests them, even if only 1-D gets reported. I'd like to know in more detail what he bases his genetics comments on... Might it help humanity survive pandemic to know that? Probably not, but I'd be might curious to know...

The fascinating thought that it's genetics means that random humans around the world have a genetic difference that this virus, in its multiple clades/strains, is identifying.

So, there's a part of this virus that's not virulent which is identifying the genetically predisposed to the disease?

And of course, it's very limited to only a few people who are predisposed.

Cumon' folks. Let's be aggressive in our review of this idea. We got reassortmented by Webster early last year. He did inestimable damage due to his continuing reticense to seize on recombination and take a hard look.
I'm not gonna get Webstered again.

If truth be said, Niman, well over a year ago, posted as part of the body of his website, the mission of Recombinomics, that he believed that vaccines can be tailored to humans so those who do have a genetic predisposition could acquire a vax that is designed just for them... a designer drug.

Clearly, if that service were available now, I'd be one of the first, if not the first, to ask for that vaccine.

However, as recombination has been so utterly ignored and thereby dismissed by Webster, he is Johnny Come Lately to the genetics party. Again, he's late, he's ignorant, and he's to me a loose cannon on deck.

The part that I think is even a greater insult to me and to each of you is that Webster in all of his and he's not alone at all great love for his own mirror would not deign to have read the Recombinomics website. There he would have seen that Niman well over a year ago had already identified the genetic aspect of virus and wants his company to enter and lead in that field. But alas, we have the same bullWebster that we had a year ago.

Either I'm a way too voracious seeker of fact to illuminate the topic of vaccine against viri, or there are those who are so full of themselves, and it ain't Webster alone...you know the list...are doing immeasureable societal harm by their refusal to look outside their own dirty underwear hamper.

It is undeniable that genetics play a part in every single bio-response that a person has.

I do not care whether Webster believes in recombination or not. He has something to offer, just as Niman does. Niman's problem is that he is advocating a new theory, and those brave persons always pay a price.

Do not doubt Webster's commitment to send a message about the threat of H5N1.

Do not shoot the messenger.
