Penanggulangan Flu Burung Selama Semester Satu Tahun 2008

Sampai saat ini, penularan Flu Burung (FB) masih berasal dari unggas ke manusia. Berdasarkan hasil pemeriksaan laboratorium Badan Litbangkes dan Lembaga Eijkman terhadap kontak kasus FB di 5 provinsi yang terinfeksi, menunjukkan belum terjadi penularan antar manusia. Demikian perkembangan pengendalian FB selama semester I Tahun 2008.


Berdasarkan laporan Posko Flu Burung Depkes dari 12 provinsi yang menginfeksi manusia, sudah 7 provinsi yang tidak ditemukan lagi kasus baru antara 10 bulan hingga 2 tahun lebih. Ketujuh provinsi yang tidak ditemukan kasus baru adalah Provinsi Lampung, infeksi tahun 2005 (2 tahun 9 bulan tidak ditemukan kasus Flu Burung/FB). Provinsi Sulsel, infeksi tahun 2006 (2 tahun tidak diemukan kasus FB). Provinsi Sumatera Selatan, infeksi Maret 2007 (15 bulan tak ada infeksi), Provinsi Jawa Timur, infeksi Maret 2007 (15 bulan tidak ditemukan kasus). Provinsi Riau, infeksi April 2007 (14 bulan tak ada infeksi). Provinsi Sumatera Utara, infeksi Mei 2007 (13 bulan tidak ditemukan kasus). Provinsi Bali, infeksi bulan Agustus 2007 (10 bulan tidak ada infeksi).

Sedangkan 5 provinsi yang masih ditemukan kasus adalah DKI Jakarta (kasus terakhir 13/5/08), Jawa Barat (kasus terakhir 14/5/08) , Banten (kasus terakhir 11/7/08), Jawa Tengah (kasus terakhir 23/4/08), dan Sumatera Barat (kasus terakhir 29/3/08).

Di Banten, kasus terakhir menimpa AS (L, 38 tahun), warga kota Tangerang pekerjaan sopir cargo, mulai sakit (onset) 4 Juli 2008, tanggal 8 Juli dengan gejala panas, batuk dan kemudian sesak nafas, Foto thoraks Pnemonia kiri, progresif. Faktor risiko ditemukan adanya lingkungan dengan unggas (ayam, bebek). Spesimen unggas telah diambil dan diperiksa di laboratorium hewan di Wates (Deptan).Hasil spesimen unggas menunggu.

AS dirawat di RS Usada Insani Tangerang. Tanggal 9 Juli dirujuk ke RSUD Tangerang akhirnya meninggal 10 Juli 2008. Hasil pemeriksaan laboratorium Badan Litbangkes dan Lembaga Eijkman Jakarta tanggal 11 Juli 2008, AS dinyatakan positif terinfeksi Flu Burung.

Untuk pemantapan kesiapsiagaan menghadapi kemungkinan pandemi influenza, pemerintah telah menyelenggarakan simulasi lapangan penanggulangan Episenter Pandemi Influenza di Kabupaten Jembrana Bali pada tgl 25 – 27 April 2008. Ini merupakan simulasi secara komprehensif pertama di dunia. Simulasi dilakukan di Puskesmas, Wilayah Desa Penanggulangan, RS Umum Daerah Kab. Jembrana dan RS rujukan Tabanan di Kabupaten Tabanan dan RS Sanglah di Denpasar. Selain itu disimulasukan pula pengawasan/screening di Bandar Udara Internasional Ngurah Rai Denpasar.

Simulasi dihadiri dan diliput pengamat dan media massa baik nasional maupun internasional. Secara umum simulasi mendapat apresiasi dari para pengamat. Saat ini protokol penanggulangan episenter pandemi influensa sedang dalam proses penyempurnaan, sesuai dengan hasil temuan pengalaman simulasi. Sementara film tutorial simulasi penanggulangan Episenter Pandemi Influenza sedang dalam proses editing.

Pemerintah terus berupaya mengendalikan kasus Flu Burung. Pemeriksaan laboratorium suspek FB selama Januari – Juni tahun 2006, 2007, 2008 berturut-turut 335, 507 dan 225 suspek (total 1.067 kasus). Pemeriksaan spesimen, semester I tahun 2006, 2007, 2008 berturut-turut 714 spesimen, 1025 spesimen dan 483 spesimen (total 2.222 spesimen).

Sementara jumlah kasus Flu Burung selama Juli – Desember tahun 2005, 2006, dan 2007 cenderung mengalami penurunan. Data kasus semester II tahun 2007 menurun 6,2% dibanding semester II tahun 2006. Jumlah kasus tahun 2006 juga menurun 20% dibanding tahun 2005. Demikian pula dengan jumlah kasus suspek Flu Burung semester II tahun 2007 mengalami penurunan sebesar 30,6% dari tahun 2006.

Upaya yang dilakukan adalah pemeriksaan laboratorium adalah penyelidikan kasus positif FB, pengamatan kontak kasus positif, dan penyuluhan kesehatan. Selain itu pasien yang teridentifikasi Flu Burung di RS sudah diberlakukan secara khusus sejak yang bersangkutan masih diduga (suspek) menderita Flu Burung. Pasien itu dirawat di ruang isolasi, diberikan obat anti virus oseltamivir, dilakukan tata laksana oleh tim yang dipimpin oleh dokter spesialis dan semua biaya perawatan ditanggung oleh Depkes. Sementara bagi kasus positif Flu Burung yang meninggal biaya pemulasaraan jenazah juga ditanggung oleh Depkes.

Selain itu juga telah dilakukan berbagai macam pelatihan seperti pelatihan teknis Jurubicara, Epidemiologi /Tim Gerak Cepat dan lainnya. Departemen Kesehatan bekerjasama dengan Universitas Indonesia Jakarta dan Universitas Gajah Mada Yogyakarta telah menyusun Kurikulum Ahli Epidemiologi Lapangan (Field Epidemilogy Training Program/FETP) Indonesia. Serta penyelesaian laboratorium BSL-3 oleh Badan Litbangkes Depkes.

Seasonal likely won't peak for another four or five months in the US and Europe but we should expect to start seeing cases in the northern hemisphere soon. A pandemic strain could happen at any time. The 1918 flu's second wave started in late August, so the timing of the start of a pandemic is not so predictable. At least one things seems certain, however. We shouldn't expect to see it starting in the current hot spot for human bird flu, Indonesia. Because the Indonesian government, in the person of Health Minister Siti Fadilah Supari, is batching their bird flu death reports to 6 month intervals. This is a violation of the International Health Regulations but Indonesia doesn't seem to care. They just decide on their own which international agreements are in their national interest and which ones aren't. Who do they think they are? The United States?

We definitely ALL need to be working on this. It's our community [the world one] and we need to close ranks.
I am still looking for financing to produce more films on this subject so if anyone is interested in me spreading the word by filming things on the ground here I am way open to suggestions.
