'Mysterious' bird flu baffles Indonesian scientists

Indonesian scientists and officials said they were baffled by the "mysterious" behaviour of the bird flu virus here, which has already claimed nine lives this year in the world's worst-hit nation.

Indonesia has reported 126 cases of H5N1 bird flu, 103 of them fatal, since 2005. This year's victims have all come from the capital Jakarta and its satellite cities.

Officials from the ministry of agriculture's bird flu control unit told a media briefing that the risk factors for human infection remained unclear after studies were conducted around victims' homes.

"In some of the cases we found the virus in the water and chickens, but in many other cases the studies showed no signs of the virus in the surroundings," said the unit's Tjahjani Widjastuti at the briefing late Tuesday.

The usual mode of transmission of the deadly H5N1 strain of bird flu is directly from an infected bird -- typically poultry -- to humans.

"The behaviour (of the virus in Indonesia) is mysterious and we are competing with the dynamics of the virus. There needs to be deeper study on why there are more cases in humans, what are the risk factors... so we can cut the chain of infection to humans," Widjastuti said.

Globally, scientists fear that the virus will eventually mutate into a form easily spread between people.

Indonesia has been sharply criticised for being slow to act in its fight to control bird flu, which has spread easily in a nation where many people keep chickens and other birds in their gardens and homes.

Bird flu is endemic in all of Indonesia's 33 provinces except for Gorontalo on Sulawesi island and in North Maluku, said Widjastuti.

Meanwhile, early partial results of a study conducted by the UN's Food and Agriculture Organisation bird flu programme in Indonesia in traditional markets in western Java -- which covers Jakarta and its surrounds -- showed a high level of penetration by the virus.

About half of all markets tested were positive for the virus, said James McGrane, leader of the FAO programme.

"It's a very difficult virus to control," he said, adding that the team could not say why the number of cases had spiked here.

Minister asks for end to city poultry farming

Health Minister Siti Fadilah Supari said Tuesday she expected poultry farming in Jakarta to stop in order to halt the spread of bird flu.

"If possible, both collective and backyard poultry farming must be restricted to curb the spread of the bird flu virus," she said at City Hall after a meeting.

"(It will enable us to) prevent the number of patients contracting bird flu from increasing," she said.

In a meeting presided over by President Susilo Bambang Yudhoyono, senior Cabinet members and government officials discussed efforts to curb bird flu in the three hot-spot provinces of Jakarta, Banten and West Java; flood mitigation in the capital and supply stability for commodities like cooking oil, soy bean and flour.

The meeting was attended by Jakarta Governor Fauzi Bowo and a number of city and state officials, including Transportation Minister Jusman Syafii Djamal, Trade Minister Mari Elka Pangestu and Public Works Minister Djoko Kirmanto.

Also present at the meeting were Meteorological and Geophysics Agency head Sri Woro B. Harijono, National Police chief Gen. Sutanto, Indonesian Military chief Gen. Djoko Santoso and the Health Ministry's Drug and Food Monitoring Agency head Husniah Rubiana Thamrin Akib.

In response to Siti's statement, Fauzi said the move was unnecessary, saying zoning regulations were more than enough to make sure poultry farming was under control and that the spread of the virus could be limited.

"The President hopes there will be no more deaths caused by bird flu in Jakarta, Banten and West Java this year," he said.

The three provinces have been centers for the H5N1 virus, with a combined 71 fatalities having been recorded out of 103 reported bird flu deaths nationwide.

Jakarta has been the hardest hit, with 25 deaths from 29 reported cases.

"Therefore, Jakarta will set aside special zones for the poultry industry as soon as possible," Fauzi said. "The zones must be far from residences."

He said such special zones would enable the administration to control the poultry industry.

"It will be easier for us to disinfect poultry as well," he said.

Earlier, the government urged the three provinces' administrations to strengthen their enforcement of bird flu regulations following a string of avian influenza deaths.

The most recent deaths were reported in Bekasi, Tangerang and Jakarta.

The call was made during a meeting at the Coordinating Ministry for the People's Welfare. The meeting was presided over by the coordinating minister, Aburizal Bakrie, and attended by Home Minister Mardiyanto and Siti.

The meeting discussed special zones and traditional markets' obligation not to sell live chickens.


Apart from the zoning regulations, Fauzi previously said he would strengthen restrictions on poultry in the capital, citing weak enforcement of a 2007 ordinance banning backyard poultry and regulating the poultry industry.

He said his administration would find a way to punish those violating the ordinance.

Offenders of the bylaw are currently only required to register their pet birds or have their backyard poultry culled.

These backyard chicken are a common feature of rural life in Indonesia, but have hampered efforts to curb bird flu. Badung - Oseltamivir atau tamiflu merupakan obat flu burung yang selama ini digunakan baik di Indonesia maupun negara-negara lain. Namun tamiflu ini sudah resisten di Vietnam dan Hongkong. Di Indonesia, virus flu burung terancam resisten terhadap obat ini.

"Di Indonesia kita identifikasi adanya perubahan. Dari sudut molekular kalau kita teliti memang ada sedikit perubahan dari aslinya. Hingga itu mengarah kepada kemungkinan adanya resisten," kata Deputi Bidang Pengembangan Sistem Iptek Nasional Kementerian Negara Riset dan Teknologi Amin Soebandrio.

Hal itu disampaikan dia dalam pertemuan tahunan soal flu burung ke-enam di Hotel Wastin Nusa Dua, Badung, Bali, Kamis (27/3/2008).

Menurut Amin, resisten terjadi karena penggunaan tamiflu yang banyak sekali dan dosisnya yang kurang. Karenanya, tidak bisa membunuh virus malah menjadikan virusnya terlatih sehingga tambah kuat.

"Untuk itu harus kita cegah. Penggunaan tamiflu harus kita kontrol," ujarnya.

Amin mengatakan, ditemukan juga beberapa perubahan genetik ke mutasi. Dari uji coba obatnya belum ada bukti resisten. Tetapi dilihat dari rangkaian genetiknya virus H5N1 sudah mengarah pada kemungkinan terjadinya resistennya. ( ziz / nvt )

Tamiflu Masih Efektif bagi Penderita Flu Burung di RI


Denpasar (ANTARA News) - Penggunaan tamiflu untuk pengobatan flu burung di Indonesia masih efektif, walau di identifikasi telah terjadi resistensi, kebenarannya belum dapat dibuktikan, kata Deputi Bidang Pengembangan Sistem Iptek Nasional, Kementerian Negara Riset dan Teknologi, Prof DR Amin Soebandrio.

Di Nusa Dua, Bali, Kamis, ia mengatakan, hingga saat ini masih dianggap efektif sebagai obat penyakit flu burung yang disebabkan virus Avian Influensa (AI).

"Kita belum sampai pada tingkat kekhawatiran untuk penggunaan tamiflu tersebut," katanya, di sela-sela pertemuan tahunan ke-6 flu burung yang diikuti 25 negara, diantaranya Jepang, Indonesia, China, Vietnam dan Afrika Selatan yang berlangsung hingga 28 Maret 2008.

Dia mengatakan, obat tersebut bisa saja terjadi risistensi, jika dalam penggunaan terhadap penderita tidak sesuai petunjuk atau karena kelebihan dosis.

"Oleh karena itu dalam pemberian obat tamiflu tidak diberikan secara sembarangan bagi setiap gejala flu. Sebelum memberikan dosis tersebut dilakukan cek laboratorium untuk mengetahui si pasien, apakah si pasien positif menderita flu burung atau hanya flu biasa," ucapnya.

Menurut Amin, pemberian dosis tamiflu sangat ketat dan obat itu hanya ada di Puskesmas dan rumah sakit. Obat itu pun tidak dijual sembarangan termasuk di berbagai apotek.

"Langkah yang dilakukan pemerintah melalui Departemen Kesehatan untuk tidak menjual obat tersebut dengan tujuan menghindari agar masyarakat tidak cepat-cepat meminum tamiflu, pada hal sakitnya belum tentu mengarah kesana," kata Amin menambahkan. (*)

Viruses recovered from domestic birds in Cambodia, a clade 1 country, became 6 to 7 times less sensitive to oseltamivir between 2004 and 2005, as measured by lab tests. And 2005 isolates from Indonesia, the country where the clade 2 grouping arose before spreading west as far as Europe, were 25 to 30 times less sensitive to oseltamivir than any of the clade 1 viruses collected in 2004.
