Whooping cough is a highly contagious bacterial infection whose transmission occurs mainly within the family or in communities through contact with a sick person with a cough. Even though the number of cases of whooping cough has decreased significantly since the introduction of the vaccine, the bacteria continues to circulate. Infants too young to be vaccinated and adolescents and adults who have lost vaccine protection are the most affected populations. Outside our borders, Europe is currently experiencing an upsurge in cases of whooping cough with significant epidemics in Croatia, Denmark and the United Kingdom and significant increases in Belgium, Spain and Germany. In France, since the start of 2024, around twenty grouped cases (or clusters) have been reported to Public Health France in 8 French regions versus 2 grouped cases in a single region (Ile-de-France) for throughout 2023. Given this clear increase in the number of cluster cases reported, Public Health France remains vigilant and recalls the importance of vaccination to protect people at risk of serious forms.

A resumption of the circulation of whooping cough begins in France

Whooping cough progresses in cycles of recrudescence every 3 to 5 years. In France, surveillance data from the RENACOQ network (the hospital pertussis surveillance network) have shown six epidemic peaks in recent years: 1997, 2000, 2005, 2009, 2012-2013 and 2017-2018. Since the last peak of whooping cough (162 cases reported), the number of cases has continued to decrease to reach 34 cases in 2022 and 4 cases in 2021 in infants under 12 months. Same observation regarding the data from the Sentinelles network (the research and monitoring network in primary care general medicine and pediatrics in mainland France) reporting 1 case per year in the general population over the same period. Even if a rebound in the disease could be expected in France in 2021-2022, the exceptional context and the health measures implemented as part of the COVID-19 epidemic have probably reduced the transmission of whooping cough.

The data published today, from the latest surveillance report, indicate a greater resumption of the circulation of whooping cough in recent months in mainland France. Thus, the RENACOQ network (monitoring hospitalized infants under 12 months since 2016) reported in 2022, 45 cases and in 2023, 39 cases (unconsolidated data). Regarding grouped cases in 2023, only 2 grouped cases were reported to Public Health France between October and December with 18 cases of whooping cough in Ile-de-France: they concerned intrafamilial grouped cases (13 cases), clusters in the community (4 cases) and a final isolated case with no link in the surroundings of the reported clusters. In the first quarter of 2024, around fifteen clusters, mainly in communities (nursery schools, primary schools, daycare centers and nursery homes) but also in families and totaling 70 cases were reported to Public Health France (data not consolidated for the current year 2024 ).

The multiplication in the number of cases compared to 2023 and the sharp increase in grouped cases indicate a resumption of community circulation of the bacteria which could intensify in the coming months. Vigilance remains essential, with the need to increase public awareness of this disease and its prevention methods. To date, the French situation is not comparable with that of our European neighbors and across the Atlantic who have reported several hundred cases per week since the last quarter of 2023.

The importance of reporting and the measures to be taken with regard to the patient and those around them

Whooping cough is not a notifiable disease but cases must be reported to the Regional Health Agency in two specific situations:
  • as part of the reporting of nosocomial infections or

What is whooping cough?

Whooping cough is a respiratory infection caused by a bacteria, Bordetella pertussis. Why this outbreak?

      • Regarding the monitoring of strain resistance to macrolides, 3 resistant strains have been characterized at the CNR since the beginning of 2024 (3/188 isolates tested, i.e. 1.6%) and since June, no additional resistance has been detected.
  • The magnitude of the peak and the duration of this epidemic cycle are not predictable and if some indicators appear to be decreasing, interpretation must be cautious because the data for July are not yet consolidated.
  • Given the very high contagiousness of the disease and the epidemiological situation in France and internationally, the need for increased vigilance during the Olympic Games remains essential, particularly in situations of grouped cases involving people at risk of serious forms.
  • Public Health France recalls the importance of vaccination for pregnant women, recommended since April 2022 to protect newborns and young infants.

WHOOPING COUGH: HOW TO EXPLAIN THE RESURGENCE OF THE DISEASE IN 2024 IN FRANCE?

In a work coordinated by the Pasteur Institute, scientists describe the dynamics of the 2024 whooping cough epidemic in France, the most significant in at least 25 years. The researchers also sequenced the genome of nearly 70 bacterial isolates collected during this epidemic from January to mid-2024. These analyses make it possible to put forward several hypotheses regarding the current recirculation of the disease.

Whooping cough is a particularly contagious and potentially serious or even fatal respiratory disease for the most vulnerable, such as newborns who have not yet been vaccinated. It is sometimes wrongly considered a disease of the past: however, each year, it is the cause of 40 million cases and 300,000 deaths worldwide, according to a WHO estimate. While it is true that in France, vaccination has largely reduced the number of cases, whooping cough nevertheless continues to circulate cyclically, with epidemic peaks observed every three to five years between 1997 and 2019.

Whooping cough epidemic unprecedented for at least 25 years

Since the “Covid period”, the regularity of cycles has been disrupted. While we could have expected an epidemic peak in 2022 or 2023, it has been delayed. In France, as in other European countries, the period coinciding with the Covid-19 pandemic was accompanied by a very sharp reduction in the circulation of Bordetella pertussis, the bacteria causing the majority of cases of whooping cough.

In France, since the beginning of 2024, the situation has been radically different. Between January 1 and May 31, 2024, 5,616 people infected with Bordetella pertussis were recorded. According to Public Health France (SpF), between January 1 and July 24, 2024, 28 people, mainly young children, died while carrying whooping cough.

In addition, the number of people infected and deceased since the beginning of 2024 is much higher than the numbers recorded during previous epidemic peaks: whooping cough is making a comeback in an unprecedented way.

Whooping cough bacteria different from “pre-Covid” period

How then can we explain such a strong resurgence of the disease? In a recently published study coordinated by scientists from the Pasteur Institute, conducted in collaboration with SpF and city diagnostic laboratories, several hypotheses are put forward. First of all, the lockdowns and barrier gestures linked to the “Covid period” have limited exposure to several pathogenic viruses and bacteria, including those causing whooping cough. As immunity gradually declines over time, all these measures, combined with the low amplitude of the last peaks observed, may have gradually reduced the overall immunity of the population against the disease, which is usually maintained by often asymptomatic infections acting as natural boosters in our vaccinated population. When whooping cough returned, it encountered a population that was more “naïve” than in the past, from an immunological point of view with regard to the disease.

But this may not be the only explanation. To gain more insight, scientists sequenced the genome of several bacterial isolates of Bordetella pertussis, taken from people infected in 2024. "Out of 67 cases of B. pertussis cultured and whose entire genome was sequenced, we looked for the production of vaccine antigens: we showed that 66 isolates of B. pertussis produce pertactin and 56 the FIM2 adhesin, unlike in the pre-Covid years when these two antigens were much less commonly observed." explains Sylvain Brisse, head of the Biodiversity and Epidemiology of Pathogenic Bacteria Unit and the National Reference Center for Whooping Cough and Other Bordetelloses . These two proteins produced in the vast majority of isolates analyzed are virulence factors targeted by vaccination: they play an essential role in the adhesion of the bacteria to the cells of the respiratory epithelium and in the modulation of the host response. Their current predominance, contrary to what was observed before the “Covid period”, could also explain the current very high circulation of whooping cough.

Last important result, the scientists also identified, for the first time in France since 2011, a bacterial isolate resistant to macrolides, the first-line antibiotics used against whooping cough . While this type of isolate is still very rare in France and more widely in Europe, its presence is more regularly observed in Asia and particularly in China where it is very widespread. An additional discovery that further underlines the importance of monitoring bacterial strains causing whooping cough.

In 2024, France and Europe experienced a significant resurgence of whooping cough , with a marked increase in the number of cases across their territories. Whooping cough evolves in epidemic cycles every 3 to 5 years, the last of which was in 2017-2018, and the circulation of the bacteria had been low after the COVID-19 pandemic. Although the arrival of a new cycle was expected, the scale and intensity of this new epidemic surprised by their speed and extent. Surveillance indicators saw a constant increase in the first quarter of 2024, reaching a peak in July-August.

Despite a notable decline in cases in the last quarter of 2024, indicating the end of the epidemic cycle for 2024, the levels of circulation of the bacteria remained high compared to previous years. In response, health authorities, including Public Health France, recommended increased vigilance and reiterated the importance of preventive measures and updating vaccinations to reduce the risk of severe disease in newborns and infants and to protect at-risk adults.


Whooping cough epidemic situation in France in 2024

In 2024, in community medicine, the Sentinelles network reported an estimated number of cases seen in general practice consultations of 162,612. After peaking in July, weekly incidences began to decline. The total number of SOS doctors' procedures for whooping cough was 9,817. These procedures increased steadily until June, then remained stable but high throughout the summer, before decreasing for all age groups and in all regions.
In hospitals, the total number of emergency department visits for whooping cough in 2024 was 7,012. After 33 consecutive weeks of increases since the beginning of the year, the weekly number of visits began to decline from mid-August. The total number of hospitalizations after emergency department visits for whooping cough in 2024 was 1,471.

The weekly number of hospitalizations has increased since the beginning of the year, then began to decrease sharply from June. By the end of the year, this number was back at levels comparable to those of the last epidemic in 2017-2018.

The RENACOQ network, which monitors pediatric forms of whooping cough seen in hospitals, reported 500 infants under 12 months of age for 2024 (provisional data), of which 74% (376 infants) were under 6 months old. This total number of cases in 2024 is higher than those reported during the last peaks in 2012 and 2017.

In mortality data, 46 deaths were reported in 2024, including 24 children (21 were under 1 year old) and 22 adults (including 15 aged 80 and over).

Regarding antibiotic resistance: The CNR identified 17 patients infected with a macrolide-resistant strain of B. pertussis (MRBP) in 2024, representing a prevalence of 1.9%.
Until the beginning of 2024, only one case of macrolide resistance had been reported in France, in 2011. These new data show that macrolide resistance appears to be increasing in France.

Internationally, only the WHO-Europe region appears to have experienced an epidemic outbreak in 2024 compared to other WHO regions, with an estimated incidence rate of 386 cases/1 million inhabitants. Neighboring European countries were also experiencing the end of the pertussis epidemic cycle at the end of 2024, with no resumption of circulation in 2025.

Since the beginning of 2025, the indicators monitored (general medicine, hospital and data from medical biology laboratories) over the first 6 months of 2025 do not seem to show a resumption of circulation of the bacteria over the spring-summer 2025 period (provisional data) even if a seasonal peak of very moderate magnitude could be observed from the data from the laboratory network.

Vaccination is essential against whooping cough

Hospitalizations and deaths related to whooping cough occur primarily in infants under 6 months of age, who are the population most at risk of severe disease. In the context of the major whooping cough epidemic that raged in 2024 and to protect the youngest infants, vaccination against whooping cough for new mothers remains essential and the best possible protection.

Among women who gave birth in 2024, vaccination coverage (at least 1 dose) was estimated at 62.3% compared to 43.4% for those who gave birth in 2023, representing a coverage gain of 18.9 points.
Thus, the vaccination policy aims to reduce the risks of severe forms and is based on three complementary strategies:

  • compulsory vaccination with a primary vaccination course of two injections two months apart, i.e. at the age of 2 months (8 weeks) and 4 months, followed by a booster at the age of 11 months, and the administration of repeated boosters at 6 years, 11-13 years and up to adulthood (25 years with the possibility of catching up up to 39 years);
  • vaccination of pregnant women, recommended from the second trimester of pregnancy, favoring the period between 20 and 36 weeks of amenorrhea;
  • in the absence of vaccination of the mother during pregnancy, vaccination of the mother postpartum and of people likely to be in close contact with the infant during the first 6 months of life (so-called cocooning strategy).
Beyond infants not protected by vaccination, people at risk of severe forms of whooping cough include people suffering from chronic respiratory disease (asthma, chronic obstructive pulmonary disease, etc.), immunocompromised people, obese people, and people over 80 years of age. It is therefore recalled that vaccination is recommended for:
  • immunocompromised people,
  • healthcare professionals (including in residential establishments for dependent elderly people (EHPAD),
  • people working in close and repeated contact with infants under 6 months of age,
  • students in medical and paramedical fields,
  • early childhood professionals including childminders and people who regularly babysit.
Furthermore, for the entire population: a whooping cough booster is also recommended at the ages of 6, 11-13 years, then 25, 45, 65 years, then every 10 years from the age of 65 in association with diphtheria, tetanus and polio boosters.

Wearing a mask, an effective barrier
