Thanks, I think his clinical course as told to the press by his parents is entirely consistent with a very rapid novel H1N1 death. IMO, he deserves our attention. There is a picture of him too Appleblossm in case you wish to add it to the others.

Since we are sifting the tea leaves for old possibles, and I am not proposing that these two cases be added to the unconfirmed, as I recall, there were reports of two young adult males from Southern California who died in hospital of respiratory failure in April 2009. These two were mentioned in the news as suspicious deaths that might have been due to influenza. I have not heard anything else about these two deaths which preceded #1 TX, the Mexican infant by several weeks.

As I recall, these two deaths were unexpected and were reported in the media, since this is the only way I would have found out about it. If they were due to an ILI, they may well be #1 and 2 CA respectively.

The official statement attempts to invalidate the case based on the fact that no tests were preformed, it also states...

"Minnick said the medical investigator has not, at this time, determined that swine flu may have caused the boy?s death. If it does, it will contact the health department, which will send samples to the CDC for testing."

They were also supposed to have results on Friday and instead released the above statements the same day the case hit the News Paper on Wednesday. Notice they never sent the samples to the CDC for testing.

The circumstances around the death do sound non H1N1 related, but then again the whole story contains iffy details.

I still think this is a good suspect case until we can find official coroner results on what did cause the death.

Well this is an interesting question. What is needed is a plot of cases on a map to see if they are concentrated in urban areas in an unexpected way. Unexpected means that the number of deaths seen in urban areas are out of proportion given their numbers in the US population as a whole.

I think that many of the AZ deaths have taken place in rural areas but in NY, most of the deaths have been in NYC.

Deaths due to a severe flu pandemic will be concentrated in urban areas due the influence of crowding on the transmission rate. However, although we are in a pandemic, as bad as it has been, it is far from severe. At the most, it can be described at present as moderate at the worst.

What the future holds for this pandemic is unknown although I think it will become much worse than at present.

The issue here though relates to the possibility that the deaths reported so far by officials and the media are of those who live within urban areas and if so, is this an anomaly.

This is an important question because if this proves to be the case, one needs to explore the reason for this finding.

The null hypothesis is that US deaths should be evenly distributed across the nation. If there are clusters of deaths and these clusters are concentrated within urban areas and the numbers of deaths reported within these areas of the country exceed their percentage of the population, then this is an example of an anomaly that needs to be investigated and explained.

Could it be due in large part to a testing/reporting bias? The rural areas are less likely to have the ability to do the initial testing (nasal swab to test for flu a or b) and therefore cases may be misdiagnosed, or only secondary complications noted (for instance, how likely is it someone will have strep throat at the same time as influenza?) Also, rural peoples are probably less likely to go to the doctor for the flu or a cold, instead opting to tough it out.

THE FACES OF THE VICTIMS OF H1N1
I have been following H1N1 from the beginning and have been keeping track of the deaths that were occurring. The higher the numbers mounted and the more impersonal it got, the more I felt the need to put faces to the statistics. I found Flutrackers when I typed in some of the names while hunting for photos to make a visual record. I also found the slide show by appleblossom (wonderful work, thank you). Attached is one page. Of the approximately 116 (as of 22 Jun 09) Americans who have passed away from H1N1, only a few had photographs appear in the media. I think that is unfortunate as it allows us to rationalize away the severity and to depersonalize the death. I am convinced that economic interests have lobbied to keep this low key. That low-key attitude has allowed it to spread around the world where people have started to die (Phillipines 1, Columbia 2, Chile 7, Honduras 1, Argentina 7, Australia 1, Scotland 1, Canada 16, Mexico 113, Costa Rica 1, Dominican Republic 1). And I am afraid that is just the beginning.

SARS killed fast and we took it very seriously and defeated it. H1N1 kills slowly, spreads widely, and has lulled us into complacency. There are those of us who have read extensively about viruses, plagues, and pandemics and we are nervous. Please look at the photos attached. If you develop any of the symptoms of flu this summer, stay home, quarantine yourself and avoid passing it on to others. For heavens sake, please do not travel. Not staying home when you have a mild case of H1N1 could cause the death of someone like one of the people shown? a baby, a child, a pregnant mother, a young woman, a police officer, a school principal, a disabled child, an older person, really, just about anyone. The other thing to be learned from some of the people who have lost loved ones is that if you are in one of the risk groups and you develop symptoms of H1N1 or if your symptoms worsen at home, go to your doctor or emergency room immediately.

I agree that crowding has an effect. Look at the sad story in Manitoba (a rural area) for a good example of the presumed effect of crowding. But the higher numbers of deaths in cities are not all due to crowding.

It would be very surprising to see novel H1N1 deaths to be evenly distributed across the US. The deaths WOULD be expected to be concentrated in population centers. After all, the US population is not distributed evenly across the US, people are concentrated in cities.

So, the first point is that number of deaths should be evaluated as a percentage of the underlying population. If you evaluate the numerator (deaths), without the denominator (city or county population), you miss the big picture.

The second point is that it is early in the outbreak. Not all areas in the US have had the opportunity or optimum conditions for community spread that say, NYC or Salt Lake City has. Therefore, I would not expect all communities to have an equivalent percentage of H1N1 deaths at this point.

I agree with pflu that the media report isn?t crystal clear, and that there has been no official, definitive report publicized. So, from that perspective, you could continue to view it as a suspicious case.
But, I guess I have a difficult time believing that, after almost 2 months since his death, the NM health department, the cdc, the child?s parents, the community, etc would be so silent if there were any evidence otherwise. Gaining access to the post-mortem exam results is unlikely, so one could be suspicious of this case indefinitely.
I am suspect of how many public health departments and officials are handling this outbreak, but I think most of the definitive cases will be discovered and disclosed during the outbreak. I think the less-obvious/less-definitive cases will be discovered much later (months? a year?) by those who have access to the specific medical data we lack access to.

I would keep him as a definite case. I saw a lot of quotes from the county health department official that was pretty emphatic that his Legionella tests were all negative, and that his H1N1 tests were positive. And, it would seem (seen from a distance) that his hospital course and treatment is consistent with what has occurred in the demise of other swine flu victims. Plus, the other 20 or so officers in his precinct all followed a flu-like course, unlike what you would expect in a Legionella outbreak, I think.

I would, of course, defer to anyone else having strong feelings for listing these individuals as they see fit.
Anyway, those are my 2 cents.

Definite upward trend. We are currently just under 43% of deaths in healthy individuals.

There are some problems with this. I used as the source for my data a different spreadsheet mentioned in this thread. It seems rather complete. The issue is that firm death dates are not given. When no death date was given I tried to use the lag time from other reports from that state to determine where to set a death date. If I couldn't make a determination, I made an assumption of 3-4 days lag. So other people might come up with slightly different graphs, but the trend and final number should be a good match.

To determine if a person was healthy I looked at the remarks. A null comment is assumed to be healthy. If an underlying condition is marked disputed I assumed a healthy individual. Given the risks of pregnancy in this pandemic I assumed pregnancy to be a valid underlying condition.
