Every Year 30,000 Die by Measles

In Indonesia, estimated 30.000 children die every year because of measles complication. It means a child dies in every 20 minutes. Measles is potential for outbreak, and even in malnutrition sufferer it will easily kill the sufferer. Therefore, it is a main cause of children death.

It was mentioned by minister for health, Dr. Siti Fadilah Supari, Sp. JP (K), in press conference entering measles immunization, Sub PIN Polio and Vitamin A provision at MOHRI office, Jakarta on February 19, 2007. It was also attended by Director General of Disease Control and Environmental Health MOHRI, Dr. I Nyoman Kandun, MPH, WHO representative for Indonesia , George Petersen, chairman of PP KIPI national committe, Prof. Dr. Sri Rezeki and representatives of UNICEF and American Red Cross.

Previously, Mrs. SUpari accepted aid of 600,000 dosages of TT (Tetanus Toxoid) vaccine costs Rp.235 million. It is for immunizing volunteers which evacuating flood victims in Jadetabek ( Jakarta, Depok, Tangerang, Bekasi).

She believes that every year in the world estimated 30 million people infected by measles. In 2002, 777,000 were reported dead, with 202,000 of them occurred in ASEAN countries and 15% came from Indonesia . In 2005, 345,000 deaths are estimated, and mostly are children.

To Decrease morbidity and mortality rates on children caused by measles, starting on February 20, 2007, about 13,611,899 children in 5 provinces in Java (Banten, West Java, DKI Jakarta, Central Java and East Java ) will participate in programs of measles immunization, Sub PIN Polio and vitamin A provision in the same time.

The same programs had been implemented in NAD, North Sumatera, Maluku, North Maluku, West Irian Jaya, Papua, West Sumatera, Riau, Riau Islands , Jambi, South Sumatera, Bengkulu, Lampung, Bangka belitung, East Nusa Tenggara and DI Yogyakarta.

Immunization will provide immune system toward measles for a whole life. Additional measles immunization, together with regular one, has decreased measles-related death to 48%. It indicates that measles is preventable, and measles-related death can be eliminated dramatically, said Mrs. Supari.

Target of measles immunization covers 13.611.899 children, which 11,526,160 are under five (6-59 months) and 2,085,739 are elementary students first to sixth grade in Banten and East Java provinces. Meanwhile, target of polio immunization (0-59 months) is 12,803,814 children.

Logistics needed for this program covers 924,490 vial of measles vaccine, 14,292,494 sets of ADS 0.5 ml, 970,715 Syringes 10 ml and 566,455 of mOPV and 295,320 TOPV polio vaccines.

Budget for operational of measles immunization 2007 is Rp.36,820,771,000. It comes from ARC (American Red Cross) through WHO, while procurement cost comes from WHO and UNICEF, except for polio vaccine, which is from State Budget 2007.

Measles vaccine is one of safe vaccines. Although, it can give effects to some children, it is rare and not serious. The effects can be skin eruption, fever, running nose. When serious symptoms are found, parents have immediately to bring their children to health care facilities for further treatment.

Measles transmission is through direct contact with infected person, such as droplet infection from cough and sneeze. The virus attacks body immune system. Clinic symptoms are fever, running nose, cough, skin eruption, red eyes and some complication, such as pneumonia, diarrhea, ear inflammation, brain inflammation which can cause death, said Mrs. Supari.

WHA Resolution

In spite of unreachable areas, measles immunization of second dosage is appropriate to World Health Assembly 2003, said Mrs. Supari. Globally, it is agreed that every country gradually reduces and eliminates measles by providing routine immunization to infants and second chance to every child, such as under-five and elementary school age as they are susceptible toward it.

In accordance with that, Indonesia provides additional immunization in 4 phases following the previous in NAD and North Sumatera after Tsunami in 2005.

First phase was in 4 provinces: Maluku, North Maluku , West Irian Jaya and Papua, the same time with PIN V on April 2006. Total target was 850,105 children, consisted of 234,528 (6-59 months) in Maluku and North Maluku , 615,577 (6 months � 15 years) in Papua and West Irian Jaya.

Second phase was on August 29, 2006 in 9 provinces: West Sumatera, Riau, Riau Islands , Jambi, South Sumatera , Bengkulu, Lampung, Bangka Belitung and East Nusa Tenggara. Total target was 7,390,229 children, consisted of 3,862,329 aged 6-59 months and 3,527,900 aged elementary school 1-6 grade. DI Yogyakarta, which previously held measles campaign 2007, provided it on September 2006 after earthquake disaster. From the 16 provinces, the coverage averagely is 90%, only 5 provinces having coverage > 85%.

Third Phase will be on February 20, 2007 in 5 provinces in Java: Banten, West Java, DKI Jakarta, Central Java and East Java .

Fourth phase will be on August 2007 in 12 provinces which has not implemented it, such as in Kalimantan, Sulawesi, Bali and West Nusa Tenggara.
