Male vs Female Restrooms

A comparison between the relative cleanliness of male and female restrooms (Table 8) showed that male restrooms were 1.5 times "cleaner" overall (1.5 times less total coliform contamination) and that female restrooms were contaminated with E. coli to a much greater extent. In female restrooms the sink (around the drain) was three times more likely to be contaminated with E. coli than the sink drains of the male restrooms. The trends are even more obvious when the toilet, sink, and floor areas were grouped for male and female restroom (Table 9). In frequency of total coliform occurrence, women out-do men nearly 2:1 in all areas. More specifically, in female restrooms, the floor area was 1.6 times more likely to be contaminated, the sink area was 1.9 times, and the toilet area was 1.8 times. The frequency of E. coli isolation was also greater in female restrooms. The E. coli was 4.5 times more often isolated in the sink area of female restrooms. There is a similar trend for the toilet area. Toilet area sites were contaminated with E. coli 1.9 times more often in female restrooms. The floors in male restrooms, however, are more often contaminated (1.7 times more often) with E. coli than the floors in the female restrooms.

Molecular genetics in China is providing answers in the frantic effort to solve the urgent public health crisis in EU.
Rapid work in China has applied third generation DNA decoding technologies to decode the German outbreak disease bacterium genome. It has revealed the germ to be a hybrid.
The novel germ has some virulence abilities of a class of pathogenic E. coli bacteria called entero-aggregative E. coli (#EAEC). It has similarities to a bacterial strain called EAEC 55989 , which was isolated in the Central African Republic and is known to cause serious diarrhea. EAEC typical carry extra mini-chromosomes called plasmids. The German outbreak strain has the typical plasmid genes of EAEC.
The work in Shenzhen, China, is a triumph of rapid genetic investigation using high-technology methods. The German outbreak strain is a new strain which has acquired specific gene sequences that have a role in pathogenicity, causing hemorrhagic colitis and hemolytic-uremic syndrome (HUS). In response to this situation, and immediately after the reports of deaths, the University Medical Centre Hamburg-Eppendorf and BGI-Shenzhen began working together to sequence the bacterium and assess its human health risk. BGI-Shenzhen has just completed the sequence and carried out a preliminary analysis that shows the current infection is caused by an entirely new super-toxic E. coli strain.
According to the latest announcement from German health officials, the death toll in Europe from the epidemic has risen to at least 17. Over 1,000 new cases of infection have also been reported in other parts of Europe, including Sweden, Denmark, the Netherlands, the UK, and others. The University Medical Center Hamburg-Eppendorf received the majority of the infected patients from northern Germany and found that antibiotic treatment was ineffective.

The World Health Organization says the E. coli strain responsible for the deadly outbreak in Europe is a new bacteria that has never been seen before.



The agency says Thursday that preliminary genetic sequencing suggests the strain is a mutant form of two different E. coli bacterium, with lethal genes that could explain why the Europe-wide outbreak appears to be so massive and dangerous.

'Killer cucumbers': Three Britons infected with E.coli

Three British nationals in the UK have been infected with E.coli linked to the German outbreak, the Health Protection Agency (HPA) said.

It brings the total number of cases in the UK to seven. Four cases are among German nationals.

Experts believe all the patients caught the infection in Germany and brought it back to the UK.


Three of them have developed the potentially deadly complication of haemolytic uraemic syndrome (HUS).

The victims either went to A&E, NHS walk-in centres or had been to their GP.

According to the World Health Organisation (WHO), cases of HUS and enterohemorrhagic E.coli (EHEC) continue to rise in Germany.
Overall, 10 countries have now reported cases to WHO.

Nine patients in Germany have died of HUS and another six of EHEC. One person has also died in Sweden, bringing the total number of deaths to 16, according to the latest figures from WHO.

Many other patients are in hospital, with several needing intensive care, including dialysis.

Because pathogens that might otherwise seldom come together are crowded together during sewer treatment, the opportunity for genetic exchange is also greatly enhanced. The results can be a sort of “super germ” totally resistant to antibiotics just as is E. coli 0157H:7 Thus the sewage treatment environment gives potential for highly accelerated genetic exchange to this variety of pathogens and non-pathogens that can continue to exist once released to the environment.....

BGI Sequences Genome of the Deadly E. Coli in Germany and Reveals New Super-Toxic Strain


According to the results of the current draft assembly (available for download at ftp://ftp.genomics.org.cn/pub/Ecoli_TY-2482), the estimated genome size of this new E. coli strain is about 5.2 Mb. Sequence analysis indicated this bacterium is an EHEC serotype O104 E. coli strain; however, this is a new serotype ? not previously involved in any E. coli outbreaks. Comparative analysis showed that this bacterium has 93% sequence similarity with the EAEC 55989 E. coli strain, which was isolated in the Central African Republic and known to cause serious diarrhea. This new strain of E. coli, however, has also acquired specific sequences that appear to be similar to those involved in the pathogenicity of hemorrhagic colitis and hemolytic-uremic syndrome. The acquisition of these genes may have occurred through horizontal gene transfer. The analysis further showed that this deadly bacterium carries several antibiotic resistance genes, including resistance to aminoglycoside, macrolides and Beta-lactam antibiotics: all of which makes antibiotic treatment extremely difficult.

The research team will further analyze the integrity of the virulence genes, their expression profiles, drug resistance, and gene transfer mechanisms followed by validation of these genes in other strains. Patient in Norway with possible connection to EHEC outbreak in Germany


The Norwegian Institute of Public Health has been notified of a case of EHEC infection with a possible connection to the current outbreak in Germany.
illustration photo The patient is a man in his forties who was admitted to S?rlandet Hospital in Kristiansand with bloody diarrhoea. It is not yet known if the bacterial strain is the same as that which is involved in the German outbreak.

Local health authorities and the Food Safety Authority are following up the case in co-operation with the Norwegian Institute of Public Health, and are working to identify the source of infection. The number of confirmed cases in Europe is increasing so it would be unsurprising if more cases arise in Norway.

Phillip I. Tarr, MD, an HUS expert at the Washington University School of Medicine in St. Louis, told CIDRAP News today that it's not clear how many of the reported HUS cases in Europe really are HUS.

"This is the fog of an outbreak. It's not clear that we have stringent case definitions," said Tarr, who is the Melvin E. Carnahan professor of pediatrics and directs the Division of Pediatric Gastroenterology and Nutrition at the school.

He said the deaths leave no doubt that severe cases are occurring. "Below that, it becomes murky as to how many patients are really meeting the case definition of HUS. We generally have used a fairly definable cut point by lab testing, and I think in retrospect we'll be able to confirm" the cases. But for now, the situation isn't clear, he said.

"The other thing we're not clear about is the denominator: how many people are culture-confirmed, highly credibly infected versus not so credibly infected," Tarr said. He said it's not known how many patients have undergone appropriate diagnostic testing, adding, "In many outbreaks?Walkerton, for example, and Sakai in Japan?microbiologically confirmed cases were really a small minority of the putatively infected group."

Tarr also noted that in E coli cases, antibiotic treatment seems to bring an increased risk of HUS, but said he had no information on the potential role of antibiotic treatment in the European outbreak. .....

CIDRAP: E coli cases in Europe exceed 1,600

Jun 2, 2011 (CIDRAP News) ? The official toll of enterohemorrhagic Escherichia coli (EHEC) cases in Europe climbed above 1,600 today, with at least 16 deaths, as the pathogen's source remained elusive and one team of scientists called the strain new and "super-toxic."

The World Health Organization (WHO) said today that the outbreak grew to 1,614 cases in Europe, including 1,115 cases classified as EHEC only and another 499 that involved hemolytic uremic syndrome (HUS), or potentially fatal kidney failure. The proportion of HUS cases and the percentage of HUS cases in adults (88%) have both been far higher than is usually seen in E coli outbreaks.

In Germany, 9 patients have died of HUS and another 6 have died of EHEC, and one person in Sweden has died, the WHO said. But an Associated Press (AP) report today put the total death toll at 18. Most cases have occurred in Germany, but nine other European countries have also had illnesses.

Three people in the United States are now hospitalized with HUS cases suspected to be related to the European outbreak, Lola Russell of the US Centers for Disease Control and Prevention told CIDRAP News today. The count is up one since yesterday.

Today's case numbers seemed to signal that the outbreak is slowing down little, if at all. A report published today in Eurosurveillance, covering events to May 31, showed that the daily number of HUS cases reported in Germany peaked at 39 on May 16 and has been generally dropping since then. But today's WHO statement noted that the number of German HUS cases yesterday was 470, an increase of 97 from the day before.

Also, because of the delay in reporting of cases, the Eurosurveillance report said, "The current notification data cannot be interpreted as a decrease in case numbers." The report was authored by specialists from Germany's Robert Koch Institute and the European Centre for Disease Prevention and Control in Stockholm.

Despite multiple investigations, there was no official word today of new clues about the source of the pathogen, the rare E coli strain O104:H4. Case-control studies have cast suspicion on cucumbers, tomatoes, and lettuce sold in northern Germany. But the outbreak strain has not been found on any of those items, despite an early finding of a pathogenic E coli strain on some Spanish cucumbers tested in Hamburg.

Meanwhile, a Chinese genomics laboratory, BGI (formerly the Beijing Genomics Institute), announced today that it has sequenced the outbreak strain and completed "a preliminary analysis that shows the current infection is an entirely new super-toxic E coli strain." The analysis was done by BGI-Shenzen in collaboration with the University Medical Centre Hamburg-Eppendorf, the BGI statement said.

The analysis confirmed that the pathogen is an E coli O104 but said it is a new serotype, "not previously involved in any E coli outbreaks," according to BGI. The strain is 93% similar to a strain found in the Central African Republic, but it has acquired sequences that seem similar to those involved in causing "hemorrhagic colitis" and HUS, the statement said.

The statement also said the E coli strain carries genes that confer resistance to several classes of antibiotics. Earlier reports from Europe had said the strain was resistant to multiple drugs.
A WHO official agreed that the outbreak strain is new, according to the AP report. "This is a unique strain that has never been isolated from patients before," said Hilda Kruse, a WHO food safety expert.

Earlier this week, the CDC called the outbreak strain very rare but not brand new. In today's AP story, Dr. Robert Tauxe, a CDC foodborne disease expert, said the strain was seen in a case in Korea in the 1990s. He said the genetic fingerprints of the current strain and the Korea one may vary slightly, but not enough to call the European strain new, according to the AP.
Craig Hedberg, PhD, a foodborne disease expert at the University of Minnesota School of Public Health in Minneapolis, cautioned that there is still too little information, genomic or otherwise, to explain why the outbreak strain is causing so many severe illnesses.
"I think it is too early to fully characterize this agent and what makes it as virulent as it appears to be," Hedberg commented by e-mail. "The rapid genomic analysis presents some interesting information, but detecting a gene is not the same as demonstrating what it means from the standpoint of disease transmission.

"The large number of cases diagnosed with HUS implies either a highly pathogenic organism, or a very large population exposed to the vehicle. It is too soon to really make those distinctions. A relatively small case-control study seemed to implicate fresh produce items, and this is certainly consistent with the demographics of the outbreak, but the size and distribution of the outbreak is also somewhat difficult to explain with a contaminated, imported produce item.

"Something very unusual has happened (is happening?), and we cannot be certain what it is," he added.
Referring to Tauxe's comments, Hedberg agreed that an HUS case involving a similar strain occurred in a 29-year-old Korean woman. "How this appeared suddenly and overwhelmingly in northern Germany is very disturbing," he said.

He also commented that the Robert Koch Institute has already learned much about the strain and shown that it has an unusual combination of features, but said the full extent of its virulence and its pathogenic mechanisms still need to be determined.

"The fact that it is a multi-drug resistant organism that is ESBL [extended-spectrum beta-lactamase]-positive may also have implications about the origin of the pathogen (ie, human source more likely than food animal source)," Hedberg said. "Much more will be learned about this as things develop."

Today's Eurosurveillance report gave a long list of continuing investigations into the outbreak, including case-control studies and an inquiry into human-to-human transmission in a "special outbreak in a canteen."

The report also said that among 13 deaths reported in Germany, 8 of the victims were between 75 and 91 years old, while the other 5 were between 22 and 40.

German hospitals battle with flood of E coli patients



Kiel, Germany - 'It's completely new to us, we've never experienced anything like it,' says Michaela Kiemes, ward manager at Kiel University Clinic.

The 44-year-old and her colleagues have been caring for a flood of patients over the past two weeks who have been infected with the enterohaemorrhagic Escherichia coli (EHEC) bacteria, and are experiencing its alarming symptoms first hand.

The cause of the outbreak - which has spread across Europe but is centred in northern Germany - remains unclear, and little research has been done on its treatment.

Doctors and nurses, patients and their relations have all been pushed to their limits.

'We're sleeping badly, and have bad dreams,' Kiemes says quietly. 'We can't get the images of suffering patients out of our heads.'

The neurological breakdowns of patients are particularly concerning. 'When their condition gets worse, then it's dramatic.'

The clinic is currently treating around 180 patients suffering from EHEC, of whom 95 have life-threatening haemolytic-uraemic syndrome (HUS), a form of kidney failure caused by E coli. Around every other HUS patient suffers serious neurological complications.

The patients become disoriented, have difficulty expressing themselves, suffer convulsions. Some become apathetic, others make strange noises. Some get such strong cramp attacks they have to be restrained.

Since the first EHEC patients arrived in the hospital, the wards treating them have been on a 'complete emergency footing,' Kiemes says.

Doctors and co-workers have worked through weekends and days off, delaying their holidays in order to help out. In daily meetings they examine the situation from all angles.

The patients 'spend all their time thinking,' says Keimes. 'They're mentally exhausted because they don't know what EHEC is, where it's come from and what happens next.'

Many fruit and vegetable vendors in Hamburg say that they know where their produce comes from -- their own farms, or farms nearby -- and that this local factor makes their food safe to eat, something they desperately try to convey to shoppers as fruit and vegetable sales plummet.

However, Prüfer-Storcks told Food Safety News she doesn't think people should eat any red-flagged vegetables, even if they were grown locally.

"The protection of human life must be more important than economic interests," Prüfer-Storcks said during a meeting with reporters.

Farmers are not convinced. Their stands are still brimming with cucumbers, tomatoes and lettuce from Germany, Italy and the Netherlands, vegetables they can't sell.

"This is a catastrophe," said one seller at Hamburg's biggest outdoor market, which features only organic produce. The one-kilometer stretch of stands is usually bustling with buyers, residents say, but only a handful of stands had customers on Tuesday.

No vendors are even attempting to sell produce from Spain, whose vegetables have been shunned ever since the finger was first pointed at Spanish cucumbers as a possible cause of the outbreak.

"They were very fast to say, 'Oh, it's from Spain, Malaga,' " said David Salvatierra Martin, a Malagan resident vacationing in Hamburg. "I think they didn't know [where the EHEC came from]." he told Food Safety News.

"There was no proof of this and so we will demand explanations from who has attributed this matter to Spain," said Diego Lopez Garrido, Secretary of State for the European Union.

Many theories have arisen about how vegetables could have become contaminated, wherever they came from. Some think bacteria came from cow manure used as fertilizer. Others say cucumbers were accidentally dropped on the ground during transit.

"Every day, a new theory," said Rico Schmidt of the Hamburg Health Ministry in an interview with Food Safety News. "But everyone is searching for someone so they can say, 'Oh, it was not me,' " he said.

From Monday to Tuesday morning alone, 80 more cases were confirmed to be part of the outbreak, according to Prüfer-Storcks. Hamburg is treating 110 patients critically ill with hemolytic uremic syndrome (HUS), 82 of whom are women. The European Centre for Disease Prevention and Control reported a total of 373 HUS patients, two thirds of them women.

Why so many women are ill remains a puzzle. If the source is fresh produce, were they more likely to have eaten the vegetables or chopped and prepared them?

"We don't know why women, particularly younger women, seem to be most vulnerable," Willich said. "Maybe that has something to do with exposure. Maybe those women just tend to eat more salad than others."
