Introduction: The estimated incidence of disease is essential to assess its impact on the population. The purpose of this study is to estimate the burden of disease caused by influenza virus (H1N1) 2009 in the Mexican Institute of Social Security (IMSS) from April 2009 to January 2010. Methods: To estimate the number of symptomatic cases, the number of hospitalizations and deaths we used the method recommended by the Centers for Disease Control in the U.S. (Center for Disease Control [CDC]), which takes into account the underestimation associated with the use of health services, the practices of confirmation and registration of cases. To estimate the incidence of infection, apply to the population of the IMSS seroincidence by age group in London recently reported. Results: Each confirmed symptomatic case of influenza accounted for an average of 51 cases in the first outbreak and 18 in the second. This translates to 537.167 symptomatic cases (range: 378.439 to 813.008). Each case represented the hospital confirmed to 2.2 people. The estimate of hospitalizations was 10.063 (range 7.441 to 14.610).
The reason for hospitalization among the total number of cases was 1.8%. The incidence of infection was estimated at about 24%. Conclusions: The confirmed cases in the surveillance system are only a small proportion of the population infected and symptomatic cases, which should be taken into account in the future care of the disease
