Location: China - Event: A(H7N9) influenza

On 03rd and 04th April 2013, Chinese health authorities reported a total of 11 human cases (including 4 deaths) of A(H7N9) influenza virus infections in 4 different provinces (cf. Table 1 and map 1).


In Shangai and Anhui province
  • On 1st April 2013, a total of 3 human infections (including 2 deaths) was reported to WHO, in these provinces :
    • 2 males (aged 87 and 27 years-old), with symptoms onset end of February 2013. Both died on 4th and 10th March 2013 respectively.
    • 1 woman, resident in Anhui (province border to Shangai) with symptoms onset on 15/03/2013. She is in severe condition and hospitalised in Jiangsu.
  • There is no epidemiological link between these 3 cases.
  • For only one of the three cases, contacts with dead poultry have been reported (the case from Anhui).
  • The 27 year old man is working as a butcher in a market.
  • In total, 88 case-contacts have been identified. Investigations are ongoing.
Comments

Cases of A (H7N9) infections have already been reported in the literature in birds in Europe and the United States. However, no human cases infected with A (H7N9) have been described to date.

In mainland China, human cases of influenza A (H9N2) infections have been reported: Guangdong Province and Hong Kong in 2009 (cf. eWEB n?85).

To date, the source of contamination or probable exposure is currently unknown.

Currently, no A (H7N9) or A (H9N2) epizooties have been reported in birds or in other animals in this region. However, China is considered by the FAO as an endemic country for influenza A (H5N1).

In China, it is common that A (H5N1) infections in human cases are reported without any outbreaks occurring or reported in the region, reflecting thus, an under-reporting of outbreaks. At this stage, no epidemiological link has been identified between the 11 confirmed cases from the 4 Chinese provinces.

To date, we cannot yet exclude the possibility of a human-to-human transmission.


Province de Jiangsu
  • On 03 April 2013, a total of 5 human infections (including 1 death) was reported to WHO in Jiangsu province (cf. table 1) :
    • 2 males aged 83 and 48 years-old and, 3 women aged 32, 45 and 48 years-old. All are in critical conditions.
    • The 48 years-old male died on 04th April 2013.
    • There is no epidemiological link between these 5 cases. For only one case, contacts with dead poultry have been reported.
    • In total, 167 case-contacts have been identified. Investigations are ongoing.
Province de Zhejiang
  • On 03rd and 4th April 2013, a total of 3 human infections (including 1 death) was reported in Zhejiang province :
    • 1 male aged 38 years-old who died on 27th March 2013. He was a cook in Jiangsu province restaurant.
    • 2 males aged 67 and 64 years-old who were both hospitalized.
    • There is no epidemiological link between these 3 cases.
  • In total, 183 case-contacts have been identified. Investigations are ongoing.
In human, the infection with new variants is rare. These infections can lead to the occurrence of severe sporadic cases (e.g. A (H5N1) infections without sustained human-to-human transmission or moderate/severe infections with human-to-human transmission (e.g. pandemic A (H1N1), 2009).

To date, the available information is insufficient to estimate the impact of this epidemic.

However, the occurrence of these 11 cases over a short period time in four different provinces lead health authorities to follow the situation with the greatest attention.

Several of the onset dates in Table 1 appears to be incorrectly reported as occurring in February.

Several of the onset dates in Table 1 appears to be incorrectly reported as occurring in February.

We should e-mail to Episouth. I am sure they will correct promptly the mistakes.
