Monkeypox: semen testing for viral DNA


Information for clinicians on testing semen for monkeypox DNA.

There is a body of evidence documenting the detection of RNA and DNA viruses in semen after acute infection. Monkeypox DNA has been detected in semen up to day 11 after acute infection in men in Italy, although longitudinal sampling after this point was not carried out.

It remains unclear whether this is from seminal carriage as the testes are immunologically-protected sites (as with other viral infections such as Ebola virus disease), or from resolving meatal or urethral lesions that disappear as the skin lesions resolve.

However, in view of this, current UKHSA advice takes a cautious approach and recommends that men who have been confirmed as a case of monkeypox should use condoms during sex for 12 weeks after complete recovery from monkeypox infection. This is in line with advice from the World Health Organization (WHO).

It should be recognised that there is currently no evidence to support this or any alternative advice.

Following the initial 12 weeks and up to 6 months after recovery from infection, UKHSA therefore recommends performing a monkeypox PCR on semen samples (and where necessary, oropharyngeal and/or rectal swabs) if the patient:
  • is undergoing fertility treatment or planning pregnancy
  • is undergoing planned semen storage (for example prior to chemotherapy)
  • has an immunocompromised sexual partner (including a pregnant partner)
  • is concerned about transmission to sexual partner or partners for any other reason and requests a test from their clinician
Testing can also be offered on patient request following discussion with the local infection/genitourinary team and then with the Rare and Imported Pathogens Laboratory (RIPL).

A single negative PCR after 12 weeks post recovery is considered adequate, with recognition that this is based on expert opinion in the absence of a clear evidence base.

It should also be noted that a positive PCR does not necessarily indicate that the patient is potentially infectious, only that fragements of DNA have been detected. However, since viral culture is not feasible for clearance testing, if the PCR is positive it is prudent to consider the patient potentially infectious.

Repeat samples should be submitted every 2 weeks thereafter, and the patient should continue to use condoms until a negative test result is obtained.

Any samples sent without a minimum serial interval of 2 weeks will not be processed without prior discussion with RIPL.

Any testing that is requested prior to 12 weeks post recovery must first be discussed and agreed with RIPL prior to sending. Samples received without discussion will not be processed. For semen tested prior to 12 weeks post-recovery, 2 samples collected at least 48 hours apart should be tested, and both need to be negative to be deemed clear.

Sample collection must take place in a facility capable of arranging transport of a Category B sample. These are usually hospitals where cases of monkeypox are cared for and not genitourinary medicine (GUM) clinics.

Samples should be submitted in a universal sterile container and sent to:

Samples must be packaged and sent as a Category B infectious substance. Please ensure that samples are properly packaged and sealed. Leaking samples will not be processed. Samples will only be tested during working hours Monday to Friday.

Appropriate counselling from clinicians should include that this is an unvalidated diagnostic test.

It is expected that evidence will emerge for duration of monkeypox DNA detection in semen which will allow this guide to be updated, and earlier testing may be offered if the evidence supports this.

Monkeypox is a zoonotic infection, caused by the monkeypox virus, that occurs mostly in West and Central Africa. Previous cases in the UK had been either imported from countries where monkeypox is endemic or contacts with documented epidemiological links to imported cases.

Between 2018 and 2021, there had been 7 cases of monkeypox in the UK. Of these, 4 were imported, 2 were cases in household contacts, and 1 was a case in a health care worker involved in the care of an imported case. There was no documented community transmission in previous outbreaks.

Current epidemiological situation as of 18 July 2022

Cases of monkeypox infection were confirmed in England from 6 May 2022. The outbreak has mainly been in gay, bisexual, and men who have sex with men without documented history of travel to endemic countries. Further details on the epidemiology are available in the monkeypox technical briefing.

Up to 18 July 2022 there were 2,137 laboratory confirmed cases in the UK. Of these, 51 were in Scotland, 13 were in Northern Ireland, 23 were in Wales and 2,050 were in England.

Please note that today’s increase in case numbers in England since last report (Table 1) includes positive tests with result dates between 12 and 17 July 2022. This is a longer timeframe than is usually reported, so should not be interpreted as a sudden rise in the rate of cases. These were included in externally published surveillance data for the first time on 18 July after an NHS diagnostic laboratory’s monkeypox testing was assured. All individuals received their test results at the time.

A high proportion of England cases were London residents (73%, 1,492 of 2,034 with location information). For confirmed cases in the UK, where gender information was available, 2,004 (99.3%) were male and 14 were female. The median age of confirmed cases in the UK was 37 years (interquartile range 31 to 44).

Since 25 July 2022, the monkeypox case definition recognises those who are orthopox-positive as highly probable cases, and those who test positive on a monkeypox PCR test as confirmed cases. The counts below combine both of these categories.

Up to 25 July 2022, there were 2,367 confirmed and 65 highly probable monkeypox cases in the UK: 2,432 in total. Of these, 59 were in Scotland, 18 were in Northern Ireland, 30 were in Wales and 2,325 were in England.

A high proportion of England cases were London residents (73%, 1,699 of 2,313 with location information). For confirmed and highly probable cases in the UK, where gender information was available, 2,288 (99.3%) were men and 18 were women. The median age of confirmed and highly probable cases in the UK was 37 years (interquartile range 31-43).

Data sources

Monkeypox surveillance data in England is currently compiled daily, based on monkeypox virus test results from the Rare and Imported Pathogens Laboratory (RIPL), which is the UKHSA monkeypox reference laboratory, and other UK laboratories with monkeypox testing assured by RIPL. This is combined with case information recorded by local Health Protection Teams in the UKHSA national case management system.

Counts of confirmed cases in Wales, Northern Ireland, and Scotland are submitted to UKHSA by Public Health Wales, Public Health Agency Northern Ireland, and Public Health Scotland respectively.

This update is currently published on Tuesdays and Fridays, using data extracted by 9.30am on Mondays and Thursdays. As of 21 June 2022, this report replaces the news stories previously published 3 times a week in the UKHSA News Stories page.

These figures are used for official reporting of monkeypox confirmed case counts in the UK.

England counts may differ from those published in the Notifications of infectious diseases (NOIDs) causative agents weekly report. This is due to differences in timings of when the data is compiled, and differences in processing of duplicate data (for example, if a person has tested more than once but insufficient information is provided to identify them).

To expand the UK’s capability to detect monkeypox cases, some NHS laboratories are now testing suspected monkeypox samples with an orthopox polymerase chain reaction (PCR) test (orthopox is the group of viruses that includes monkeypox). Since 25 July 2022, the monkeypox case definition recognises those who are orthopox-positive as highly probable cases, and those who test positive on a monkeypox PCR test as confirmed cases. The counts below combine both of these categories.

Up to 22 August 2022 there were 3,207 confirmed and 133 highly probable monkeypox cases in the UK: 3,340 in total. Of these, 78 were in Scotland, 27 were in Northern Ireland, 44 were in Wales and 3,191 were in England.

A high proportion of England cases were London residents (70%, 2,232 of 3,177 with location information). For confirmed and highly probable cases in the UK, where gender information was available, 3,270 (98.7%) were men and 42 were women. The median age of confirmed and highly probable cases in the UK was 36 years (interquartile range 31 to 44).

Monkeypox case linked with travel to West Africa identified

UKHSA has confirmed that an individual has been diagnosed with monkeypox linked to recent travel to West Africa.

Preliminary genomic sequencing conducted by UKHSA indicates that this case does not have the current outbreak strain circulating in the UK. The individual has been admitted to the High Consequence Infectious Disease (HCID) unit at the Royal Liverpool University Hospital in line with standing advice from the Advisory Committee on Dangerous Pathogens (ACDP).

Contact tracing of close contacts of the individual is underway. So far, no further linked cases have been identified.

Dr Sophia Maki, Incident Director, UKHSA, said:

We are working to contact the individuals who have had close contact with the case prior to confirmation of their infection, to assess them as necessary and provide advice.

UKHSA and the NHS have well established and robust infection control procedures for dealing with cases of imported infectious disease and these will be strictly followed and the risk to the general public is very low.

We remind everyone who is planning to travel to West and Central Africa to be alert for the symptoms of monkeypox and to call 111 if you have symptoms on your return.

The ACDP has previously advised that importations of monkeypox directly from West Africa as well as cases caused by Clade I of the virus should still be classified as HCIDs as we cannot predict their characteristics.
