This report of the serine and metallo-carbapenemases KPC-2 and NDM-1 in the same highly resistant patient isolate is an event that the infectious disease community has been dreading.

As a result of these beta-lactamases, together with the presence of the rmtB 16S RNA methylase, this Klebsiella pneumoniae isolate was multidrug-resistant to most major classes of antibiotics, demonstrating the kind of pathogenic bacteria that are emerging in our antibiotic-rich environments.

The NDM-1 metallo-carbapenemase and the KPC serine carbapenemase have been spreading globally, such that each has been responsible for major outbreaks of 'superbugs' carrying multiple mobile antibiotic-resistance determinants, each in a slightly different geographical niche.

Now that the two enzymes have become associated with the same organism in the same patient, it is likely that this kind of pathogen will continue to spread such that few, if any, antibacterial agents will be able to interrupt their dissemination.
