H5N1 Spread into Two More Nile Delta Governorates

Veterinary Directorate executed Bakulaiobh 9 thousand chickens layer farm Bkivr thanked emerged after the avian influenza virus .. Al-Mukhtar said Abed Assistant Secretary General of the Directorate that Qaliubia governorate veterinary medicine discovered the existence of injuries positive for avian influenza virus

The above translations describe new H5N1 outbreaks in Al Minufiyah and Al Qalyubia governorates (see satellite map of the above outbreaks and all OIE reported outbreaks since July 1, 2007, including first confirmed case (25F) in Beni Suef this season here). These outbreaks are in addition to Nile Delta outbreaks in Ash Sharqiyah and Al Gharbiyah reported earlier this month. The outbreaks included backyard holdings as well as a farm with 9000 layer chickens. Previously, Egypt filed OIE reports of 922 outbreaks (see satellite maps here here here here here), which also detailed an extensive vaccination campaign. The above outbreaks this month developed in spite of the vaccination campaign. The first fatal case of this season was also confirmed this week.

Sequences from human and poultry outbreaks identified a number of regional markers found in isolates from Egypt, Israel, Gaza, and Djibouti. However, recent outbreaks in Europe, as well as an earlier outbreak in Kuwait, were due to the Uva Lake strain, which is clade 2.2, but distinct from the earlier isolates in Egypt.

Sequence data on the recent outbreaks, including the first human case, would be useful.

Having explained Dr. Sabir Abdul Aziz, director general poultry diseases Rafei, the increasing incidence of avian influenza illness during the current week to the owners of poultry farms lax procedures in the immunization against disease, in addition to the growing activity of the virus that causes the disease during the winter.

He stressed that veterinary body organs mandated to follow procedures immunization within poultry farms, with the daily follow-up of the health situation of poultry farms, saying that it had been decided to start procedures for the active investigation of the disease in all governorates in cooperation between the General Authority for Veterinary Services and the Central Laboratory for control of poultry production.

And he mentioned that the medical team is taking samples from the girl mother of the victim and from the present birds in 49 houses in the village and she is not fortified [vaccinated]. And he said that the team found a single house by him dead birds and the Ministry of Health laboratories have showed that the sent samples are positive.

Dramatic Spread of H5N1 in Egypt

Recombinomics Commentary 22:46
December 28, 2007

According to the results of tests conducted veterinary labs in Cairo concerning examination results of samples number of dead birds in three houses La Republic of the Section on the first Mahala it positive and deaths of these birds within minutes up to more than 150 flying due to the illness of avian influenza,

The above translations describe a dramatic rise in reported outbreaks of H5N1. It was found on farms in three governorates as well as nine backyard farms, primarily in the Nile Delta (see satellite map). These increases are in spite of the aggressive vaccination program in Egypt.

The outbreaks help explain the three confirmed H5N1 cases in two days. The WHO situation update for Egypt indicated on of the new cases was in critical condition, while the second case was stable in the ICU. These two cases, coupled with the earlier fatality suggest a more aggressive H5N1 is circulating, as was seen twelve months ago when the first seven confirmed cases died.

Detail on the sequences from these new outbreaks would be useful.

These increases are in spite of the aggressive vaccination program in Egypt.

Why didn't the vaccination program prevent the poultry infections?

Mismatch, failure to vaccinate poultry that caused infections, or ???

It would be good if officials included an analysis of why the vaccination program didn't impact cases that caused infections.

For one thing, I don't think they managed to vaccinate all the poultry in the country. They may have an aggressive vaccination campaign, but it's not comprehensive. The recent news stories about the vaccination program refer to having 85 or 90 million doses (see here and here) -- but if this FAO report is right, there is a standing population of poultry in Egypt at any given time of ca. 218,000,000+ poultry (no one really knows for sure 'cause the number of backyard birds is a guesstimate).

Regarding mismatch -- I dunno. They did import a lot of their poultry vaccines from China. ??

H5N1 in Egypt Spreads to Qena

Recombinomics Commentary 09:03
December 30, 2007

Avian influenza attacked more than 8 Egyptian governorates during the Eid al-Adha as appeared several hotbeds infected with H5N1 in the governorates of Menoufiya, East [Ash Sharqiyah] and West [Al Gharbiyah], Qaliubiya, Beni Suef, Qena, Cairo, was the surprise that these cases emerged in the farm, which claimed officials with the Ministry of Agriculture said that it had no hope of avian influenza in return

The above translation describes the spread of H5N1 in Egypt. Some of these outbreaks may overlap earlier reports, but the list above includes Qena, which extends newly reported outbreaks to the south (see satellite map), where there was a human cluster last season.

The translation above also notes that H5N1 is again appearing on farms, in spite of a major vaccination program. Although farms were affected at the beginning of 2006, the outbreaks at the end of 2006 and beginning of 2007 were largely confined to backyard holdings.

The rapid spread to multiple locations, including farms, raises concerns that the Uva Lake strain is moving into Egypt.

Sequence data on these new outbreaks would be useful
