So how does an identical sequence eliminate infection by the same bird, as WHO suggested?

You are posting NONSENSE.
The silly statement by WHO was silly because the TIME GAP in DISEASE ONSET dates was NINE DAYS! It had NOTHING to do with the sequences.

The issue of common source or H2H is not a new issue. It is not specific to H5N1.

However, it has continued for YEARS (after cluster after cluster has a significant time gap in disease onset dates), because of utter nonsense put out by the WHO (as show by statements made on the Jiangsu cluster where the disease onset dates were separated by 9 days) to an audience that doesn't have a background in science (or in some cases logic), and repeated again and again by its audience (supported by selective copy and pastes).

Common source and H2H are distinugished by disease onset dates PERIOD. No hocus pocus or sequences required.

Despite the stepped onset dates both cases could have a common poultry source. There are many poultry out breaks in that province.

Agreed since the original story was that the father and son were exposed to H5N1 infected chicken at a restaurant on the same day but had disease onset dates that were 9 days apart.

So what role does a sequence have? If the infection was at the restaurant (common source) the sequences would match. If the infection was from son to father (H2H), the sequences would match.

It was OBVIOUS H2H (nonsense statements by the WHO notwithstanding) because of the TIME GAP, and the sequence would match regardless of mechanism of transmission.

Please. There were ZERO confirmed poultry cases in the area, and TWO human cases who were neighbors. There were only 12 cases in the entire country in 3 months (and earlier cases had POSITIVE dead birds)!

B2H is EXTREMELY rare. Two cases in neighbors in 4 days is ABSURD. This is a simple math problem, with a likelihood close to ZERO (and coincidence nonsense used up YEARS ago).

This is well into the OBVIOUS H2H category (to anyone who can read a calendar).

I don't think I can make this any clearer.

If you don't get it by now (after five YEARS), you never well.

As I noted above the cases were initially reported to have come from the Badr Center (District). The saidr reports have the two children from the Kom Hamada Center (District). Each of these districts encompasses a number of villages. I await official confirmation that the two children lived in adjacent house in the same village and interacted with each other.

The report on adjacent houses also implied that the neighbor was discovered during canvassing because of case #61:

the 61 cases and 62 of the bird-flu-affected children, Mohamed El-Shahat Mohamed Hassan Amer, two years, and Jamil Hassan Mohamed Hassan Amer, years and 9 months, with two children and live in adjacent houses and there is no is reason for concern about the spread of the disease in the village,

Reality check.
During the outbreak in Turkey, virtually EVERY confirmed case was part of a cluster. As a result the age and gender of CONFIRMED cases in the WHO updates were WITHHELD, and 9 of the 21 positives degraded and were removed from the 21 confirmed cases.

Official confirmation of a CLUSTER????

Please, there is no evidence of a change in SOPs.

So you are saying that because one report had both coming from one location, while another report had them both coming from a nearby location, you want to make a "hopes and dreams" report and put one neighbor at one location and put the other at another location??????
Isn't that what was tried with the symptoms in another governorate routine?

My point is - if we are not sure which district these children are from, why should we believe a single news article that these the two kids are neighbors. Further officials details would be helpful.

Point taken, regarding the reporting and interpretation of human clusters by WHO (e.g. Turkey and more recently, Pakistan). With no additional cases being reported in Beheira it seems to have stopped at H2H, if it is H2H.

I would say that this discussion astonished me...

The epidemiological situation in Egypt regarding avian-to-human and human-to-human transmission of H5N1 avian influenza virus is far for being elucidated.

WHO seems to be in trouble with confirmation of the most recent cases.

We are awaiting more data and description of infection sources for these pediatric cases.

Interhuman transmission is possible.

Avian to human is probable and environmental contamination should be taken also into account.

Sero-surveillance for antibodies in humans is long overdue not only in Egypt but elsewhere, and despite years of concerns international agencies and researchers failed to conduct a wide collection of blood samples.

People should ask Agencies and experts why this surveillance has not been done until now.

Frankly, I think a debate is always fruitful but with the perspective of leave readers with some clues...

Here is the WHO confirmation that the neighbors were from Kom Hamada (which simply backs up the SAIDR site, which is a government site, so there are no surprises).

New confirmed cases of avian influenza

Case number 63

The Ministry of Health and Population of Egypt reported a new confirmed human case of avian influenza on 6 April 2009. The new case is a six-year-old male from the Shubra El Khema district, Qaliobia governorate. His symptoms began on 22 March and he was hospitalized at Ain Shams University Hospital on 28 March. The child received treatment with Tamiflu on 3 April. Infection with H5N1 avian influenza was confirmed by the Egyptian Central Public Health Laboratory on 5 April.
Investigations into the source of infection indicate a history of close contact with sick poultry prior to becoming ill. The child is in a critical condition and is on a ventilator.
This brings the total number of human confirmed cases of avian influenza in Egypt up to 63 with 23 deaths.

The Ministry of Health and Population of Egypt reported a new confirmed human case of avian influenza on 6 April 2009. The new case is a two-year-old male from the Kom Hamada district, El Behira governorate. His symptoms began on 31 March and he was hospitalized at Damanhour Fever Hospital on 1 April. The child received treatment with Tamiflu on the same day of hospitalization. Infection with H5N1 avian influenza was confirmed by the Egyptian Central Public Health Laboratory on 2 April.
Investigations into the source of infection indicate a history of close contact with dead and sick poultry prior to becoming ill. The child is in a stable condition.

Whether such cases exist still has to be put to the test and will be the focus of a planned Egyptian government study backed by the global health body, said John Jabbour, a Cairo-based emerging diseases specialist at WHO. "This is a concern only, now. It is a question to be asked," Jabbour told Reuters. He said a change in the pattern of human bird flu infections this year in Egypt had raised concerns about the existence of so-called sub clinical cases. The most populous Arab country, which has been hit harder by bird flu than any other country outside of Asia, has seen a surge in infections this year. While the H5N1 virus only rarely infects people, experts fear it could mutate into a form that humans could easily pass to one another, sparking a pandemic that could kill millions. The emergence of symptomless human carriers of the virus would be a worrisome development because it could allow the virus, undetected and untreated, more time to mutate inside the human body, Jabbour said. "If there is any sub clinical case in Egypt, the aim is to treat immediately to stop the reproduction of the virus. Because whether (through) mutation or reassortment, this will lead to the pandemic strain," he said. Of the 11 Egyptians infected with bird flu this year, all but two have been children under age three while adult cases have all but dried up. All have survived. That is starkly different from the same period a year ago, when seven people -- mostly adults and older children -- contracted the virus and three died. 'SOMETHING STRANGE HAPPENING' Jabbour said the rise in infections in children without similar cases among adults had triggered questions as to whether adults were being infected with the virus but not falling ill. "There is something strange happening in Egypt. Why in children now and not in adults?" he said. "We need to see if there are sub clinical cases in the community." Jabbour said there had been no known instances of sub clinical bird flu cases in humans in other countries where the disease was present. Some birds, like ducks, are known to carry and spread the virus without showing symptoms, he said. The Egyptian study, which Jabbour said should begin in the coming months, would test the blood of people who may have been in contact with infected birds but had not become sick. Since 2003 the avian influenza virus has infected more than 400 people in 15 countries and killed 256 of them. It has killed or forced the culling of more than 300 million birds across Asia, the Middle East, Africa and Europe. Avian influenza - situation in Egypt - update 10

The Ministry of Health of Egypt has reported 3 new confirmed human cases of avian influenza.

The first case is a 2 year-old boy from Kom Hamada District, El Behira Governorate. He developed symptoms on 27 March and was admitted to Naaora Fever Hospital on the 30 March where he was started on oseltamivir the same day (30 March). He remains in a stable condition.

The second case is also a 2 year-old boy from the same district and was detected through the investigation around the above-mentioned case. He developed symptoms on 31 March and was admitted to Damanhor Fever Hospital on 1 April where he was started on oseltamivir the same day (1 April ). He remains in a stable condition.

Both boys had contact with sick/dead poultry prior to the illness onset.

Close contacts of both boys have been identified and none has shown symptoms of the infection .

The third case is a 6 year-old boy from Shubra El Khema District, Qaliobia Governorate. He developed symptoms on 22 March and was admitted to Ain Shams University Hospital on the 28 March where he was started on oseltamivir on 3 April.

He was exposed to sick/dead poultry prior to the illness onset. He is in a critical condition.

For all of the three cases reported above, infection with H5N1 avian influenza virus was tested positive by the Egyptian Central Public Health Laboratory and subsequently confirmed by the U.S. Naval Medical Research Unit No. 3 (NAMRU-3).

Of the 63 cases confirmed to date in Egypt, 23 have been fatal.
