Other than fever, I don't see any symptoms mentioned that might indicate what illness this might be. In fact, the lack of mention of respiratory symptoms tends to me to point away from influenza; Egyptian authorities likely know the symptoms of H5N1 and H1N1 by now. The only treatment mentioned to fail was fever reducers; no indication of any Tamiflu or antibiotics.

The timing might be a bit of a clue; everything seems to have happened within the past week, and the time from onset to death seems very short, perhaps just a couple of days. I also don't see any evidence that the children had contact with each other, given thier names and ages. The fact that, despite varying onset dates, they died within hours of each other raises the possibility that they were admitted with different illnesses and something happened this morning in the hospital.

Great find, Sharon. It seems the case originally reported as 11 years old might have been 11 months, as this article refers to all the cases as aged 6 months to a year.

I wish we could get a better translation of that last sentence as to what "Khanaiser" is, what was inflamed (lungs? brain?), and what the reference to wrong treatment is.

Update: After playing around in Google Translator with the Arabic in the last sentence, I think "Khanaiser" should be "pig".

I write to you to update you about the tragic deaths of three children within El Gouna this weekend. We have today received a committee from the Ministry of Health for the red Sea who are investigating the deaths of these children. At this point they have concluded the following from the autopsy carried out on each infant:-
They are dealing with a virus (not bacterial infection)
The children effected were aged 1year and 2months; 9 months and 4 months
The fever attacks the brain
In each case the children were not referred for medical help until the situation was critical.
The siblings of each child have not shown any symptoms.
The children were not related to each other and they have no connections to nurseries in El Gouna or the school.
The virus is not connected to water.
The samples retrieved from each infant are being examined but it will be 48 hours before the medical team can give a definite diagnosis.
There is one known further case in Hurghada ? the child is 5 years old ? has been treated in hospital and is making very good progress.

Obviously this is a very serious situation and this is recognised by all concerned. The Ministry of Health is sending a team from Cairo and they will arrive in El Gouna at approximately 6.30 this evening. In addition a fever specialist will be based in El Gouna hospital 24 hours a day for the foreseeable future. If you have concerns regarding your child?s health please seek medical advice at an early stage and if necessary you will be referred to a specialist.

At this stage the school remains open; examinations for the National children continue as do the Arabic examinations in the International section. All IGCSE; AS and A level examinations will continue as timetabled.

Even more puzzling, the article itself then indicates that while H1N1 is ruled out, meningitis (bacterial or viral?) remains under investigation, and no further mention of the "Alkhanazy" virus is found in the article.

Update: I think "Alkhanazy" is the same Arabic root as "Khanaiser" above, which was "pig", so the title is merely saying that H1N1 has been ruled out.

Inflammation of the brain can be caused by the west nile virus. This virus seems to circulate in several regions in Egypt; children can have this neurological complication, however usually it is elderly people.

Hand-Foot-Mouth disease (enterovirus type 71) can also cause acute neurological complications in small childeren.

We will have to wait untill the results from the labs of the Ministry of Health.

Notably, the mention of brain involvement is not here, and neither is the conclusion that it must be a virus. Prolonged high fever itself can affect the brain, whatever its cause.

My dear friends and patients, I want first to greet u all. As a pediatrician who has been enquiring about facts before I wrote these words, I would like to share with u some facts.
First, the number of cases are 3
Second, they are all within infancy age group
Third, they all shared with the same symptom which is very high fever not responding to normal anti fever measures
Fourth, no clear evidence of the cause is clear till the moment whether it is viral or bacterial or water sourse origin
Fifth, the vector or the way of transmission is still unknown
Sixth, the very quick sudden collapse of the infants is still under investigations
From these points I reach a simple conclusion
1- It should not be related to any vaccination program as the causative reason is not identified
2- No vaccine is recommended to be given immediately but it is wise to follow ur normal vaccination schedule with ur pediatrician
3- It is not wise to spread any rumours as nothing is identified yet
4- Avoiding sending children to the schools or nurseries is not a clear protective measure
5- General hygenic meaures that should be followed by the parents and the caregivers is the best to do in the time being
6- Seeking medical advice as soon as a child or infant suffers form fever is the most important quick measure that can help the child and the whole community
7- It is wise now to wait for the official reports that should be published and declared soon
8- Any cases with fever that are identified in schools or nurseries should be segregated and called for their parents to accompany the child to their pediatrician
9- These cases can happen in any community but due to the fact that our community is a small closed community so the news whether true or fake are transmitted very fast
10- Finally, Maybe the only positive thing about this tragic experience is that this wonderful community has become ONE UNITED HAND against this test that we are facing.

As usually happens in these circumstances, a differential diagnosis is difficult since the lack of some important details about clinical and physical signs from the patients.

For example:

1) presence of cutaneous rashes;
2) petechiae, subcutaneous hemorrhages;
3) urinary burning sensation;
4) diarrhea or watery diarrhea with or without blood;
5) state of consciousness;
6) aspect of the tongue;
7) aspect of the throat;
8) physical chest examination;
9) abdomen;
10) restlessness, agitation;
11) convulsions;
12) altered mental status and / or psychiatric abnormalities;
13) lack of appetite;
14) whole blood sample results;
15) concurrent presence of animal diseases;
16) unexplained illness in other people...
