EVENT DESCRIPTION

From week 1 2021 (ending 10 January 2021) through week 1
2022 (ending 9 January 2022) a total of 3 362 Acute Flaccid
Paralysis (AFP) cases were reported among which, 19 cases
were confirmed to be circulating vaccine-derived poliovirus type
2 (cVDPV2) from four provinces (Sud Ubangi, Mongala, Nord
Ubangi and Maniema) of the Democratic Republic of the Congo.
No death was reported among these cVDPV2 cases. Majority
of the cases have been reported from Maniema province (9
cases, 47.4%) followed by Sud Ubangi with 5 cases (26.3%),
and Mongala with 4 cases (21.0%). An additional 2 environment
samples from Maniema province tested positive for cVDPV2.

During the period, week 1 through week 51 (ending 26 December
2021), a total of 10 937 polio alerts were received, of which
10 921 were investigated (99.8%) including 10 762 that were
investigated within 48 hours (98.5%). Specifically, in week 51, a
total of 152 alerts were received, of which 143 were investigated
within 48 hours (94.1%).

Since 2017, the Democratic Republic of the Congo has reported
a total of 234 (1.5%) cVDPV2 cases of 15 371 Acute Flaccid
Paralysis (AFP) cases reported. Three (3) environmental samples
tested positive for cVDPV2 in the same period. The country
experienced 12 cVDPV2 outbreaks reported in 65 health districts
across 16 provinces.

To date, the cVDPV2 outbreak is still active is the provinces of
Kasai, Sankuru, Haut-Lomami, and Maniema.

PUBLIC HEATH ACTIONS

The national polio integrated technical team has conducted
field supervisions to the affected provinces

The preparations for the 2021 polio response activities
review including polio data management is ongoing

Field teams are conducting active polio case search and
supportive integrated supervisions to the affected health
districts

The preparations for deployment of technical teams (national
consultants) to the affected health districts have started

The polio outbreak risk assessment in Maniema province has
been finalized

SITUATION INTERPRETATION

Although the Democratic Republic of the Congo has been certified
as a “polio-free country” by the WHO since November 2015, the
country remains among high-risk countries for cVDP2 outbreaks.

Effort should be maintained especially for enhanced surveillance
and response to facilitate early detection and stop occurrence of
new outbreaks.

PROPOSED ACTIONS

The required resources should be mobilized to facilitate the
country achieve its national and Global polio eradication
objectives as quick as possible.

Specific response strategies are required for hard-to-reach
areas in order to eliminate the chain of transmission and
avoid new outbreaks.

The province of Maniema is still one of the provinces with the large number of children not or insufficiently vaccinated, according to the investigations carried out in certain health zones, indicated the head of the health division, doctor Anaclet Kyanga.

According to a UNICEF expert, Souka Robert, “since 2022, the country has notified 76 cases of polio, including 61 cases in the province of Maniema. Thus the province of Maniema is the epicenter of the polio epidemic currently in the DRC”.

The head of the health division explained the causes of this situation:

"The poor application of specific strategies to reach all targets, the refusal of vaccination by some parents in the current context of the infodemic around the vaccine".

Thus, Dr Anaclet Kyanga calls on parents to have their children vaccinated.

"To enable us to contribute to the reduction of morbidity attributable to polio and its consequences at all levels, each target child must be vaccinated", he insisted.

All partners reiterated their common commitment to continue supporting the Congolese government for the eradication of poliomyelitis.

This vaccination campaign will be door to door. It will last 3 days.

Four health zones out of the sixteen in the province of Haut-Lomami are affected by several cases of wild poliovirus. "There is in particular the health zone of Mukanga with three cases precisely in the health zone of Kasenga I and Kasenga II, there is the health zone of Kinkondja with a single case, precisely this is the area of health in Kipamba. The Kitenge health zone has one case in the Kileo health area and finally the Malemba Nkulu health zone which has five cases. We dispatched a team from the DPS (Provincial Health Directorate) to the intermediate level for the investigation. They took the samples that the province sent to the Lubumbashi laboratory. The diagnosis was confirmed ,” he explained.

The provincial Minister of Health announces actions to be taken to stop the circulation of the poliomyelitis virus:

“Compared to the actions to be carried out, it is the organization of the response, the sensitization of the community with the systematic search for zero-dose children. This will allow the recovery of all those children who escape routine vaccination”.

EVENT DESCRIPTION

In 2022, from week 1 through 28 October, a total of 194 confirmed
circulating vaccine-derived poliovirus (cVDPV) cases have been
reported in the Democratic Republic of the Congo including 36
type 1 cases (cVDPV1) and 158 type 2 cases (cVDPV2). No death
has been reported so far among confirmed cases, however three
suspected polio deaths have been reported in Haut-Lomami
province, two deaths in week 35 (ending 4 September) from
Mulongo health zone and one death in week 39 (ending 2 October)
from Malemba Nkulu health zone. Unfortunately, no sample was
collected for laboratory confirmation from the suspected cases
before death.


Out of the 26 provinces for the country, nine (Bas-Uele, HautLomami,
Haut-Katanga, Lualaba Nord Kivu, Maniema, Sud-Kivu,
Tanganyika, and Tshopo and) are affected with the ongoing
vaccine derived polio outbreaks with Tanganyika province having
the highest case load reported (75 cases), followed by Maniema
(63 cases) and Haut-Lomami provinces (38 cases).

The cVDPV1 cases have been reported from two provinces so
far: Haut-Lomami province (91.6%, 33/36 cases) and Tanganyika
(8.3%, 3/36 cases) whereas cVDPV2 cases have been reported
from all the nine affected provinces with Tanganyika and Maniema
leading in number of cases reported with 72 cases and 63
cases respectively. Three environmental samples were positive
for cVDPV2, one from Haut-Katanga and two from Maniema
provinces.

Regarding surveillance performances, since the beginning of
this year, more than 3 221 acute flaccid paralysis (AFP) cases
have been reported countrywide of which more than 194 cases
were confirmed to be vaccine-derived poliovirus (VDPV) cases
(6.0%) with an estimated no polio AFP rate of 6.6/100 000
populations of less than 15 years old (country target: ≥3/100
000); the proportion ofstool adequacy is 87% (target: ≥ 80%).
The investigation rate within 48 hours of AFP notification is 96%
(target: ≥80%) and the average time of the sample transportation
is 13 days (target: ≤3 days).

Since 2017 through 28 October 2022, the Democratic Republic
of the Congo has reported a total of 437 cVDPV2 cases (36 type
1 and 401 type 2) out of 18 668 Acute Flaccid Paralysis (AFP)
cases reported. Seven environmental samples tested positive
for cVDPV2 for the same period. The country experienced 16
outbreaks of cVDPV2 reported in 108 health zones across 22
provinces. The year, 2022 has the most cVDPV cases reported
since 2017 with 194 cases (44.4%).

The country has not reported wild polio virus case for a while and
the paralysis onset date for the last wild polio virus (WPV) case
reported in the country is 20 December 2011. The Democratic
Republic of the Congo has been certified as a “wild polio-free
country” by the WHO since November 2015.

PUBLIC HEALTH ACTIONS

The preparation of deployment of technical teams (national
and international consultants) to the affected health districts
is underway

Active polio case search, case investigation and supportive
integrated supervisions to the affected health zones are
ongoing

Partner coordination including, mapping of WHO and other
partners technical support to the country during quarter 4
,2022 is planned

A meeting to discuss the classification of the inadequate AFP
cases by the experts committee was conducted

A mass polio vaccination campaign targeting 11 provinces is
planned from 10 to 12 November 2022.

SITUATION INTERPRETATION

The Democratic Republic of the Congo is facing an upsurge in
cases of type 1 and 2 vaccine-derived polioviruses with Haut Lomani
province as the epicenter of cVDPV1.
This Province
recorded its second and last polio outbreak in October 2019.
Reinforcement of routine polio vaccination activities is required
mostly in the affected health districts to slow down the outbreaks
and implement as quickly as possible the mass polio vaccination
campaign. Furthermore, required resources should be mobilized
to achieve the national and Global polio eradication targets.
