Kinshasa – Mpox continues to put pressure on the health system in the Democratic Republic of Congo (DRC). Between January 1 and May 31, 2025, the country recorded 12,208 suspected cases and 22 deaths, representing more than 50% of the cases reported this year in Africa.

In response, the World Health Organization (WHO) and the Global Outbreak Alert and Response Network (GOARN) are supporting the country's efforts to strengthen clinical management, improve detection and prevention, and strengthen the skills and commitment of frontline responders.

GOARN, a WHO-coordinated network that helps countries respond to health emergencies by deploying personnel and resources, mobilized seven specialists to support the mpox
response in the DRC. Working both in the field and remotely from Nairobi, the team provided expertise in treatment, data analysis, epidemiology, and disease surveillance.

Among them was Dr. André Basilua Muzembo, a case management specialist at the University of Hyogo in Japan. For nearly two months, he collaborated with national health professionals and WHO teams responsible for logistics, infection prevention, vaccination, and the mpox response to ensure effective coordination.

At the Clinique Kinoise, the main referral center for severe cases of mpox in Kinshasa, he provided support to address critical challenges. The facility, with 42 beds and an average of 20 patients, had only about five of its 95 professionals trained in mpox management. Chronic shortages of gloves, medications, and essential equipment, combined with limited access to laboratory tests and difficult working conditions, contributed to a mortality rate approaching 10%.

During approximately 30 visits to five treatment centers in Kinshasa, primarily the Clinique Kinoise, Dr. Muzembo trained staff in compassionate care, worked with colleagues to improve hygiene practices, and helped coordinate deliveries of medications and medical supplies. He also collaborated closely with the centers in Kokolo, Vijana, Masina Cinquantenaire, and Kinkole to facilitate patient transfers and ensure continuity of care.

The response team paid particular attention to high-risk groups, including children, pregnant women, and people living with HIV. Some pregnant patients arrived with fetal deaths, partly due to difficulties accessing essential diagnostic tools such as ultrasound. In partnership with the National Multisectoral AIDS Control Program (PNMLS) and Médecins Sans Frontières (MSF), at least five HIV-coinfected patients were able to receive appropriate care in specialized facilities.

When two cases of MPOX were detected in an orphanage in Kinshasa, emergency vaccination protected 30 at-risk individuals. Alerts about infected prisoners at Kokolo Hospital, followed by more than 30 cases reported at Ndolo Prison, highlighted the need for sustained outreach in high-risk settings.

As part of efforts to strengthen the health system beyond the immediate response, 59 frontline workers, including 27 doctors and 32 nurses from the Kinoise Clinic, were trained in clinical management of mpox.

Severe flooding then hampered the outbreak response, requiring more coordinated, multisectoral efforts. Response teams had to address multiple emergencies simultaneously—copoeia and cholera—while supporting displaced populations. At four evacuation sites, including Tata Raphaël Stadium and Bandalungwa, WHO and partners distributed essential supplies such as medicines, cholera kits, and hygiene items.

“Despite extremely difficult working conditions, I saw how important it was to work to save lives with determination, compassion, and teamwork. Resilience is not just about managing adversity, but about experiencing it with those affected, listening to those on the verge of despair, and doing what we can with whatever means we have,” says Dr. Muzembo.

These deployments were made possible with support from the UK Public Health Rapid Support Team, the Public Health Agency of Canada, the Research Institute of Nursing Care for People and Community, the University of Hyogo, and the European Centre for Disease Prevention and Control.
